This randomized, single blinded, clinical trial aims to investigate the efficacy of a multimodal pain control regimen for shoulder arthroplasty. Patients who receive a multimodal pain control regimen alone (study group) will be compared to patients who receive a multimodal pain control regimen plus a standard prescription of an opioid containing medication (comparison group). The primary outcome is average daily Numerical Rating Scale (NRS) pain score in the first 10 days after surgery. We hypothesize that there will be no significant difference in the primary outcome between the two groups.

First full-scale application of barium carbonate as an effective dispersed alkaline substrate for sulfate removal from acid mine drainage

A full-scale passive treatment plant using barium carbonate (BaCO₃), the mineral witherite, as a Dispersed Alkaline Substrate (DAS) for strongly contaminated acid mine drainage was implemented for the first time globally at Mina Concepción, located in the Iberian Pyrite Belt (SW Spain). The plant was monitored over a 105-day period, covering the wettest months of the hydrological year and with a mean flow around 4 L s^-1. The AMD treated in the plant exhibits a high strength, with an average sulfate concentration around 1500 mg L^-1 and net acidity close to 1000 mg L^-1 as CaCO₃ equivalent. According to the above, the loading rate for SO₄ and Fe was around 400 and 80 kg day^-1, respectively. The treatment process produced an alkaline effluent with low metal content. Nearly complete removal of most metal(loid)s was achieved, with significant sulfate decrease to below 500 mg L^-1 in the alkaline outflow of the barium carbonate tank. Barium carbonate demonstrated superior performance compared to magnesia, particularly in enhancing alkalinity and lowering net acidity and concentrations of sulfate and manganese. The high efficiency attained by the plant after the barium carbonate treatment is evidenced by compliance with environmental water quality standards for most contaminants.

Risk Factors Analysis for Hypermagnesemia with Magnesium Oxide

While decreased renal function is a known risk factor for hypermagnesemia caused by magnesium oxide (MgO), few studies have comprehensively investigated other contributing factors. In this study, the researchers analyzed the risk factors for hypermagnesemia development in 256 inpatients receiving MgO treatment at the Matsuyama Shimin Hospital. Multivariate analysis identified blood urea nitrogen ≧22 mg/dL, estimated glomerular filtration rate ≦43.1 mL/min, and MgO ≧1000 mg/d as risk factors. Additionally, the researchers' findings suggest a correlation between the number of risk factors and the incidence of hypermagnesemia, including the prevalence of Grade 3 cases. Interestingly, low body mass index emerged as a potential risk factor even in patients without the three identified factors. These findings highlight the importance for pharmacists to advocate for routine serum Mg level monitoring in patients with the risk factors identified in this study.

A case of acute lithium poisoning and hypermagnesemia involving advanced colon cancer-induced colonic obstruction

AbstractAn 83-year-old woman presented with disturbance of consciousness and hand tremor. She had taken lithium carbonate 300 mg/day for bipolar disorder and magnesium oxide 660 mg/day for constipation. Blood tests revealed lithium poisoning, hypermagnesemia and acute kidney injury. Computed tomography showed colonic obstruction caused by cancer of the descending colon. In the outpatient section, her blood pressure decreased to 89/54 mmHg, and her heart rate dropped to 40 bpm. We considered that the obstructive ileus induced intravascular dehydration, which led to toxic serum concentrations of lithium and magnesium, triggering the emergence of severe arrythmia induced by sinus dysfunction. The patient was treated with fluid resuscitation and hemodialysis, followed by endoscopic stent replacement for the descending colon cancer obstruction. These treatments improved her general condition and alleviated the lithium poisoning, hypermagnesemia and colonic obstruction. Such a case is considered extremely rare.

Guidelines Developed for Treating Chronic Idiopathic Constipation

THURSDAY, May 25, 2023 -- In a clinical practice guideline issued by the American Gastroenterological Association and the American College of Gastroenterology, and published online May 19 in the American Journal of Gastroenterology and Gastroenterology , recommendations are presented for the pharmacologic treatment of chronic idiopathic constipation (CIC) in adults. Lin Chang, M.D., from the David Geffen School of Medicine at the University of California in Los Angeles, and colleagues conducted systematic reviews relating to use of fiber, osmotic laxatives, stimulant laxatives, secretagogues, and serotonin type 4 agonists to provide evidence-based practice recommendations for treatment of CIC in adults. The multidisciplinary guideline panel agreed on 10 recommendations for pharmacological management of CIC. These included strong recommendations for use of polyethylene glycol, sodium picosulfate, linaclotide, plecanatide, and prucalopride, based on moderate certainty of evidence. Conditional recommendations, based on low or very low certainty of evidence, were made for fiber supplementation, lactulose, senna, magnesium oxide, and lubiprostone. "The guidelines are meant to provide a framework for approaching the management of CIC," the authors write. "Clinical providers should engage in shared decision-making based on patient preferences as well as medication cost and availability."
