Positive Preoperative Stress Test is Associated with Increased Risk of Postoperative Myocardial Infarction in Patients Undergoing Aortoiliac Reconstruction for Chronic Limb-Threatening Ischemia

BACKGROUNDAortofemoral or aortoiliac bypasses (AFBs/AIBs) are considered the gold standard treatment for patients with aortoiliac occlusive disease. Myocardial infarction (MI) is a postoperative risk in AFB/AIB surgeries and preoperative stress test (PST) is frequently utilized to inform operative planning and preoperative risk discussions. The aim of this study is to assess the utility of PST in determining outcomes following AFB/AIB in patients with chronic limb-threatening ischemia (CLTI).METHODSThis is a retrospective study from Vascular Quality Initiative database. Patients undergoing AFB/AIB for chronic limb-threatening ischemia during 2009-2023 were stratified by PST result. Primary outcomes included postoperative MI and 30-day mortality. Secondary outcomes included in-hospital death, in-hospital death/MI, postoperative respiratory complications, postoperative congestive heart failure, intensive care unit stay > 3 days, and 1-year mortality. Logistic and Cox regressions were used for multivariate analyses.RESULTSThe study included 2 cohorts of patients: negative (N = 1,809, 81.2%) and positive PST (N = 420, 18.8%). The patients with positive PST had increased rate of postoperative MI compared to the patients with negative PST (6.7% vs. 2.9%, P < 0.001). The rate of 30-day mortality was greater for patients with positive PST but the difference was not significant (3.8% vs. 2.9%, P = 0.315). After adjusting for potential confounders, positive PST was associated with increased risk of postoperative MI (adjusted odds ratio = 1.89 [95% confidence interval: 1.04-3.43], P = 0.038). However, positive PST was not associated with 30-day mortality. Pulmonary complications, congestive heart failure, and intensive care unit stay >3 days were also not associated with PST result. The overall survival rate at 1-year was 93.3% and 92.0% for patients with negative and positive PSTs (P = 0.343), respectively. After adjusting for potential confounders, postoperative MI was associated with increased hazards of death at 1 year (adjusted hazard ratio = 3.62 [95% confidence interval: 2.21-5.92], P < 0.001).CONCLUSIONThis study emphasizes the important of appropriate preoperative planning. A positive stress test before open aortic reconstruction for aortoiliac occlusive disease is associated with increased risk of postoperative MI. Although this was neither associated with increased risk of 30-day nor 1-year mortality, the negative impact of MI on patient functionality, quality of life, and 1-year death cannot be ignored.

Arbuscular mycorrhizal fungi promote functional gene regulation of phosphorus cycling in rhizosphere microorganisms of Iris tectorum under Cr stress

The mutualistic symbiotic system formed by clumping arbuscular mycorrhizal fungi (AMF) and plants can remediate heavy metal-contaminated soils. However, the specific mechanisms underlying the interaction between AMF and inter-root microbial communities, particularly their impact on organic phosphorus (P) cycling, remain unclear. This study investigated the gene regulation processes involved in inter-root soil phosphorus cycling in wetland plants, specifically Iris tectorum, following inoculation with AMF under varying concentrations of chromium (Cr) stress. Through macro-genome sequencing, we analyzed the composition and structure of the inter-root soil microbial community associated with Iris tectorum under greenhouse pot conditions. The results demonstrated significant changes in the diversity and composition of the inter-root soil microbial community following AMF inoculation, with Proteobacteria, Actinobacteria, Chloroflexi, Acidobacteria, and Bacteroidetes being the dominant taxa. Under Cr stress, species and gene co-occurrence network analysis revealed that AMF promoted the transformation process of organic phosphorus mineralization and facilitated inorganic phosphorus uptake. Additionally, network analysis of functional genes indicated strong aggregation of (pstS, pstA, pstC, TC.PIT, phoR, pp-gppA) genes, which collectively enhanced phosphorus uptake by plants. These findings shed light on the inter-root soil phosphorus cycling process during the co-remediation of Cr-contaminated soil by AMF-Iris tectorum symbiosis, providing valuable theoretical support for the application of AMF-wetland plant symbiosis systems to remediate heavy metal-contaminated soil.

Restoring soil quality in semi-arid mining-degraded soils: Effects of different combinations of organic amendments on microbial nutrient cycling after 40 months of application

Medium-term effects of different organic amendments on the recovery of mining-degraded soils in a semi-arid limestone quarry were evaluated. Five organic amendments, including composts (garden pruning and greenhouse residues) and stabilised sewage sludge (alone and in mixtures), were compared to untreated soils and natural reference soils. After 40 months, different soil physico-chemical properties, total nutrient (organic carbon -C-, nitrogen -N- and phosphorus -P-) and labile P and N fractions were analysed together with bacterial functional groups catalysing major steps in P (phoD, appA, phnX, pstS) and N turnover (chiA, archaeal amoA, bacterial amoA, nirS, nirK, nosZ, nifH), as well as total bacterial biomass. Restoration altered soil properties, including decreasing pH by up to 10% and increasing total organic C (up to 3.54%), total N (up to 0.33%) and total P (up to 0.18%). Labile P- and N-fractions increased significantly, with ammonium and nitrate doubling in some cases. Microbial activity also rose significantly, with bacterial biomass and functional genes involved in P (phoD, pstS) and N turnover (chiA, nirS, nosZ) increasing 2-3000 times compared to non-restored soils. Sewage sludge had the most pronounced effect on physico-chemical properties, nutrient content and functional groups abundance, while greenhouse compost produced conditions resembling natural reference soils. These results demonstrated that organic amendments can rehabilitate degraded soils by enhancing nutrient content and bacterial community potential for N and P turnover. Organic amendments are thus a viable strategy for medium-term restoration of degraded soils in semi-arid climates.
