Malignant melanoma is the most severe form of skin cancer, and its incidence is increasing worldwide. In melanoma, melanocytes overexpress melanin, which can act as a specific biomarker for early detection. The benzamide (BZA) structure is known to bind to melanin, and it can be used as a skeletal structure for radiopharmaceuticals for visualizing malignant melanoma. In this study, we synthesized a novel BZA derivative, including a bioorthogonal chelator, and labeled it with ⁶⁴Cu for biological evaluation. Cell uptake of the ((6-((2-(dimethylamino)ethyl)carbamoyl)pyridin-3-yl)methyl)carbamodithioic acid (MI-0201B2) pretargeted ⁶⁴Cu treatment group was threefold higher (1.73 ± 0.17%) in melanin-expressed B16F10 (mouse melanoma) cells than in negative control cells. MicroPET imaging of the B16F10-bearing C57BL/6 mice (n = 4) showed tumor uptake of the MI-0201B2 pretargeted ⁶⁴Cu injection group of 4.42 ± 0.52%ID/g at 1 h, with this increasing until 48 h (7.74 ± 1.10%ID/g). By contrast, when only ⁶⁴Cu and ⁶⁴Cu-MI-0201B2 were injected, the tumor uptakes were lower than the MI-0201B2 pretargeted ⁶⁴Cu injection group (6.49 ± 0.48%ID/g), with values of 3.94 ± 0.09 and 4.53 ± 0.09%ID/g at 24 h, respectively. Ex-vivo biodistribution studies revealed that the MI-0201B2 pretargeted ⁶⁴Cu injection group showed specific tumor uptake and retention for 48 h (4.92 ± 0.74%ID/g at 48 h). The novel chelator MI-0201B2 exhibited excellent melanin targeting activity, demonstrating its potential as a PET imaging agent for melanoma diagnosis utilizing a bioorthogonal strategy.

The use of conjugated estrogens and bazedoxifene for the management of vasomotor symptoms in premenopausal and menopausal patients with endometriosis: a systematic review

OBJECTIVE:

This systematic review investigated the impact of conjugated estrogens/bazedoxifene (CE/BZA) for treating perimenopausal and menopausal symptoms in patients with a history of endometriosis.

METHOD:

The review followed PRISMA guidelines and was prospectively registered with PROSPERO (CRD42024617174). Without randomized controlled trials (RCTs), the Joanna Briggs Institute (JBI) Critical Appraisal Checklist for Case Reports and Case Series assessed methodology and risk of bias. An information specialist completed the search in June 2025 using Ovid MEDLINE, PubMed, Ovid EMBASE, and Web of Science, combining controlled vocabularies and keywords for dysmenorrhea, dyspareunia, endometriosis, perimenopausal, postmenopausal, menopause hormone therapy, Duavive, and CE/BZA. Eligible studies included RCTs, cohort studies, case reports, and case-control studies evaluating CE/BZA for vasomotor symptoms in premenopausal and menopausal endometriosis patients.

RESULTS:

Of 1540 retrieved studies, two coauthors (J.C.M.-G., E.S.) independently screened titles and abstracts, selecting 20 for full-text review. Only two publications met inclusion criteria, one case report and one case series, representing nine patients (four detailed, five additional). No RCTs, cohort, or case-control studies directly addressed CE/BZA in endometriosis. Preliminary narrative evidence suggests pain and vasomotor symptom relief, though findings carry high risk of bias.

CONCLUSION:

Because bazedoxifene antagonizes estrogen receptors and endometriosis is estrogen-dependent, CE/BZA may alter systemic inflammation or endothelial function. Although preclinical models show reduced lesion size, human evidence remains extremely limited and biased, insufficient to assess lesion recurrence or vascular safety. CE/BZA is currently indicated only for postmenopausal vasomotor symptoms; preliminary anecdotal evidence suggests symptom improvement, but large-scale comparative trials are urgently needed to establish safety and efficacy in endometriosis.

Comparative Finite Element Analysis of Denosumab and Bazedoxifene on Pedicle Screw Stability in Osteoporotic Spines

ABSTRACT:

Introduction:

Pedicle screw fixation in osteoporotic spines remains challenging. Bazedoxifene (BZA) and denosumab (Dmab) are widely used agents for osteoporosis, but their comparative effects on spinal instrumentation are not well understood. This study aimed to evaluate the effects of BZA and Dmab on biomechanical parameters of spinal instrumentation using finite element analysis (FEA).

Methods:

In this prospective, open‐label study, postmenopausal women with primary osteoporosis were assigned to receive either BZA (daily oral, 20 mg) or Dmab (subcutaneous, 60 mg every 6 months) for 12 months. FEA models of the L4 vertebra were generated from CT scans using a calibration phantom (Mindways, Austin, TX, USA). Vertebral compression force was evaluated to represent overall vertebral strength. Pedicle screw fixation strength was assessed under axial (pullout strength) and non‐axial directional forces (cranial, caudal, lateral, medial). Inverse probability of treatment weighting (IPTW) and multivariable regression were used to balance baseline differences and compare biomechanical outcomes between groups.

Results:

Thirty patients were enrolled (15 per group); the final analysis included 12 in the BZA group and 13 in the Dmab group. Compared to BZA, Dmab significantly improved compression strength (adjusted mean difference: 8.1% [95% CI, 0.9–15.3], p = 0.030) and pullout strength (15.8% [95% CI, 6.2–25.4], p = 0.003). Directional FEA revealed greater resistance to cranial (17.4% [95% CI, 4.9–30.0], p = 0.009) and lateral (10.8% [95% CI, 0.9–20.8], p = 0.035) loading with Dmab, while no significant difference was observed in caudal‐ and medial‐directed force.

Conclusion:

Finite element modeling suggested that Dmab enhanced pedicle screw fixation more effectively than BZA, particularly against axial and cranial/lateral‐directed forces. These biomechanical differences underscore the potential advantage of Dmab in preoperative osteoporosis management to improve pedicle screw stability.

White House Strikes Deal with Pfizer for Lower Drug Prices, Online DTC Website

Amid ongoing approaches to lower drug prices for US patients, like the Most-Favored-Nation policy, this is another partnership the Trump Administration believes will improve access challenges. The Trump Administration is set to team up with Pfizer in a partnership that would bring lower prices for the manufacturer’s drugs as well as a direct-to-consumer (DTC) website for patients to purchase them, which the White House is naming “TrumpRx.”1 While the rollout of this new partnership is still in its beginning stages, Pfizer is the first drugmaker to provide actionable steps in lowering US drug prices across the board,2 an initiative perpetuated by the signing of the Most-Favored-Nation (MFN) Prescription Drug Pricing Executive Order.3 “It’s a win for American patients, a win for American leadership, and it’s a win for Pfizer because it provides the certainty and stability we need to continue advancing new breakthrough medicines for patients,” said Pfizer spokeswoman Amy Rose, according to The Wall Street Journal.2 Despite early reports of the Pfizer program providing a decrease in drug prices for Medicaid patients, there is potential for savings to reach the pockets of all US patients and consumers as President Trump continues to put pressure on manufacturers. On top of this most recent Pfizer-White House partnership, as well as provisions backed by the MFN policy, the potential impact of pharmaceutical tariffs could add a third layer to the evolution of drug prices in the US. With the new partnership, Pfizer will receive a 3-year grace period on being susceptible to Trump’s pharmaceutical tariffs, which are expected to take effect October 1. “The agreement on lower prices will cover ‘a large majority’ of Pfizer's primary care medicines, along with specialty brand-name drugs, which will be offered at discounts averaging 50% and reaching as high as 85%, a spokesperson for the company said,” according to NBC News.1 Some of the drugs correspondents close to the partnership have named for future price reductions include the menopause medication Duavee (conjugated estrogens/bazedoxifene), Toviaz (fesoterodine fumarate) for treating overactive bladder, and Pfizer’s Eucrisa (crisaborole) skin ointment.4-6 On top of these, and whatever additional drugs that Pfizer already markets, all new drugs that the company introduces will also be sold at lower prices.1 Alongside the announcement of this unprecedented partnership, the administration is set to roll out its “TrumpRx” DTC website for patients across the country to access necessary prescription medications. The website will be fully owned and operated by the US government as well as giving it the federal power to negotiate the prices of DTC drugs themselves.2 The extent of which drugs will be offered and at what type of discounts through the TrumpRx website is currently unknown. There is also uncertainty regarding what patient populations and insurance coverage would be set aside to reap the benefits of these discounts and savings. However, according to Pfizer and White House spokespeople who reported to The Wall Street Journal, the new partnership is projected to benefit as many as 100 million US patients. “Moving forward, the only thing I will accept from drug manufacturers is a commitment that provides American families immediate relief from the vastly inflated drug prices and an end to the free ride of American innovation by European and other developed nations,” said Trump, according to Politico.7 Amid the administration’s ongoing commitment to lower US drug prices, they are looking to get US consumers on a level playing field with consumers in other, developed nations. Hence, the MFN drug policy; this was signed as an executive order in June of this year and plainly states that multiple federal agencies should “take specific actions aimed at compelling drug manufacturers to lower drug prices in the United States in a manner comparable with other ‘developed nations.’”3 From the MFN drug policy to prospective pharmaceutical tariffs and a key partnership between the federal government and a major US drug manufacturer, there is significant movement from the Trump Administration to recorrect the rate of rising drug costs in the US. Many experts in the pharmacy and pharmaceutical industry have expressed disagreement and uncertainty regarding the aforementioned provisions, partnerships, and policies. Pfizer shakes hands with White House on drug discounts under MFN programme

Pfizer is the first in what could be a string of pharmaceutical companies to come to an agreement with the White House to reduce US drug prices. As part of President Trump’s Most Favored Nation (MFN) policy , the company agreed to ensure that US patients are paying less for prescription medicines, with primary care treatments and some select speciality brands set to be offered at savings that will range as high as 85% and an average of 50%. The MFN policy has been set up by Trump’s administration to tie drug prices in the US to those paid elsewhere in the world. These savings will be accessible through both Medicaid and the federal direct purchasing platform TrumpRx.gov. It was also announced that all new medications from Pfizer will be sold at MFN levels. In a 30 September press conference, Trump said: “For many years, Americans have paid the highest prices anywhere in the world for prescription drugs. The major reason is that American consumers have been subsidising research and development costs for the entire planet.
