Hemophagocytic lymphohistiocytosis is a rare, aggressive and life-threatening syndrome of excessive immune activation. Secondary hemophagocytic lymphohistiocytosis (sHLH) is the most common form of this disease and is typically associated with several other clinical conditions (eg, malignancy associated HLH (mHLH), infection, or autoimmune disease). ELA026 is a fully human immunoglobulin G1 (IgG1) signal regulatory protein (SIRP)-directed monoclonal antibody designed to deplete the myeloid and T cells driving the inflammation. The purpose of this study is to assess the safety, efficacy pharmacokinetics and pharmacodynamics of ELA026 in participants with sHLH.

BACKGROUND:

Signal regulatory protein alpha (SIRPα) is an important regulator of innate and adaptive immune responses by promoting T cell and macrophage activation and macrophage adhesion (multi-nucleated giant cell formation) while preventing phagocytic cell death to sustain inflammatory responses.

METHODS:

We determined whether SIRPα is expressed in sarcoidosis tissue and whether ELA026, a humanized SIRP-blocking Ab, suppresses sarcoidosis granuloma formation by attenuating inflammation, promoting cell death, and suppressing adhesion during granuloma formation.

RESULTS:

Based upon immunohistochemistry, SIRPα stained in greater abundance in giant cells, monocytes, macrophages and dendritic cells in sarcoidosis lung tissue than in normal lung, lung cancer, non-granulomatous lung diseases and non-sarcoidosis granulomatous diseases. All these differences were statistically significant except for non-sarcoidosis granulomatous lung diseases. We then leveraged an ex vivo human granuloma model, wherein PBMCs from sarcoidosis patients are activated to form granulomas within 7 days, to determine whether ELA026 attenuated granuloma formation, based upon MIPAR image analysis, and associated cytokine responses. A matching isotype antibody (ELA099) that does not bind SIRPs was a negative control; prednisone was a positive control. As hypothesized, pre-treatment (day 0) or post-treatment (day 4) with ELA026 or prednisone (not ELA099) caused dose-dependent suppression of sarcoidosis granuloma formation by day 7. Unexpectedly, ELA026 only modestly reduced TNFα production when used as a pre-treatment and did not suppress IL-1β release or promote cell death, as reflected by no LDH release.

CONCLUSIONS:

SIRPα is robustly expressed in monocyte/macrophage lineages within human sarcoidosis tissues, and inhibition of SIRPs (ELA026) suppresses sarcoidosis granuloma formation by preventing macrophage adhesion/aggregation.

Phase 1 study builds on early signals of anti-tumor activity in patients with refractory T and B cell lymphomas Ipsoprubart, a first-in-class SIRP-targeting therapy, expands into T cell malignancies alongside ongoing pivotal study in secondary hemophagocytic lymphohistiocytosis (sHLH) SOUTH SAN FRANCISCO, Calif., May 05, 2026 (GLOBE NEWSWIRE) -- Electra Therapeutics , a clinical stage biotechnology company pioneering therapies against novel targets for diseases in immunology and cancer, today announced dosing of the first patient in a Phase 1 study of ipsoprubart (ELA026) in patients with T cell malignancies. Ipsoprubart, a first-in-class monoclonal antibody targeting signal regulatory proteins (SIRP) on immune cells, is designed to selectively deplete pathological T cells and myeloid cells, with the potential to inhibit tumor growth and survival through both direct tumor cell elimination and tumor microenvironment modulation. Initial evidence of anti-tumor activity in lymphoma, including T cell malignancies, was observed in a Phase 1b study of ipsoprubart in secondary hemophagocytic lymphohistiocytosis (sHLH), in patients whose disease was triggered by the underlying lymphoma. These findings supported advancement into a dedicated study in T cell malignancies, extending the clinical program for ipsoprubart beyond the ongoing pivotal study in sHLH. The Phase 1 study in T cell malignancies will evaluate safety and efficacy in patients treated with ipsoprubart monotherapy. T cell malignancies are a group of rare, aggressive lymphomas and leukemias arising from mature or immature T lymphocytes. More than 13,000 patients in the US are diagnosed annually with T cell malignancies and have limited effective treatment options that provide durable disease control. “Our team has pioneered SIRP targeting as a therapeutic approach to selectively deplete pathological immune cells,” said Kathy Dong, PharmD, MBA, President and CEO of Electra Therapeutics. “We are excited to follow the science and expand into T cell malignancies, where new treatment approaches are needed for patients and ipsoprubart has shown promising potential.” The open-label Phase 1 study will enroll adults with relapsed or refractory T cell malignancies. Patients may receive up to six cycles (24 weeks) of treatment with ipsoprubart. The study consists of two parts: Part 1 will enroll up to 24 patients to identify up to two dosing regimens with acceptable safety pro Part 2 will further evaluate these regimens in expansion cohorts. The primary endpoint is safety, including drug-related toxicities and treatment-emergent adverse events. Among eight patients with lymphoma-associated sHLH, six of whom had T cell malignancies as the underlying trigger, the objective tumor response rate was 100% (8/8), with a complete response rate of 88% (7/8), including one durable complete response achieved with ipsoprubart monotherapy in a T cell lymphoma patient refractory to more than five lines of prior therapy. A trial-in-progress poster, presented by Dr. Swaminathan Iyer (MD Anderson Cancer Center), will highlight the SURPASS Phase 2/3 pivotal clinical study, which is evaluating the safety, efficacy, pharmacokinetics, and pharmacodynamics of ELA026 in newly diagnosed, treatment‑naïve adult and pediatric patients with sHLH. ELA026 is the first investigational therapy designed to treat sHLH regardless of underlying trigger, leveraging a novel mechanism of action that targets signal regulatory proteins (SIRP) on immune cells. By selectively depleting pathological myeloid cells and T lymphocytes that drive the cytokine storm and disease severity, ELA026 has the potential to reset the hyperinflammatory cascade at its source.
