Delving into the Latest Updates on Polidocanol with Synapse

This study compares two minimally invasive treatments for varicose veins in the legs, specifically the great saphenous vein.
Varicose veins can cause symptoms such as leg pain, swelling, and skin changes. One commonly used treatment is foam sclerotherapy, where a special foam is injected into the vein to close it. This method is simple and allows patients to return quickly to their daily activities, but sometimes the vein can reopen, requiring additional treatments.
Another option is mechanochemical ablation, which combines a small device with a medication to close the vein. This method also allows for quick recovery and may have a lower chance of the vein reopening.
In this study, 50 patients with varicose veins will be randomly assigned to receive one of these two treatments. Doctors will follow patients over time to evaluate:
How well the treated vein stays closed Improvement in quality of life Pain during the procedure Possible complications All procedures are performed with local anesthesia, and additional small varicose veins are treated at the same time.

This randomized, parallel clinical trial with blinded outcome assessment will compare the clinical and aesthetic outcomes of polidocanol foam sclerotherapy alone versus polidocanol foam associated with long-pulse 1064 nm Nd:YAG transdermal laser in the treatment of lower-limb tributary varicose veins. The primary endpoint is venous occlusion rate at 30 days, assessed by Doppler ultrasound. Secondary outcomes include cutaneous hyperpigmentation, pain, patient satisfaction, and adverse events.

Polidocanol augments high-intensity focused ultrasound efficacy for hyperintense fibroids on T2WI: a retrospective cohort study

OBJECTIVE:

To compare the immediate efficacy and safety of high-intensity focused ultrasound (HIFU) monotherapy with the combination of ultrasound-guided intratumoral polidocanol injection and HIFU for the treatment of uterine fibroids with high T2-weighted imaging (T2WI) signal intensity.

METHODS:

In this retrospective study, 116 patients with T2-hyperintense uterine fibroids treated between January 2024 and May 2025 were enrolled. Patients were allocated to either HIFU monotherapy (the H group, n = 63) or combined polidocanol and HIFU therapy (the H + P group, n = 53). Thereafter, therapeutic parameters, the non-perfused volume ratio (NPVR), and adverse events were compared between the two groups after propensity score matching.

RESULTS:

The matched cohort comprised 53 patients per group. The median NPVR was significantly higher in the H + P group than in the H group (92.57% vs. 77.81%, p < 0.001). Additionally, a significantly greater proportion of patients in the H + P group achieved an NPVR ≥ 80% (87% vs. 43%, p < 0.001) and an NPVR ≥ 90% (60% vs. 9%, p < 0.001). The H + P group also required significantly less sonication time and lower total energy delivery (both p < 0.05). Furthermore, the overall incidence of adverse events was similar between the two groups (73.6% vs. 69.8%, p = 0.506). However, the H + P group exhibited a markedly lower incidence of sacrococcygeal pain (5.7% vs. 45.3%, p < 0.001).

CONCLUSION:

Pretreatment with ultrasound-guided intratumoral polidocanol injection may improve the immediate ablation efficacy of HIFU in T2-hyperintense uterine fibroids, reducing energy requirements and mitigating procedure-related sacrococcygeal pain. These findings highlight the potential of this combined therapy and underscore the need for prospective studies to confirm its long-term clinical benefits.

Modified ultrasound-guided polidocanol sclerotherapy is an effective treatment for thyroid cystic-solid nodules: A single-center retrospective study

OBJECTIVES:

To evaluate the efficacy and safety of a modified ultrasound-guided polidocanol sclerotherapy protocol in treating benign cystic-solid thyroid nodules, and to provide a reference for clinical treatment.

METHODS:

This retrospective study involved 125 patients with 126 pathologically confirmed benign cystic-solid thyroid nodules who underwent ultrasound-guided polidocanol sclerotherapy at our institution between January 2021 and December 2023. Before administering the sclerosant injection, cystic components were aspirated and irrigated with saline, and the nodules were classified on the basis of the proportion of cystic components and baseline volume, as well as patient sex and age and the color and consistency of the cystic fluid. Nodule volume was measured using ultrasonography before treatment and at 1, 3, 6, and 12 months after treatment. The volume reduction rate (VRR) at 12 months was calculated, and treatment efficacy and adverse events were compared among different nodule subgroups.

RESULTS:

Among the 126 nodules in 125 patients, 115 nodules showed an effective outcome, corresponding to an overall effectiveness rate of 91.3% (115/126). The effective rates for nodules with cystic components ≤70%, 70%-90%, and > 90% were 80.0% (28/35), 94.2% (49/52), and 97.4% (38/39), respectively, and the differences among the three groups were statistically significant (P < 0.05). Overall efficacy did not differ significantly in relation to cyst fluid color, cyst fluid consistency, baseline nodule volume, patient age, or sex. Fourteen patients (11.2%) experienced minor adverse reactions, including mild local pain (n = 9), transient low-grade fever with nausea and vomiting (n = 2), intermittent swallowing discomfort (n = 2), and postoperative hemorrhage that was controllable by local compression (n = 1). All adverse reactions resolved spontaneously or with conservative management. The patients showed no significant complications, such as infection, severe pain, voice alterations, esophageal or tracheal injury, or sclerosant extravasation.

CONCLUSIONS:

Modified ultrasound-guided polidocanol sclerotherapy for benign cystic-solid thyroid nodules is safe and effective, and the treatment efficacy is positively associated with the proportion of cystic components. This technique may represent a promising alternative to ethanol injection for the management of cystic-solid thyroid nodules.

The use of fluoroscopy-guided sclerotherapy with foam polidocanol for retromolar trigon venous malformation: A case report and review of the literature

Venous malformations (VMs) are vascular anomalies that can pose significant challenges in pediatric patients. This case report describes the successful treatment of a retromolar trigone VM in an eight-year-old child using a single injection of 3 % polidocanol foam using the Tessari technique and administered under fluoroscopic guidance. This resulted in significant reduction of the lesion and complete resolution of symptoms without complications. Follow-up evaluations were scheduled at two weeks, one month, three months, six months, and twelve months post-procedure. At six months post-procedure, there was no detectable lesion, though a minimal residual bluish discoloration persisted on the lingual surface. At twelve months, this discoloration remained unchanged, with no signs of recurrence. This case highlights the efficacy and safety of polidocanol foam sclerotherapy as a minimally invasive treatment option for pediatric maxillofacial VMs. Further studies are warranted to evaluate the long-term outcomes and broader applicability of this technique.
