Utilizing Photodynamic Therapy to Amplify the Response to Immunotherapy in Patients With Non-Small Cell Lung Cancer (NSCLC) With Pleural Disease or Malignant Pleural Mesothelioma (MPM) - Phase I Study

This phase I trial evaluates the side effects of intraoperative photodynamic therapy with porfimer sodium in enhancing the response to immunotherapy with an immune checkpoint inhibitor drug in patients with non-small cell lung cancer with pleural disease. Photodynamic therapy is a technique that that works by combining a photosensitizing agent (porfimer sodium in this trial) and an intense light source to kill tumor cells. Photodynamic therapy may decrease the patients' symptoms and improve their quality of life.

Light Dosimetry for Intraoperative Photodynamic Therapy (IO-PDT) With Porfimer Sodium (Photofrin) in Patients With Malignant Mesothelioma or Non-Small Cell Lung Cancer (NSCLC), or Other Malignancies With Pleural Disease - Phase I Study

This phase I trial studies the side effects and how well light dosimetry system works during photodynamic therapy with porfimer sodium in treating participants with malignant mesothelioma , non-small cell lung cancer or any other malignancy with pleural disease undergoing surgery. Light dosimetry measures the amount of laser light given during photodynamic therapy. Photodynamic therapy uses a drug, such as porfimer sodium, that becomes active when it is exposed to light. The activated drug may kill tumor cells. Using light dosimetry for intraoperative photodynamic therapy may help doctors estimate how much light is delivered during photodynamic therapy and decide if the treatment should be stopped or continued.

Neoadjuvant bronchoscopic photodynamic therapy to facilitate airway and parenchymal sparing lobectomies in two patients with central airway neuroendocrine tumors: A patient centered approach

BACKGROUNDNCCN guidelines recommend surgical resection for well differentiated pulmonary neuroendocrine tumors (NETs), also known as carcinoid tumors, when they present in the lungs. Fortunately, most pulmonary NETs will not require extensive surgical resection beyond a lobectomy. We report two cases in which large pulmonary NETs would have required more extensive surgical resection, including a carinal pneumonectomy and a left lower lobe sleeve resection. However, after taking patient's preferences into account, neoadjuvant photodynamic therapy was able to decrease the overall tumor burden helping to facilitate airway and parenchymal sparing surgical lobectomies.MATERIALSInterventional pulmonology (IP) and Cardiothoracic surgery (CTS) discussed options extensively with each patient and formulated a plan to perform neoadjuvant bronchoscopic followed by surgical resection if there were any remaining NET on follow up bronchoscopy. Both patients received infusions of porfimer sodium (Photofrin ®, Pinnacle Biologics) at 2mg/kg. Per standard protocol, they each underwent a series of bronchoscopies 48 h after infusion, in which endobronchial NET was illuminated with a fiberoptic catheter delivering up to 200J/cm per treatment. After the initial illumination bronchoscopy, a cryo probe was used to debulk necrotic tumor and allow for repeat illumination of residual tumor. Both patients received extensive education on avoiding phototoxicity.OUTCOMESBoth patients underwent follow up restaging bronchoscopies revealing cleared central airways and only residual NET at the subsegmental level. As such, both underwent lobectomies, avoiding more extensive surgical resection. Both are free of disease at four years and 18 months follow up, respectively. Both patients were satisfied with their outcomes and the autonomy they were given in formulating their treatment plan.CONCLUSIONSWhile photodynamic therapy has been shown to be an effective stand alone, neoadjuvant, and adjuvant therapy for pulmonary NETs, surgical resection is still required in select patients. We present two cases of in which desired patient outcomes led to the use of neoadjuvant PDT to help facilitate airway and parenchymal sparing lobectomies, thus avoiding more extensive surgical resection and possible long-term morbidity.

Photostimulated Anticancer Activity of Mitochondria Localized Rhenium(I) Tricarbonyl Complexes Bearing 1H‐imidazo[4,5‐f][1,10]phenanthroline Ligands Against MDA‐MB‐231 Cancer Cells

AbstractWe have introduced Re(I) tricarbonyl complexes (ReL1 ‐ ReL6) [Re(CO)3(N^N)Cl] where N^N=extensive π conjugated imidazo‐[4,5‐f][1,10]‐phenanthroline derivatives that helps in strong DNA intercalation, enhanced photophysical behavior, increase the 3π‐π* character of T1 state for PDT and high value of lipophilicity for cell membrane penetration. These complexes exhibited prominent intraligand/ligand‐centered (π‐π*/1LC) absorption bands at λ 260–350 nm and relatively weak metal‐to‐ligand charge‐transfer (1MLCT) bands within the λ 350–550 nm range. Among the six synthesized complexes, [(CO)3ReICl(K2‐N,N‐2‐(4‐(1‐benzyl‐1H‐tetrazol‐5‐yl)phenyl)‐1H‐imidazo[4,5‐f][1,10]phenanthroline] (ReL6) exhibited outstanding potency (IC50~6 μM, PI>9) under yellow light irradiation compared to dark conditions. Importantly, extremely lipophilic complex ReL6 showed effective penetration through the cell membrane and localized primarily in mitochondria (Pearson's correlation coefficient, PCC=0.918) of MDA‐MB‐231 cells. Complex ReL6 exhibited more than 9 times higher photo‐toxicity in normoxic and hypoxic environment of tumor by inducing 1O2 generation (type II PDT), radical generation triggered by NADH oxidation (type I PDT). This complex is a promising candidate for TNBC treatment in hypoxic tumors, with efficacy comparable to photofrin and have demonstrated CO release ability under UV light irradiation.

Although porous frameworks are structurally ideal for the development of biomaterials through drug adsorption, sequestration, and delivery, integration of multiple biofunctions into a biocompatible porous framework would greatly improve its potential for preclinical investigations by increasing both therapeutic value and research and development efficiency. Herein, we report the preparation of a highly biocompatible supramolecular organic framework from an imidazolium-derived tetrahedral monomer and cucurbit[8]uril. The supramolecular organic framework has been revealed to have regular intrinsic porosity and adsorb doxorubicin, photofrin, and heparins driven by hydrophobicity and/or ion-pairing electrostatic interactions. In vivo or in vitro assays illustrate that this adsorption leads to efficient intracellular delivery of doxorubicin, which enhances its antitumor efficacy, elimination of photofrin, which inhibits its post-treatment phototoxicity without reducing its photodynamic therapeutic activity, and sequestration of (low-molecular-weight) heparins, which neutralizes their anticoagulation activity more efficiently than clinically used protamine.

Concordia Stock Rockets on Rumors of a Blackstone Buyout

April 22, 2016 By Mark Terry , BioSpace.com Breaking News Staff Concordia Healthcare Corp. , based in Oakville, Ontario, is rumored to be the takeover target of New York-based private equity investment banking firm The Blackstone Group . Concordia jumped from $25.01 on April 19 to a current price of $32.26 at the rumors. Concordia manages and acquires legacy drugs, and buys and develops orphan drugs. It has a diversified portfolio of more than 200 established off-patent compounds. Its Orphan Drug Division is currently made up of Photofrin for rare forms of cancer. The company announced yesterday that it had created a special committee of independent members of the board of directors to evaluate various strategic options. The statement does say, however, “there can be no assurance any transaction will occur.” The sources told Bloomberg that talks with Blackstone are very early and may not happen. There may also be other bidders. On its part, Concordia has been in acquisition mode since 2013. In 2015, it announced plans to buy Amdipharm Mercury Ltd in a deal priced at $3.5 billion. Since 2013, it has spent $5 billion on various acquisitions and deals. Concordia acquired Amdipharm from Cinvent , a European private equity firm. The deal was a combination of debt financing and cash. There was $2.8 billion in debt financings, $520 million is gross proceeds from a public equity offering, and cash. In addition, Concordia provided 8.49 million common shares of stock, which represented about 14.2 percent of Concordia’s outstanding common shares. Company shares fell about 73 percent since the Amdipharm deal, primarily over concerns about the company’s debt levels. It also was the target of short sellers. Another problem is comparisons to another Canadian drug company, Valeant Pharmaceuticals . A noted serial acquirer with more than 108 deals in the last 8 years, Valeant is currently in a major crisis. Company shares have tanked, William Ackman ’s Pershing Square Capital Management , one of Valeant’s biggest investors, sold about 5 million company shares on the last day of trading in 2015, and there are a number of ongoing U.S. Securities and Exchange Commission (SEC) investigations. The company is also looking for a new chief executive officer and needs to pay down about $30 billion in debt. To do so it is potentially selling off Bausch & Lomb . “Concordia has been the subject of an unrelenting and unscrupulous attack by a group of short sellers for several months,” said Mark Thompson , Concordia’s chief executive officer, in a statement last month. “We will continue to monitor this situation and will act accordingly. However, we are extremely excited about the business we have built and the opportunity to delivery long-term shareholder value.” In its annual financial reports on March 23, 2016, Concordia reported revenue of $394.2 million, up from $104.9 million in the previous year. Net income from continuing operations, however, was down, showing a loss of $29.4 million in 2015 compared to a gain of $8.9 million in 2014. The revenue increase was primarily credited to Concordia International products, and organic growth from Donnatel, its adjunctive therapy for irritable bowel syndrome. The operational losses were related to acquisitions, as well as debt-related charges. “Concordia’s accomplishments in 2015 have created a global infrastructure that diversified the Company across product lines, geographies and sales channels,” said Mark Thompson , the company’s founder, chairman, and chief executive officer, in a March 23 statement. “Our infrastructure uniquely positions the Company to provide safe and effective medicines to patients in more than 100 countries, which we believe will allow us to evaluate growth opportunities on a global scale going forward. In addition, our achievements in 2015, in particular the acquisition of the portfolio of products from Covis, the purchase of AMCo and the organic growth we have generated from key products such as Donnatal, have resulted in substantial year-over-year revenue and adjusted EBITDA growth.”
