Impact of erythropoietin and myoinositol versus metformin on insulin resistance in a rat model of polycystic ovary syndrome

This study aimed to evaluate the therapeutic role of erythropoietin (EPO) or myoinositol versus metformin (MET) in improving the reproductive functions and glucose tolerance in a rat model of polycystic ovary (PCOS). Oral letrozole (LTZ) was used for induction of PCOS in wester rats for 21 days, after that, MET, EPO and myoinositol were administered for the following 21 days. The LTZ-induced PCOS rats have lost their oestrous cyclicity and become fixed at the diestrus phase, developed insulin resistance, abnormal sex and gonadotrophin hormone serum levels, increased cystic follicles, decreased number of the growing follicles and very little or no corpora lutea on microscopic examination, which were reversed by the three drugs, MET, EPO and myoinositol. MET and myoinositol were mostly equally effective in improving the reproductive manifestations of the disease. However, EPO was most effective in decreasing the insulin level observed in this LTZ-induced model of PCOS.

Pain responses in preterm infants and parental stress over repeated painful procedures: a randomized pilot trial

OBJECTIVES:

In this pilot study, the aims were to determine the feasibility of whether pain behavior in extremely and very preterm infants and perceived parental stress change when parents are involved in pain reducing measures, either actively, performing facilitated tucking or passively, observing the intervention, in comparison to the involvement of nurses only. In addition, the infant's pain reactivity and parental stress over three time points of measurement was of interest.

METHODS:

Extremely and very preterm infants in need of subcutaneous erythropoietin were randomly assigned to the two intervention groups. The intervention encompassed that one parent of each infant was involved during the painful procedure: Either parents executed facilitated tucking themselves or stood by, observing the procedure. Usual care involved that nurse executed facilitated tucking. All infants received 0.5 ml of 30% oral glucose solution via cotton swab before the painful procedure. Infant pain was observed with the Bernese Pain Scale for Neonates (BPSN) and measured with the MedStorm skin conductance algesimeter (SCA) before, during, and after the procedure. Parents' stress levels were measured before and after the painful procedure on the infant, using the Current Strain Short Questionnaire (CSSQ). Feasibility of a subsequent trial was determined by assessing recruitment, measurement and active parental involvement. Quantitative data collection methods (i.e. questionnaires, algesimeter) were employed to determine the number of participants for a larger trial and measurement adequacy. Qualitative data (interviews) was employed to determine parents' perspectives of their involvement.

RESULTS:

A total of 13 infants (98% participation rate) were included along with their mothers. Median gestational age was 27 weeks (IQR 26-28 weeks), 62% were female. Two infants (12.5%) dropped out of the study as they were transferred to another hospital. Facilitated tucking turned out to be a good method to actively involve parents in pain reducing measures. No significant differences between the two intervention and control groups were found concerning parental stress and infant pain (p = .927). Power analysis indicated that at least N = 741 infants (power of 81%, α = .05) would be needed to obtain statistically significant results in a larger trial, as effect sizes were smaller than expected. Two of the three measurement tools - i.e. the BPSN and CSSQ) - proved easy to implement and were well accepted. owever, the SCA was challenging in this context. Measurements were also found to be time-consuming and resource-intense (i.e. health professionals as assistants).

CONCLUSIONS:

Although the intervention was feasible and was readily accepted by parents, the study design was found to be challenging along with the SCA. In preparation of the larger trial, the study design needs to be revisited and adjusted. Thus, issues of time and resources may be resolved. In addition, national and international collaboration with similar neonatal intensive care units (NICU) needs to be considered. Thus, it will be possible to conduct an appropriately powered larger trial, which will yield important results to improve pain management in extremely and preterm infants in NICU.

Background::

Nucleated red blood cells (NRBC) are very uncommon in the blood of children and adults, but small numbers are not rare in neonates on the day of birth. Elevated NRBC counts in neonates can be seen following erythropoietin dosing. Limited studies in human neonates suggest the time-interval between erythropoietin dosing and the first appearance of NRBC in the blood (the “NRBC emergence-time”) is in excess of 24 hours.

Methods::

We made serial blood counts (Sysmex veterinary analyzer) on ten newborn lambs; five were dosed with darbepoetin (10 μg/kg), and five were dosed with a vehicle-control to assess the NRBC emergence time under relatively controlled laboratory conditions.

Results::

The first appearance of NRBC was at 24 h (2757 ± 3210 NRBC/μL vs. 0/μL in controls). Peak was 48-72 h (16,758 ± 8434/μL vs. 0/μL in controls), followed by fewer at 96 hours (7823 ± 7114/μL vs. 0/μL in controls). Similarly, reticulocytes peaked at 48-72 h (113,094 ± 3210/μL vs. 10,790 ± 5449/μL in controls), with no changes in platelets or leukocytes.

Conclusions::

The NRBC emergence time in newborn lambs is similar to reports from newborn hu-mans. By extrapolation, if a neonate has a high NRBC at birth, the erythropoietic stimulus likely occurred within the interval 24 to perhaps 96+ hours prior to birth.

GSK scraps plan to launch oral anemia drug in EU, ceases further expansion efforts

GSK's decision to not market daprodustat in EU comes shortly after Astellas took a sizable impairment charge for its rival oral anemia drug. Despite a positive opinion from local drug reviewers, GSK has decided not to bring its FDA-approved oral anemia drug daprodustat to Europe—or any additional countries for that matter. Daprodustat, marketed as Jesduvroq in the U.S., earned backing from the drug evaluation committee at the European Medicines Agency a month ago. The recommended indication was for treating anemia associated with chronic kidney disease (CKD) in patients on chronic dialysis. From there, an official approval from the European Commission was largely considered a done deal. But in reporting its second-quarter financial results (PDF) on Wednesday, GSK said it has decided not to commercialize daprodustat in Europe. The company said it will withdraw the application in Europe and won’t seek approvals in other territories. Besides the U.S., daprodustat is also available in Japan under the brand name Duvroq. It's distributed there by Kyowa Kirin. In making this decision, GSK cited the “significant reduction” in the size of the drug’s potential market opportunity, the fact that the EMA didn’t include non-dialysis patients in its opinion, and other existing rivals. Thanks to a 2021 go-ahead, FibroGen’s partner Astellas has been selling rival drug Evrenzo, or roxadustat, in Europe for CKD anemic patients regardless of their dialysis status. And in April, Akebia's vadadustat won an EU green light for dialysis-dependent patients. Despite Evrenzo's broad label in Europe, Astellas recently booked an impairment charge worth 47.1 billion Japanese yen ($340 million) based on its reduced sales outlook for the drug. During the 12 months that ended in March, Evrenzo only brought in 3.2 billion yen ($23 million) for Astellas across Japan and EU. In China, where FibroGen is partnered with AstraZeneca, the drug generated $209 million in 2022. In recent years, discrepancies among drug regulators in the U.S., EU and Japan have been laid bare in their reviews of the HIF-PHI drug class. Jesduvroq, Evrenzo and Vafseo all belong to this group, which once looked promising enough to replace injectable erythropoietin-stimulating agents. Japanese authorities embraced all three drugs early on, endorsing them in dialysis-dependent and nondialysis CKD patients. In the U.S., heart disease and other safety signals prompted the FDA to completely reject FibroGen and AstraZeneca's roxadustat in 2021. The agency knocked back vadadustat, too, based on similar cardio risk factors, but it now appears open to re-considering the med in the dialysis-dependent population after Akebia's appeal. As for GSK's Jesduvroq, the drug in February became the first oral treatment for CKD-related anemia approved in the U.S., but it’s not allowed for patients who aren’t on dialysis. Before the safety fallout, analysts had predicted blockbuster sales for members of the HIF-PHI class. GSK had previously predicted around 500 million pounds to 1 billion pounds of peak sales for Jesduvroq. Now, the drug will obviously have a hard time reaching that mark.
