Age-related macular degeneration (AMD) is a leading cause of vision loss in adults. Abnormal blood vessels grow under the macula at the back of the eye, and also leak blood and fluid, which damages and scars the macula, affecting vision. The current standard of care for patients with neovascular (exudative / wet) AMD is anti-vascular endothelial growth factor (anti-VEGF) therapy, which prevents or slows down the growth of the abnormal blood vessels. SCD411 is being developed as a biosimilar to the reference product Eylea® (aflibercept), an anti-VEGF drug. The study aims to prove equivalence of SCD411 to Eylea in adults with wet AMD, and will look at safety, tolerance, effectiveness, immune response and the movement of the drug through the body.

A pharmacokinetic drug-drug interaction study between pregabalin and tramadol in healthy volunteers

PURPOSECombination therapy of pregabalin and tramadol is used to treat chronic neuropathic pain; however, the pharmacokinetic (PK) interactions of these drugs has not been studied. This study aimed to evaluate PK interactions between pregabalin and tramadol and the safety of combination therapy.METHODSA randomized, open-label, multiple-dose, three-treatment, three-period, six-sequence cross-over study was conducted in healthy subjects. All subjects received the following three treatments for 4 days in each period: pregabalin 150 mg twice daily; tramadol extended-release (ER) 200 mg in the morning, and 100 mg in the evening; and co-administration of pregabalin 150 mg and tramadol ER 200 mg in the morning, and pregabalin 150 mg and tramadol ER 100 mg in the evening.RESULTSA total of 21 subjects completed the study with no clinically significant safety issues. For pregabalin, the geometric mean ratio (GMR) (90% CI; confidence interval) of combination therapy to monotherapy for maximum concentration at steady state (C_max,ss) and area under the concentration curve from 0 to dosing interval time at steady state (AUC_τ,ss) were 0.8801 (0.8043-0.9632) and 1.0830 (1.0569-1.1098), respectively. The corresponding values for tramadol were 1.0177 (0.9839-1.0526) and 1.0152 (0.9896-1.0414), respectively. The GMR (90% CI) of combination therapy to monotherapy of O-desmethyl-tramadol for C_max,ss and AUC_τ,ss was 1.0465 (1.0095-1.0848) and 1.0361 (1.0001-1.0734), respectively.CONCLUSIONSThere were no significant drug interactions between pregabalin and tramadol, considering that all of the 90% CI of PK measures were within the conventional bioequivalence range. Both drugs were well tolerated when administered concomitantly.

Physicochemical Characterization and Dissolution Properties of CS-891 with Different Crystallinity

Ground CS-891 (N-[1-(4-methoxyphenyl)-1-methylethyl]-3-oxo-4-aza-5a-androst-1-ene-17β-carboxamide) of poorly water soluble drug was obtained using a Heiko Seisakusho model TI-100 vibration mill, and samples with different crystallinity were prepared at mixture ratios of 10:0, 7:3, 5:5, 3:7 and 0:10 (intact:ground CS-891).Physicochem. characterizations were obtained using qual. and quant. x-ray diffractometry, different scanning calorimetry (DSC), SEM, Quantasorb surface area analyzer, and controlled atm. microbalance.With increase of amorphous CS-891 in mixture ratios, the intensities of x-ray diffraction peaks of crystalline CS-891 were decreased, whereas surface area, water absorption, and exothermic peaks in DSC were increased.The apparent solubility of ground CS-891 was 4.4 μg/mL and the solubility of intact CS-891 was 3.1 μg/mL at 37 ± 1°.The apparent precipitation rates of CS-891 in a supersaturated solution during the solubility test were increased with an increase of amorphous CS-891, and a crystalline form of CS-891 transformed from amorphous CS-891 after the solubility test was found by x-ray diffraction anal., DSC and SEM.The dissolution profiles of CS-891 with different crystallinity at 37 ± 1° by the USP paddle method were investigated, and the apparent dissolution rate constant of ground CS-891 was about 5.9-fold higher than that of intact CS-891.A linear relationships between the crystallinity of CS-891 and the apparent dissolution rate constant (r > 0.96) were obtained.

In Vivo Prediction and Biopharmaceutical Evaluation of Nicotine Transdermal Patch

Nicotine transdermal therapeutic systems (TTS_s) have been regarded as an effective mean to aid smoking cessation.However, most of nicotine TTS_s in the market have some problems such as unpleasant side effects and skin irritation due to the excess amount of the drug permeated and the properties of the additives employed.In order to solve these problems, new nicotine TTS_s were formulated using biocompatible additives.The optimized formula of the drug layer consisted of nicotine, propylene glycol and poloxamer 188 at the ratio of 1.2: 17.0: 2.0.The drug layer had the thickness of 1,250 μm, the pH of 8.12.The skin permeation rate of nicotine from optimized nicotine patch (NP) was 21.5 μg/cm²/h.Transdermal administration of nicotine patch has been carried out for the determination of pharmacokinetic parameters in rats.Steady-state plasma concentration of nicotine following transdermal application of NP (area of patch = 15 cm²) on the dorsal skin of rats was 143.2 ng/mL and AUC for 24 h was 3,022 ng · h/mL.In case of EXODUS and Nicotinell TTS, the steady-state plasma concentration of nicotine and AUC for 24 h were 428.9 ng/mL, 9,121 ng · hr/mL and 155.3 ng/mL, 3,152 ng · h/mL, resp.NP showed the exptl. plasma nicotine concentration profile was very similar to the simulated one and had an appropriate skin permeation rate and a steady-state concentration of nicotine, which can show therapeutic blood levels of the drug for 24 h without severe side effects.
