AKI is classified as stages 1, 2 and 3 depending on the degree of kidney injury. In the presence of AHRF, stage 2 and stage 3 AKI, both classified as severe, put patients at a 50% or greater risk for death while hospitalized and in the 90 days after discharge.1 Survivors of severe AKI may develop or progress to chronic kidney disease, leading to an eventual need for dialysis. There are approximately 1.1 million patients in the United States suffering from stage 2 and 3 AKI over half of whom have associated AHRF. There are currently no approved therapies for AKI. KOURAGE is a randomized, double-blind, placebo-controlled study that will evaluate 150 patients with stage 2 and 3 AKI who have AHRF and are receiving oxygen by non-invasive mechanical ventilation, high flow nasal cannula or intermittent mandatory ventilation (IMV). Patients will be stratified by classification of stage of AKI as well as the use of IMV. Patients will receive either a four-hour infusion of Auxora or placebo at 1.25 mL/kg as a first dose, after which they will receive Auxora or placebo at 1.0 mL/kg at hours 24, 48, 72 and 96. The primary endpoint of the trial will be evaluation of patients through day 30 to determine days alive, ventilator-free and dialysis-free. Secondary endpoints will include a composite of all-cause mortality, decrease in estimated glomerular filtration rate (eGFR), and the incidence of dialysis over a period of 90 days, also known as MAKE-90 (Major Adverse Kidney Events at 90 days). "The rationale for and design of our Phase 2 KOURAGE study is supported by our prior clinical trials and pre-clinical studies that have indicated the potential benefits of Auxora for the treatment of AKI," said Sudarshan Hebbar, M.D., Chief Medical Officer of CalciMedica. AKI is a common consequence of severe COVID-19 pneumonia and in CalciMedica's CARDEA trial, which studied Auxora in patients with severe and critical COVID-19 pneumonia, results showed a nearly 40% reduction in reported AKI in Auxora-treated patients as compared to placebo-treated patients. In a post-hoc analysis of CARDEA patients with compromised kidney function (eGFR< 60 mL/min/1.73 m2) at enrollment, the drug was well tolerated and there was a survival benefit for patients treated with Auxora compared to those on placebo. Biomarker analysis from blood samples taken from over 190 CARDEA patients showed that Auxora increased Angiopoietin-1 while decreasing Angiopoietin-2, suggesting stabilization of the endothelium and the potential to treat AKI. Finally, published work from others showed that elevated serum IL–17 levels, a CRAC channel-mediated cytokine, were differentially elevated in critically ill patients with stage 2 and 3 AKI when compared to those without AKI, and the elevation was independently associated with both hospital mortality and long-term adverse outcomes.2 CalciMedica's initial pre-clinical studies in an ischemia/reperfusion injury (IRI) model of AKI were encouraging. A single dose of Auxora after IRI increased GFR by 61% and decreased mononuclear (inflammatory) cell infiltration by 30%. About AKI Acute kidney injury (AKI) denotes a sudden reduction in kidney function, or the organ's ability to clean and filter the blood, as measured by increased serum creatinine (a cellular waste product) or decreased urine volume. Regeneron's U.S. sales for macular degeneration treatment Eylea fell from $6.3 billion in 2022 to $5.9 billion in 2023, as Roche's Vabysmo has quickly become a powerhouse in the indication. One day after Roche said it was starting to establish Vabysmo as the new standard of care in age-related macular degeneration (AMD) and diabetic macular edema (DME), Regeneron clapped back with the same claim about its new treatment—high-dose Eylea.   “We continue to see physicians prescribe Eylea HD in both treatment experienced and treatment naïve settings as Eylea HD is increasingly recognized as the new standard of care,” Marion McCourt, Regeneron’s commercial chief, said as the company presented its fourth-quarter and 2023 earnings. Eylea HD is Regeneron’s response to Vabysmo, which hit the market in the first quarter of 2022 as a game-changing, longer-acting treatment. Vabysmo allowed many patients to reduce dosing from six times a year—with Regeneron’s original version of Eylea—to three times a year. Vabysmo has been an immediate hit as illustrated by Roche’s sales figures, which were presented on Thursday. In its first full year on the market, the VEGF and Ang-2 inhibitor generated sales of 2.36 billion Swiss francs ($2.7 billion). Meanwhile, U.S. sales of Regeneron’s Eylea franchise have taken a hit, falling from $6.3 billion in 2022 to $5.9 billion last year. Eylea HD—which matches the duration of Vabysmo and was approved in August—accounted for $166 million in sales over the last four months of 2023. On Friday, McCourt said that more than two-thirds of eligible patients are covered to receive Eylea HD, but the company doesn’t expect a significant sales surge in the first quarter of this year because its recently approved permanent J-code will not kick in until April 1, allowing for more ready access to the treatment. J-codes are part of an identifying system that allows pharmacies, hospitals and physicians to bill for medications. Roche’s numbers indicate that Eylea HD has not slowed the momentum for Vabysmo. On Thursday, the Swiss company said it now has a 22% market share in AMD and a 15% share in DME, both up three percentage points since August. Additionally, nearly half of all patients who are starting on Vabysmo are new to any treatment, which is up from a percentage range in the teens at the beginning of 2023. This statistic backs Roche’s claim that Vabysmo is becoming the new standard of care, according to the company’s new pharma chief Teresa Graham. Regeneron is answering with clinical results, which indicate Eylea HD can extend the dosing interval for many patients beyond every four months, and from strong early reports from physicians.   “The experience in the market to date with Eylea HD has been very favorable for physicians and that is probably the most important differentiating characteristic of the product,” McCourt said. Regeneron has no such concerns with its bell cow Sanofi-partnered Dupixent, which saw sales up 31% to $3.22 billion in the fourth quarter and up 33% over the year to $11.6 billion. The company anxiously awaits an FDA decision on Dupixent to expand its label to treat COPD. “Dupixent led in new-to-brand prescription share in the United States across all five of its approved indications,” said Regeneron CEO Len Schleifer. The metric is a key indicator pointing to future growth. Regeneron’s revenue for the fourth quarter came in at $3.43 billion, which topped consensus estimates comfortably and was a 1% bump sequentially. Excluding sales of its COVID-19 antibody cocktail REGEN-COV, it was a 14% increase year over year. For the full year, Regeneron scored $13.1 billion in revenue, up 8%. The company did not provide a revenue estimate for 2024. Cancer drug Libtayo routed consensus estimates with a $244 million haul in the fourth quarter, which was up 44%. Vabysmo sustained robust drying of retinal fluid, often associated with distorted or blurry vision Up to 60% of people receiving Vabysmo were able to extend treatment intervals to three or four months apart Detailed results from two global Phase III RVO studies will be presented at Angiogenesis, Exudation, and Degeneration 2024 Vabysmo is approved in the US for RVO, and in more than 90 countries around the world for people living with nAMD and DME 1 February 2024 - Roche (SIX: RO, ROG; OTCQX: RHHBY) announced today new 72-week data from two global Phase III studies, BALATON and COMINO, evaluating Vabysmo® (faricimab) in macular edema due to branch and central retinal vein occlusion (BRVO and CRVO).1,2 Whereas available RVO treatments are typically given every one to two months, the data showed nearly 60% of people receiving Vabysmo in BALATON and up to 48% of people in COMINO were able to extend their treatment intervals to three or four months apart.1,2,3,4 In addition, patients in the studies maintained vision gains and robust retinal drying achieved in the first 24 weeks of the studies for more than one year. Retinal drying is an important clinical measure as swelling from excess fluid in the back of the eye has been associated with distorted and blurred vision.5 In both studies, Vabysmo was well tolerated and the safety profile was consistent with previous studies. “This is the first time that vision and anatomical improvements have been maintained for more than a year in global Phase III studies for both branch and central retinal vein occlusion,” said Levi Garraway, M.D., Ph.D., Roche’s Chief Medical Officer and Head of Global Product Development. “These long-term results build on the strong clinical and real-world data reinforcing Vabysmo as an effective treatment option for people affected by retinal conditions that can cause vision loss.” Results will be presented virtually on 3 February 2024 at Angiogenesis, Exudation, and Degeneration 2024, organised by Bascom Palmer Eye Institute in Florida, United States (US). “The sustained vision improvements and retinal drying seen up to 72 weeks reaffirm Vabysmo as an effective treatment for retinal vein occlusion,” said Ramin Tadayoni, M.D., Ph.D., head of ophthalmology at the Cité University in Paris, France, and president of EURETINA, who is presenting the data at Angiogenesis. “More treatment options are needed to better serve people living with this condition, and these data show Vabysmo can potentially improve outcomes while reducing the number of clinic visits needed.” Both studies evaluated the average change in best-corrected visual acuity (BCVA) score (the best distance vision a person can achieve – including with correction such as glasses – when reading letters on an eye chart) from baseline. The studies also tracked the amount of swelling in the back of the eye due to retinal fluid, as measured by central subfield thickness (CST). Reductions in CST indicate improvement. Overall, results showed the vision improvements and reductions in retinal fluid achieved in the first 24 weeks of the studies were maintained up to 72 weeks. Results for BRVO (BALATON)1: Vision gains: At 72 weeks, people receiving Vabysmo as a first-line treatment gained 18.1 letters on the eye chart, while people switched from aflibercept to Vabysmo gained 18.8 letters. During the first 24 weeks, vision gains were +16.8 eye chart letters in people receiving Vabysmo and +17.5 letters in people receiving aflibercept. Retinal drying: At 72 weeks, people receiving Vabysmo as a first-line treatment saw a 310.9 µm reduction in retinal swelling, as measured by CST, while those switched from aflibercept to Vabysmo saw a reduction in CST of 307 µm. During the first 24 weeks of the study, CST reductions were 314.5 µm in people receiving Vabysmo and 307.6 µm in people receiving aflibercept. Results for CRVO (COMINO)2: Vision gains: People receiving Vabysmo as a first-line treatment gained 16.9 eye chart letters at 72 weeks, while people switched from aflibercept to Vabysmo gained 17.1 eye chart letters. During the first 24 weeks of the study, vision gains were +16.9 eye chart letters in people receiving Vabysmo and +17.3 letters in people receiving aflibercept. Retinal drying: People receiving Vabysmo as a first-line treatment saw a 465.9 µm reduction in retinal swelling, as measured by CST, while those switched from aflibercept to Vabysmo saw a reduction in CST of 460.6 µm at 72 weeks. During the first 24 weeks of the study, reductions in CST were 462.3 µm in people receiving Vabysmo and 447.8 µm in people receiving aflibercept. Vabysmo is the first and only bispecific antibody approved for the eye, uniquely engineered to target and inhibit two signalling pathways, which are linked to a number of vision-threatening retinal conditions, by neutralising angiopoietin-2 (Ang-2) and vascular endothelial growth factor-A (VEGF-A) to restore vascular stability.6-9 To date, Vabysmo is approved in more than 90 countries around the world for people living with neovascular or ‘wet’ age-related macular degeneration (nAMD) and diabetic macular edema (DME), with public reimbursement in over 25 markets and more than 2.5 million doses distributed globally.10 In October 2023, the U.S. Food and Drug Administration approved Vabysmo for the treatment of macular edema following RVO.6 Data up to 72 weeks from the BALATON and COMINO studies have been submitted to health authorities around the world, including the European Medicines Agency, for the treatment of macular edema following RVO. BALATON (NCT04740905) and COMINO (NCT04740931) are two randomised, multicentre, double-masked, global Phase III studies evaluating the efficacy and safety of Vabysmo® (faricimab) compared to aflibercept. For the first 20 weeks, people were randomised 1:1 to receive monthly injections for six months of either Vabysmo (6.0 mg) or aflibercept (2.0 mg). From weeks 24 to 72, all individuals received Vabysmo (6.0 mg) up to every four months, using a treat-and-extend dosing regimen. The BALATON study was conducted in 553 people with branch retinal vein occlusion. The COMINO study was conducted in 729 people with central retinal or hemiretinal vein occlusion. The primary endpoint of each study was change in best-corrected visual acuity (BCVA) from baseline at 24 weeks. Secondary endpoints for weeks 0-24 of the studies included change in central subfield thickness (CST) and drying of retinal fluid, from baseline over time up to week 24. Secondary endpoints for weeks 24-72 of the studies assessed change in BCVA from baseline, change in CST from baseline and the proportion of individuals on treat-and-extend intervals. RVO is the second most common cause of vision loss due to retinal vascular conditions. It affects an estimated 28 million adults globally, mainly those aged 60 or older, and can lead to severe and sudden vision loss.11,12 The level of angiopoietin-2 (Ang-2) is elevated in RVO and it is thought that increased Ang-2 expression drives disease progression, alongside vascular endothelial growth factors (VEGF).13,14 RVO typically results in sudden, painless vision loss in the affected eye because the vein blockage restricts normal blood flow in the affected retina, resulting in bleeding, fluid leakage and retinal swelling called macular edema.12,15,16 Currently, macular edema due to RVO is typically treated with repeated intravitreal injections of anti-VEGF therapies.15 There are two main types of RVO: branch retinal vein occlusion, which affects more than 23 million people globally and occurs when one of the four smaller ‘branches’ of the main central retinal vein becomes blocked; and central retinal vein occlusion, affecting more than four million people worldwide, and occurs when the eye’s central retinal vein becomes blocked.11,16 Roche has a robust Phase III clinical development programme for Vabysmo. The programme includes AVONELLE-X, an extension study of TENAYA and LUCERNE, evaluating the long-term safety and tolerability of Vabysmo in neovascular or ‘wet’ age-related macular degeneration (nAMD), and RHONE-X, an extension study of YOSEMITE and RHINE, evaluating the long-term safety and tolerability of Vabysmo in diabetic macular edema (DME).17,18 Roche has also initiated several Phase IV studies, including the ELEVATUM study of Vabysmo in underrepresented patient populations with DME, the SALWEEN study of Vabysmo in a subpopulation of nAMD highly prevalent in Asia, as well as the VOYAGER study, a global real-world data collection platform.19-21 Roche also supports several other independent studies to further understand retinal conditions with a high unmet need.10 Vabysmo is the first bispecific antibody approved for the eye.6,7 It targets and inhibits two signalling pathways linked to a number of vision-threatening retinal conditions by neutralising angiopoietin-2 (Ang-2) and vascular endothelial growth factor-A (VEGF-A).
