TIGIT is mainly expressed on T cells and is an inhibitory checkpoint receptor that binds to its ligand PVR in the tumor microenvironment. Anti-TIGIT monoclonal antibodies (mAbs) such as Ociperlimab and Tiragolumab block the TIGIT-PVR interaction and are in clinical development. However, the molecular blockade mechanism of these mAbs remains elusive. Here, we report the crystal structures of TIGIT in complex with Ociperlimab_Fab and Tiragolumab_Fab revealing that both mAbs bind TIGIT with a large steric clash with PVR. Furthermore, several critical epitopic residues are identified. Interestingly, the binding affinity of Ociperlimab toward TIGIT increases approximately 17-fold when lowering the pH from 7.4 to 6.0. Our structure shows a strong electrostatic interaction between ASP103_HCDR3 and HIS76_TIGIT explaining the pH-responsive mechanism of Ociperlimab. In contrast, Tiragolumab does not show an acidic pH-dependent binding enhancement. Our results provide valuable information that could help to improve the efficacy of therapeutic antibodies for cancer treatment.

BackgroundOciperlimab, a novel, humanized monoclonal antibody (mAb), binds to T-cell immunoreceptor with immunoglobulin and immunoreceptor tyrosine-based inhibitory motif domains (TIGIT) with high affinity and specificity. Tislelizumab is an anti-programmed cell death protein 1 mAb. We report results from a phase I, first-in-human, dose escalation study evaluating the safety, pharmacokinetics (PK), and preliminary antitumor activity of ociperlimab plus tislelizumab in patients with advanced solid tumors.MethodsEligible patients previously treated with standard systemic therapy, or for whom treatment was not available or tolerated, received ociperlimab intravenously on Cycle (C) 1 Day (D) 1 and tislelizumab 200 mg intravenously on C1 D8. If tolerated, patients received ociperlimab plus tislelizumab 200 mg sequentially on D29 and every 3 weeks (Q3W) thereafter until discontinuation. Dose escalation for ociperlimab was planned with four dose levels (50 mg, 150 mg, 450 mg, and 900 mg) according to a 3+3 design. An additional dose level of ociperlimab 1800 mg was also assessed. Primary endpoints were safety, determination of the maximum tolerated (or administered) dose, and the recommended phase II dose (RP2D). Secondary endpoints included overall response rate (ORR), duration of response (DoR), disease control rate (DCR) (Response Evaluation Criteria in Solid Tumors version 1.1), PK, and biomarker analysis.ResultsAt data cut-off (September 29, 2022), 32 patients had received ≥1 dose of ociperlimab plus tislelizumab 200 mg Q3W. The maximum administered dose was ociperlimab 1800 mg plus tislelizumab 200 mg Q3W. The median age of enrolled patients was 59.5 years (range: 31–79). Most patients (96.9%) experienced ≥1 treatment-emergent adverse event (TEAE); 62.5% of patients experienced ≥grade 3 TEAEs and 50.0% of patients experienced serious TEAEs. No dose limiting toxicity events were reported. The maximum tolerated dose was not reached. The RP2D was ociperlimab 900 mg plus tislelizumab 200 mg Q3W. Overall, ORR was 10.0%, median DoR was 3.6 months, and DCR was 50.0%.ConclusionsOciperlimab plus tislelizumab was well tolerated in patients with advanced solid tumors, and preliminary antitumor activity was observed with 450 mg, 900 mg, and 1800 mg ociperlimab. Phase II/III trials of ociperlimab 900 mg plus tislelizumab 200 mg Q3W are underway in a range of solid tumors.Trial registration numberNCT04047862.

Study on the Effect of EZH2 Inhibitor Combined with TIGIT Monoclonal Antibody against Multiple Myeloma Cells.

EZH2, a member of the polycomb repressive complex 2, induces trimethylation of the downstream gene at the histone three lysine 27 (H3K27me3) position to inhibit tumor cell proliferation. Here, we showed that the apoptosis rate and apoptotic protein expression increased after EZH2 inhibition, whereas key molecules of the NF-κB signaling pathway and the downstream target genes were inhibited. Additionally, the expression of CD155, a TIGIT high-affinity ligand in multiple myeloma (MM) cells, was decreased by the mTOR signaling pathway. Furthermore, the combination of EZH2 inhibitor and TIGIT monoclonal antibody blockade enhanced the anti-tumor effect of natural killer cells. In summary, the EZH2 inhibitor not only plays an anti-tumor role as an epigenetic drug, but also enhances the anti-tumor effect of the TIGIT monoclonal antibody by affecting the TIGIT-CD155 axis between NK cells and MM cells, thus providing new ideas and theoretical basis for the treatment of MM patients.

Arcus, Gilead's TIGIT combo shows survival benefit in scrapped lung cancer study

In a landscape marred by high-profile setbacks for TIGIT inhibitors, Arcus Biosciences has emerged with a potential bright spot. The biotech is reporting positive overall survival (OS) and progression-free survival (PFS) data from a now-discontinued study evaluating domvanalimab, an Fc-silent anti-TIGIT monoclonal antibody partnered with Gilead Sciences, in combination with the PD-1 inhibitor zimberelimab.Results from the ARC-10 study, shared at the Society for Immunotherapy of Cancer (SITC) annual meeting this week, come as the TIGIT inhibitor class struggles to deliver on its promise. Fizzling enthusiasm for the approach follows clinical failures for Roche's tiragolumab, while Merck & Co. has scrapped studies of vibostolimab in both lung cancer and melanoma. The first part of the trial, whose results were reported at the SITC meeting, evaluated domvanalimab plus zimberelimab, zimberelimab alone, or platinum doublet chemotherapy in countries where anti-PD-(L)1 monotherapy was not yet standard of care.Arcus and Gilead decided earlier this year to discontinue enrollment in ARC-10, citing a "strategic prioritisation" in favour of advancing two other late-stage trials, STAR-121 and STAR-221, in lung and gastrointestinal cancers, respectively.The data presented at the SITC conference focused on patients with advanced or metastatic NSCLC who had a high level of the PD-L1 marker (TPS ≥50%) and no genetic mutations that could be treated with targeted therapies. As of the May 17 data cutoff, 95 participants had received treatment in the first part of the study.OS benefit of 36%According to Arcus, pairing domvanalimab with zimberelimab led to a 36% reduction in the risk of death compared to zimberelimab alone. Median OS for the combination arm was not reached, while the zimberelimab and chemotherapy arms reached median OS of 24.4 months and 11.9 months. respectively. PFS was also improved with the combo, at 11.5 months versus 6.2 months for zimberelimab and 9.6 months for chemotherapy."These are the first results demonstrating an improvement in overall survival reported for domvanalimab and zimberelimab," said Dimitry Nuyten, chief medical officer of Arcus. "They add to the growing body of evidence that domvanalimab…may have a differentiated efficacy, safety and tolerability profile relative to published data from studies with Fc-enabled anti-TIGIT antibodies."Regarding safety, the combination of domvanalimab plus zimberelimab was also generally well tolerated, with lower rates of treatment-related adverse events (TRAEs) leading to discontinuation compared to chemotherapy. Patients were more likely to stop treatment due to side effects with chemotherapy (23.5%) than with the combination (10.5%) or zimberelimab alone (7.5%).Grade ≥3 TRAEs were also more common with chemotherapy (47.1%) than with the combination (21.1%) or zimberelimab monotherapy (15%). Deaths related to treatment were lower in the combo arm (2.6%, one case of sudden death) compared to zimberelimab (10%, various causes) and chemotherapy (11.8%, various causes).
