Targeting IL-7Rα with PNU-159682 antibody–drug conjugates in acute lymphoblastic leukemia: translational implications

Relapsed acute lymphoblastic leukemia (ALL), particularly with central nervous system (CNS) involvement, remains a major cause of treatment failure and is inadequately controlled by existing antibody-drug conjugates (ADCs) with tubulin inhibitors. To address this limitation, we developed IL-7Rα-targeted monoclonal antibodies and identified clone 577 as the lead candidate. Using this antibody, we generated ADCs conjugated with either monomethyl auristatin E (MMAE) or the highly potent DNA-damaging payload-PNU-159682 (PNU). In head-to-head comparisons, 577-PNU showed >50-fold greater potency than 577-MMAE in vitro and induced complete tumor regression in xenografts at a 20-fold lower dose. Additionally, 577-PNU provided durable systemic disease control and markedly reduced leukemic infiltration in the brain and spinal cord in both preventive and established murine CNS disease models, offering direct evidence of effective CNS penetration. Safety assessments demonstrated stable body weight, normal hematology and serum biochemistry, and no treatment-related pathologies. Collectively, these findings provide the first preclinical evidence that IL-7Rα-directed ADCs armed with DNA-targeting payload PNU-159682 can achieve durable elimination of systemic and CNS leukemia at tolerable doses, demonstrating both clinical feasibility and CNS disease control, and establishing a compelling rationale for their translational and clinical development in relapsed and refractory ALL.

Abstract:

Effective treatment for metastatic cancer has remained elusive because of the persistence of drug-resistant metastasis stem cells (MetSC) that drive relapse. MetSCs are tumor cell subpopulations enriched for their ability to reinitiate and sustain metastatic growth, displaying phenotypic plasticity and resistance to chemotherapy. These cells express the L1 cell adhesion molecule (L1CAM), a transmembrane protein detected in numerous human solid tumor types and at multiple disseminated organ sites. As a selective surface marker of MetSCs, L1CAM is a promising candidate for molecularly targeted drugs aimed at eliminating metastases, yet strategies to date have not achieved clinical success. In this study, we develop antibody–drug conjugates (ADC) to deliver highly toxic PNU-159682 payloads to L1CAM-expressing cells. We report the generation of monoclonal antibodies (mAb) with high binding affinity, specificity, and selectivity for the human L1CAM extracellular domain. Optimized L1CAM-targeting mAbs were conjugated to PNU-159682 to generate ADC variants with both cleavable and noncleavable linkers, with an average drug–antibody ratio of four. ADCs derived from three antibodies targeting various epitopes of the L1CAM extracellular portion potently killed cells exhibiting varying levels of surface L1CAM expression. L1CAM ADCs given as monotherapy resulted in robust tumor control and extended survival in mice harboring subcutaneous L1CAM+ xenografts or L1CAM+ lung metastases from triple-negative basal breast cancer and lung adenocarcinoma. Safety analyses with mouse cross-reactive antibodies indicate a feasible therapeutic window. Our findings offer strong proof of concept to support the preclinical development of these novel L1CAM ADCs as therapeutic agents for advanced solid tumors.

CDCP1-targeting ADC outperforms standard therapies in Ras-mutant pancreatic cancer

R AS mutations are found in 10%-30% of various cancers and in up to 90% of pancreatic cancers, where they are associated with aggressive phenotypes, poor prognosis, and reduced overall survival. CUB domain containing protein 1 (CDCP1), a transcriptional target of activated RAS, is implicated in these cancers irrespective of the specific R AS mutation. Given the limited effectiveness of small-molecule inhibitors against mutant Ras-driven cancers, we developed a CDCP1-targeting antibody-drug conjugate (ADC). In this study, we demonstrate that CDCP1 overexpression significantly correlates with R AS mutations in pancreatic cancer. We generated and characterized a CDCP1-specific monoclonal antibody, 2G10, and conjugated it to the topoisomerase II inhibitor, PNU159682, to produce 2G10-PNU159682. The anti-tumor activity of this ADC was evaluated in vitro and in vivo using pancreatic cancer cell lines. 2G10-PNU159682 exhibited superior efficacy compared to MRTX1133 and sotorasib in G12D- and G12C-mutant cell lines. In a mouse xenograft model, 2G10-PNU159682 demonstrated robust anti-tumor activity against R AS-mutant pancreatic cancers, outperforming gemcitabine and FOLFIRINOX and achieving complete tumor remission for up to 100 days-even following relapse after standard chemotherapy. These findings support the potential of 2G10-PNU159682 as a promising therapeutic candidate for the treatment of Ras-mutant cancers.
