Dive Brief:The Foundation for the National Institutes of Health has selected eight rare diseases for its gene therapy research partnership with drugmakers and nonprofits aimed at speeding new therapies path to clinical trials.Launched in 2021, the Bespoke Gene Therapy Consortium has chosen to focus on three ocular, three neurological and two systemic rare conditions from an initial list of 62 nominated diseases, it said Tuesday.The diseases include a type of retinal degeneration caused by mutations to the NPHP5 gene, a neurological disorder called Charcot-Marie-Tooth disease type 4J that damages the peripheral nerves, and multiple sulfatase deficiency that affects the breakdown of sugars and fats.Dive Insight:After decades of setbacks and slow progress, gene therapies for inherited diseases are entering commercial markets at an accelerating pace. Since 2017, the U.S. has approved five such therapies, with three cleared since August, and several more up for Food and Drug Administration decisions this year.While the progress is considerable, the treatments cover only a small fraction of about 4,000 single-gene rare diseases that could be treated by gene therapy.A major challenge is the complexity, time and cost of developing, testing and producing gene therapies as underscored by the long path for biotechnology company Bluebird bio to win approvals of two therapies and the close vote last week by a panel of FDA advisers to back biotech Sarepta Therapeutics treatment for Duchenne muscular dystrophy.The public-private consortium, with nearly $100 million in financial and in-kind commitments from more than 30 organizations, government agencies and companies, aims to speed the process for developing therapies that use adeno-associated viral vectors, or AAVs.With this clinical trial portfolio, we can build the bridge that creates a standardized and publicly available roadmap for all AAV gene therapies to follow, with repeatable solutions for templates, regulatory files, and manufacturing processes, said Courtney Silverthorn, the associate vice president of science partnerships at the foundation.Also among the selected diseases were rare eye conditions known as congenital hereditary endothelial dystrophy and retinitis pigmentosa 45, the neurological disorder spastic paraplegia 50, and two systemic conditions known as Morquio A Syndrome and propionic acidemia that affect the bodys ability to break down certain chemicals.The consortiums industry partners include large drugmakers like Biogen, Johnson & Johnson, and Pfizer, gene therapy specialists Spark Therapeutics and Regenxbio, and contract manufacturers like Thermo Fisher Scientific.Nonprofit groups include the Alliance for Regenerative Medicine, the American Society of Gene and Cell Therapy, CureDuchenne, the National Organization for Rare Disorders and the National Institute for Innovation in Manufacturing Biopharmaceuticals.At least five gene therapies for rare diseases could be approved in the U.S. this year including potential treatments for sickle cell disease, Duchenne muscular dystrophy and hemophilia A.Correction: A previous version of the story misstated how many partners the consortium has, and misidentified AbbVie as one. TumorGen and PhenoVista Biosciences partnered to explore metastatic cancer cell clusters to validate technology targeting these clusters and preventing cancer metastasis, which causes 90% of cancer-related deaths. Using PhenoVista’s analytical imaging technology, the two companies, who partnered last year, reported TumorGen’s micro-fluidic technology platform was able to detect multiple metastatic clusters from several lung cancer patients. The platform showed “tremendous sensitivity and specificity,” which proved to be more effective than previous efforts to collect and characterize the clusters. The clusters contained cancerous and non-cancerous cells, James Evans, CEO of PhenoVista, noted in the announcement. The companies announced the advancements could lead to improved therapeutic options for these patients. The study, which the National Cancer Institute supported, notes that these metastasized clusters can be captured. This will allow for multiple analyses that could identify weaknesses and potential therapeutic approaches, which TumorGen dubbed “cluster busters.” This therapeutic could then be able to target the cells and prevent metastasis. Jeffrey K. Allen, founder and president of TumorGen, noted in the company’s announcement that there is a “tremendous unmet medical need” for anti-metastatic therapies that can prevent tumors from forming across the body. He said the company’s platform is able to identify where these metastatic cancer cell clusters are vulnerable, enabling the development of those needed therapeutics. TumorGen plans to partner with oncology-focused companies to bring new therapies to market. Gene Therapy Developed for Rare Blindness Disease Those two companies were not the only ones to offer new seeds of understanding. Researchers from the National Institutes of Health have developed a potential new gene therapy approach to Leber congenital amaurosis (LCA), a disease that causes blindness in children. A research team from the National Eye Institute, a division of the NIH, revealed that mutations cause a type of LCA in the NPHP5 gene, which leads to defects in the primary cilium found in cells throughout the body, Technology Networks reported. A deficiency in NPHP5 can cause blindness in children. However, the team explained that deficiency could lead to kidney disease in severe cases. Defects in 25 different genes can cause LCA. The FDA has approved Spark Therapeutics gene therapy Luxturna for the treatment of patients with confirmed biallelic RPE65 mutation-associated retinal dystrophy, a form of LCA. That leaves other forms of this disease without treatment. Using stem cells collected from two patients with NPHP5 deficiency, the NIH team generated retinal organoids, which possess functional features of the natural retina. A deficiency of NPHP5 was found in the retinal organoids, along with lower levels of the protein CEP-290, which works alongside NPHP5. Using an adeno-associated viral vector that contained a functional version of NPHP5, the NIH researchers were able to target these extracted stem cells and demonstrate a “significant restoration of opsin protein concentrated in the proper location in outer segments.” The NIH research warrants further study and suggests a functional protein could prevent blindness in these children. Further research is warranted, particularly in more severe forms of the disease.
