More than 70% of patients with cancer in the palliative phase have pain that often requires treatment with opioids (morphine-like agents). Constipation occurs in 59% of patients treated with opioids. Opioid-induced constipation (OIC) has consequences that range from daily discomfort with social insecurity and disability to intestinal obstruction. It leads to limitations in self-management, a reduced quality of life and a risk of the need for more care. In the guideline "Diagnosis and treatment of pain in patients with cancer" it is recommended to start preventively with an osmotic laxative such as macrogol/electrolytes or magnesium hydroxide when starting opioids. Macrogol/electrolytes has been proven to be effective for OIC, but is sometimes perceived by patients as unpleasant due to its taste. Magnesium hydroxide, which is less commonly prescribed for OIC, has a neutral taste. Although it is mentioned in the guideline, it is not studied for the treatment of OIC and also not officially registered for this. To support the advice of the guideline and to prove that a choice is possible, it is important to investigate whether there are differences in effectiveness and/or side effects between macrogol/electrolytes and magnesium hydroxide in the prevention of OIC.
The aim of this study is to compare macrogol/electrolytes with magnesium hydroxide in the prevention of opioid-induced constipation in patients with cancer in the palliative (incurable) phase, who start opioids because of pain. The choice of laxative is determined by drawing lots (randomisation). After two weeks, its effect will be assessed and will be presented as the percentage of patients who have not developed constipation after starting opioids. If the laxative, as assigned by lot, is described as satisfactory by the patient, the patient can continue with the drug that the patient used, after the study through regular prescription.
The investigators will ask the patients in the study about their satisfaction with the laxative used, any side effects and the degree of pain. Furthermore, the appeal to care for possible constipation will be examined. Because the best attainable quality of life in the palliative phase is the objective of all care, the investigators will also measure this perceived quality with a questionnaire in this study. The results of this study will lead to the best achievable prevention of opioid constipation in patients with cancer in the palliative phase.

Modulating poly(lactic acid) degradation rate for environmentally sustainable applications

The huge amount of plastics generated by the massive use of packaging makes it difficult to manage waste safely. Introducing biodegradable polymers, such as poly(lactic acid) (PLA), can at least partially reduce the environmental pollution from plastic waste. Biodegradable polymers must have a degradation rate appropriate for the intended use to replace durable plastics. This work aims to introduce PLA fillers that can modulate the degradation rate during hydrolysis and composting. For this purpose, fumaric acid and magnesium hydroxide have been proposed. The experimental findings demonstrated that magnesium oxide makes hydrolysis faster than fumaric acid. A model describing the hydrolysis reaction, which also considers the effect of crystallinity, is proposed. The model can capture the filler effect on the kinetic constants related to the autocatalytic part of the hydrolysis reaction. Degradation of the PLA and compounds was also conducted in a composting medium. The compound with fumaric acid shows faster degradation than the compound with magnesium oxide; this behavior is opposite to what is observed during hydrolysis. Degradation in a composting medium is favored in a narrow pH window corresponding to the optimum environment for microorganism growth. Magnesium oxide leads to a pH increase above the optimum level, making the environment less favorable to microorganism growth. Vice-versa, fumaric acid maintains the pH level in the optimum range: it represents an additional carbon source for microorganism growth.

Magnesium Oxychloride and Magnesium Oxysulfate Cements as Temporary Plugging Agents in Geothermal Drilling

The development and utilization of geothermal resources are effective ways to alleviate the current haze situation, adjust the energy structure, and achieve energy conservation and emission reduction. Geothermal formations typically contain extensive fracture networks, with fracture openings. These fracture networks can result in substantial losses of the drilling fluid and increased costs for geothermal drilling. Temporary plugging cements are used to solve the problem of lost circulation due to their high strength and high acid solubility. In this paper, two types of temporary plugging materials, magnesium oxysulfate (MOS) cement and magnesium oxychloride (MOC) cement, were prepared. The influence of the plugging agent on the flow field and the force exerted on the solid under the action of the fluid was analyzed using fluid-solid coupling software. The simulation results show that when subjected to a flow rate of 10 m/s, the edge of the cement experiences a significant force, while the stress is not widely transmitted to the middle and rear of the cement. This indicates that the cement has a strong resistance to the fluid flow. The fundamental characteristics of MOC cement and MOS cement, such as compressive strength and setting time, were investigated. The test results show that adjusting the molar ratio of the two types of cements can shorten the setting time by 60% and increase the compressive strength to up to 23 MPa. In addition, the acid solubility of the cement with different ratios of raw materials is above 95%. The plugging performance of these two cements as loss circulation materials was evaluated by using a physical simulation device. The pressure bearing capacity of the MOC cement with different MgO/MgCl₂·6H₂O/H₂O molar ratios ranged between 13.4 and 23.6 MPa. The maximum bearing capacity of the MOS cement can reach up to 18.6 MPa. The results showed that both cements possess excellent plugging and pressure bearing capacity.

Coadministration of Lactulose with Probiotics Ameliorates Loperamide-Induced Constipation in Mice

We evaluated the efficacy of mixtures of lactulose with probiotic strains to ameliorate constipation and to identify suitable probiotic strains. Constipation was induced in Institute of Cancer Research mice (6-week-old, male) by the administering loperamide (5 mg/kg, twice a day) orally for 5 days, whereas the control group was not treated. To evaluate the laxative effects of the lactulose-probiotic and lactulose-magnesium hydroxide mixtures, fecal parameters, the gastrointestinal (GI) transit ratio, and fecal short-chain fatty acid (SCFA) content were analyzed. The administration of lactulose and Bacillus licheniformis or Saccharomyces boulardii significantly improved stool number and water content, which were reduced by loperamide. The GI transit ratio was significantly increased compared with that of the control group. The combined administration of lactulose and probiotics (B. licheniformis or S. boulardii) increased total SCFA content, including that of acetate, more effectively compared with lactulose alone. Similarly, coadministration of lactulose and magnesium hydroxide improved the loperamide-induced changes in fecal parameters and GI transit as well as increased total SCFA content. Overall, the combination of lactulose and probiotics relieves the symptoms of constipation by increasing SCFA content and is more effective compared with lactulose alone.

Janssen Submits Supplemental New Drug Application to U.S. FDA Seeking Expanded Pediatric Indication for HIV-1 Therapy EDURANT ®

TITUSVILLE, N.J., July 28, 2023 – The Janssen Pharmaceutical Companies of Johnson & Johnson announced today the submission of a supplemental New Drug Application (sNDA) to the U.S. Food and Drug Administration (FDA) seeking to expand the indication of EDURANT® (rilpivirine) to include the treatment of HIV-1 infection in children weighing 10 kg or more. A parallel Marketing Authorization application has also been submitted to the European Medicines Agency (EMA) in support of a type II variation and line extension for expanded pediatric use in Europe.
