Evidence-based recommendations for the treatment of recurrent aphthous stomatitis: insights from an umbrella review

OBJECTIVE:

To compare the effectiveness and safety of pharmacological, physical, and complementary interventions for recurrent aphthous stomatitis (RAS) across clinically relevant outcomes.

METHODS:

This umbrella review was conducted according to PRISMA and Cochrane guidance and registered in PROSPERO (CRD42024594292). PubMed, Scopus, and the Cochrane Library were searched through August 2025. Eligible studies were systematic reviews, meta-analyses, or network meta-analyses evaluating treatments for RAS. Methodological quality was assessed using AMSTAR 2, and overlap of primary studies was quantified using the corrected covered area.

RESULTS:

A total of 41 reviews were included. Topical corticosteroids and low-level laser therapy consistently reduced pain and shortened healing time, although evidence for recurrence prevention was limited. Hyaluronic acid and herbal agents demonstrated favorable short-term efficacy with good safety profiles. Systemic agents such as colchicine and thalidomide showed benefit in severe or refractory RAS, but were constrained by the adverse effects and low-certainty evidence.

CONCLUSION:

Evidence supports topical corticosteroids, hyaluronic acid, and laser therapy for short-term symptom control in RAS, while systemic agents should be reserved for selected refractory cases.

Midface volumization and skin quality enhancement with a hyaluronic acid–polynucleotide hybrid filler: a 24-week prospective open-label study

BACKGROUND:

Facial aging is characterized by midface volume loss from structural and dermal atrophy.

OBJECTIVES:

To evaluate the efficacy and safety of a hyaluronic acid-polynucleotide (HA-PN) hybrid filler for temporary restoration of anteromedial cheek volume.

METHODS:

In this single-center, open-label trial, 15 adults with moderate-to-severe anteromedial cheek volume loss (Mid-Face Volume Deficit Scale [MFVDS] score ≥ 3) received up to 1 mL HA-PN hybrid filler per side. Evaluations at baseline and weeks 4, 12, and 24 comprised blinded photographic MFVDS rating (primary endpoint: ≥1-grade improvement at week 4), GAIS (Global Aesthetic Improvement Scale) score, participant satisfaction, transepidermal water loss, skin hydration, elasticity, and adverse events.

RESULTS:

All participants completed the study without serious adverse events. MFVDS scores improved significantly at week 4 and remained improved through weeks 12 and 24. GAIS and satisfaction scores paralleled these gains. Transepidermal water loss decreased and hydration increased over time, and Cutometer parameters (R2 R5 R7) showed progressive improvements in elasticity, indicating enhanced skin quality along with midface volumization.

CONCLUSIONS:

The HA-PN hybrid filler demonstrated improvements in midface volume restoration with concurrent improvements in skin barrier function, hydration, and elasticity, and was well tolerated over 24 weeks within the constraints of the study design.

Intra-articular injection of high versus low molecular weight hyaluronic acid in adhesive capsulitis; randomized trial

BACKGROUND:

Adhesive capsulitis (AC) is a chronic inflammatory condition with limited range of motion (ROM) in the glenohumeral joint. The main goals in managing AC are pain reduction and returning joint function. Intra-articular hyaluronic acid (HA) has been considered a safe modality in AC. This study compared the effectiveness of intra-articular injection of low molecular weight (LMW) with high molecular weight (HMW) HA in patients with AC.

METHODS:

Fifty-six patients with AC were randomized in this triple-blinded clinical trial. All underwent standard physical therapy. The outcomes were visual analog scale (VAS), Oxford shoulder score (OSS), active ROM, and patient satisfaction at 4, 12, and 24 weeks.

RESULTS:

No significant differences were detected in between-group variables at baseline. Both groups showed marked improvement in VAS, OSS, and ROM over time. At four weeks LMW-HA group had lower pain (P = 0.049). Conversely, the trend of VAS at 12-week and 24-week endpoints favors HMW-HA (Baseline to endpoint: -5.48 ± 1.68 and -3.91 ± 1.31 reduction in VAS as the primary outcome, P < 0.001). Satisfaction had significantly improved in both groups.

CONCLUSION:

HMW-HA was associated with greater pain reduction and functional improvement compared with LMW-HA.

Knee osteoarthritis affects hundreds of millions of people worldwide and remains one of the leading causes of chronic pain, disability, and reduced mobility. While intra-articular corticosteroid and hyaluronic acid injections are among the most commonly used non-surgical treatment options, many patients experience only temporary symptom relief, creating continued interest in regenerative medicine approaches capable of providing more durable clinical improvement. The systematic review identified controlled human clinical trials evaluating intra-articular allogeneic umbilical cord-derived stem cell therapy and synthesized the available evidence using a random-effects meta-analysis. Across the pooled analysis, stem cell therapy demonstrated lower WOMAC pain scores at 12 months compared with active comparator injections. The direction of benefit remained consistent across studies using hyaluronic acid and corticosteroid comparator groups. The analysis also found lower total WOMAC scores at 12 months among patients receiving stem cell therapy, supporting broader improvements in pain, stiffness, and physical function. The quantitative analysis included two controlled clinical trials using different comparator therapies. One trial evaluated repeated stem cell therapy against hyaluronic acid, while a separate randomized controlled trial evaluated stem cell therapy against intra-articular triamcinolone corticosteroid. Although the comparator therapies differed, the pooled random-effects analysis demonstrated a consistent direction of benefit favoring stem cell therapy across both comparator subgroups. They further noted that while the current controlled evidence base remains limited, the consistent direction of benefit supports continued evaluation in larger randomized clinical trials. Nexsphere-F™ Clinical Study Published in Radiology, Showing 57% Reduction in Knee Osteoarthritis Pain

SEOUL, South Korea, July 6, 2026 /PRNewswire/ -- NEXTBIOMEDICAL CO., LTD. (KOSDAQ: 389650), an innovative medical device company based in South Korea, announced today that Radiology, the leading peer-reviewed journal in radiology and medical imaging with an impact factor of 17.6, has published clinical results for Nexsphere-F™, the Company's fast-resorbable microsphere for embolization. The study, titled "Genicular Artery Embolization Using Rapidly Resorbable Gelatin-based Microspheres for Osteoarthritis-related Knee Pain," was conducted by researchers at Charité – Universitätsmedizin Berlin, ranked eighth on Newsweek's World's Best Hospitals 2026 list. The Company believes this is the largest prospective, single-center observational study to date evaluating genicular artery embolization (GAE) using a resorbable embolic agent. The prospective study enrolled 194 patients with knee osteoarthritis who had not responded to conventional conservative treatments, including corticosteroid and hyaluronic acid injections, and evaluated the safety and clinical outcomes of GAE performed with Nexsphere-F™. Prospective studies, which follow patients according to a predetermined protocol, are widely regarded as providing a higher level of clinical evidence than retrospective analyses. Across 239 procedures, GAE using Nexsphere-F™ achieved a 100% technical success rate. Patients' pain scores, measured on the Numeric Rating Scale, decreased from a median of 7 before the procedure to 3 at 12 months post-procedure, a 57% reduction. Knee function and quality of life, assessed using the Knee Injury and Osteoarthritis Outcome Score (KOOS), improved significantly across all measures, with approximately 80% of patients achieving a minimum clinically important difference (MCID). No moderate or severe procedure-related adverse events were reported. NEXTBIOMEDICAL expects the results to support its expansion into the European market, where Nexsphere-F™ holds CE marking under the Medical Device Directive (CE-MDD). The clinical data is also expected to serve as key evidence for future efforts to expand insurance reimbursement and adoption among medical institutions. Permanent microspheres and IPM-CS have been associated with skin discoloration rates as high as 65% and 18%, respectively, in prior studies, with skin ulceration reported in one such study involving permanent particles. By contrast, Nexsphere-F™ was associated with transient skin discoloration in just 6.3% of procedures, resolving within 24 hours. AAOS Releases Clinical Practice Guideline for Management of Ankle Osteoarthritis

First ankle osteoarthritis guideline highlights burden in younger patients and evidence gaps in treatment ROSEMONT, Ill., July 2, 2026 /PRNewswire/ -- The American Academy of Orthopaedic Surgeons (AAOS) issued a new Clinical Practice Guideline (CPG) for the Management of Ankle Osteoarthritis (OA). This is the first CPG to provide recommendations on non-operative and operative treatments for ankle OA. "The introduction of this CPG to the orthopaedic community is important because ankle OA disproportionately affects younger, active, working-age patients," said Christopher Gross, MD, FAAOS, co-chair of the CPG workgroup. "Unlike the hip and knee OA population, which traditionally affects an older population, ankle OA is a distinct clinical entity, and I'm glad that it has been recognized with its own guideline." Treatment options for ankle OA, which include joint-preserving procedures, ankle arthrodesis (a procedure that permanently joins the shinbone to the ankle bone), and total ankle arthroplasty, have expanded, underscoring the need for an ankle-specific, evidence-based guideline. The CPG features two recommendations: a moderate recommendation against using intra-articular platelet-rich plasma (PRP) to treat symptomatic ankle OA, and a strong recommendation against using intra-articular hyaluronic acid (HA) alone to treat ankle OA. However, the CPG workgroup noted that there may be a short-term benefit in pain and function when HA is combined with corticosteroids. "I administered PRP injections for ankle OA for about 10 years, and I remain supportive of biologic treatment options, but unfortunately, there is no high-quality evidence demonstrating that PRP provides better pain control than injecting saline into the ankle," said Dr. Gross. Additionally, access to HA remains limited because many insurance payers do not cover its use for ankle OA. At present, there is no clear, complete protocol for combining HA with a corticosteroid, making it harder to offer these treatment options to patients. "When you combine HA with a corticosteroid, it seems to have a better effect than either the steroid or the HA alone," said Jeannie Huh, MD, FAAOS, co-chair of the CPG workgroup. "We hope these findings will help improve patient access to effective treatment options." The guideline also contains 11 consensus recommendations and one limited-strength option, which are formed when there is little to no evidence on the topic. Highlights of the consensus recommendations include: Intra-articular corticosteroid injections may provide short-term symptom relief There is no reliable evidence that intra-articular stem cell therapy is beneficial for symptomatic ankle OA Patients with mild to moderate symptomatic ankle OA who wish to avoid surgery may benefit from skilled physical therapy, which could improve patient-reported outcomes and potentially affect OA progression Physical therapy may improve range of motion, return to work or activity, strength and gait restoration for patients who have surgery Prescription opioids should not be used in the management of ankle OA; however, using nonsteroidal anti-inflammatory drugs and/or acetaminophen may be used for initial symptomatic relief, when no other medical contraindications exist Weight reduction may improve patient-reported outcomes and affect the progression of OA symptoms. "While this CPG provided a broad overview of various interventions to help manage symptomatic OA, we are hopeful that future CPGs will review definitive surgical treatments," said Dr. Huh. "In the meantime, we encourage patients and orthopaedic surgeons to engage in shared decision making, analyzing the pros and cons of available procedures to find the option that is best for the patient."
