Introduction

Mucocellular carcinoma of the ovary, usually metastatic from the gastrointestinal tract, characterized by areas of mucoid degeneration and the presence of signet-ring-like cells. It accounts for 30%-40% of metastatic cancers to the ovaries and possibly 1%-2% of all malignant ovarian tumors. The lesions may not be discovered until the primary disease is advanced, and most patients die of their disease within a year. In some cases, a primary tumor is not found. Tirellizumab Combined With SOX and Hyperthermic Intraperitoneal Chemotherapy (HIPEC) in the Treatment of Peritoneal Metastatic Gastric/Gastroesophageal Junction Adenocarcinoma: A Single-arm, Phase II Clinical Trial (Solids-03)

For peritoneal metastatic gastric/gastroesophageal junction adenocarcinoma (cT3-4NanyM1), PD-1 antibody combined with chemotherapy and hyperthermic intraperitoneal chemotherapy (HIPEC) can downstage tumor stage, increase the conversion resection rate, and may improve the long-term survival. Tislelizumab, an anti-PD-1 antibody, has recently been proved in the first- and second-line standard treatment for advanced or metastatic gastric or gastro-oesophageal junction adenocarcinoma.In the subgroup analysis of RATIONALE-305 trial, tislelizumab also showed good efficacy in gastric/gastroesophageal junction adenocarcinoma patients with peritoneal metastasis. Combination of tirellizumab,SOX and HIPEC for peritoneal metastatic gastric/gastroesophageal junction adenocarcinoma could be a novel therapeutic strategy to increase response rate and therapeutic efficacy. This study is a monocenter, single-arm phase 2 clinical trial to evaluate tolerability, safety and efficacy of perioperative tirellizumab in combination with SOX and HIPEC in peritoneal metastatic gastric/gastroesophageal junction adenocarcinoma.

A Phase 1 First-In-Human Study of the Anti-CD73 IPH5301 Alone or in Combination With Chemotherapy and Trastuzumab in Patients With Advanced Solid Tumors

CHANCES-IPC 2021-008 is First In Human, Phase I, multicenter, European study evaluating an anti-CD73, IPH5301 in advanced and/or metastatic cancer.
The trial will be conducted in two parts, Part I- Dose escalation: This part aims to identify the maximum tolerated dose (MTD) of IPH5301 agent in monotherapy and recommended phase 2 dose (RP2D) for future trials, followed by a safety expansion study part cohort.
Part II- Expansion cohort: A total of 12 HER2+ cancer patients, respectively 6 breast cancer patients and 6 gastric cancer patients, is planned to be enrolled into the next expansion cohort to select a recommended dose of IPH5301 to be administered in combination with chemotherapy and trastuzumab for evaluation in future trials with selected advanced solid tumors.

Efficacy and safety of subtotal pelvic peritonectomy for colorectal cancer patients with peritoneal metastasis confined to the pelvic cavity

BACKGROUNDCytoreductive surgery has shown survival benefits for colorectal cancer (CRC) patients with peritoneal metastasis. However, the optimal extent of peritonectomy remains controversial in cases of limited peritoneal metastases. This study modified selective pelvic peritonectomy (SPP) into subtotal pelvic peritonectomy (STPP) for metastasis confined to pelvic cavity, and aimed to evaluate its feasibility, safety, and impact on survival outcomes.MATERIALS AND METHODSCRC patients with limited peritoneal metastasis confined to the pelvic cavity who underwent CC0 (no macroscopic residual cancer remained) resection were included from a prospectively collected database. Surgical complications, disease-free survival (DFS), and overall survival (OS) were analyzed.RESULTSA total of 67 patients were included (26 in the STPP group and 41 in the SPP group). Clinically, STPP was found to be feasible and without increased surgical complications or mortality rates. At a median follow-up of 33.9 months, the 3-year DFS was 65.9 % and 30.7 % in STPP and SPP groups, respectively (P= 0.002). The 3-year OS was 84.1 % and 68.5 % in STPP and SPP groups, respectively (P= 0.006). Moreover, STTP was independently associated with improved DFS (HR = 0.351, 95 % CI 0.165-0.745, P= 0.006) and OS (HR = 0.324, 95 % CI 0.116-0.902, P=0.032). Female gender was also independently associated with poor DFS (HR = 2.146, 95 % CI 1.078-4.271, P= 0.031). Among 24 female patients with remaining ovaries, 9 (37.5 %) cases developed metachronous ovarian metastasis, and of these 6 underwent a second operation.CONCLUSIONSSubtotal pelvic peritonectomy is associated with promising long-term outcomes in CRC patients with peritoneal metastasis confined to the pelvic cavity. Prophylactic bilateral oophorectomy should be strongly considered during cytoreductive surgery.

Krukenberg tumor secondary to gallbladder adenocarcinoma: A case report

Krukenberg tumour is defined by the development of metastasis in both ovaries, characterised by the presence of mucine secreting cells, hence the name carcinoma mucocellular; However, the term "Krukenberg" has often been broadly applied to any metastases to the ovaries, irrespective of the site of origin. It is uncommon representing about 1%-2% of all ovarian tumours. It is in general secondary to a gastric adenocarcinoma but other sites have been found to be responsible of krukenberg's tumour, notably large bowels adenocarcinoma, pancreas, breast cancer, lung adenocarcinoma and biliary tract cholangiocarcinoma. In this article we will report the case of a 29 years old patient presenting with metachronous krukenberg tumour and peritoneal carcinosis with a history of surgery for a gallbladder adenocarcinoma 9 month prior. This case study sheds light on this pathology, familiarizing us with its clinical and radiological picture, as well as its prognosis.

Unexpected encounters: The surprising link between ovarian cancer and axillary lymphadenopathy

Ovarian tumors represent a significant health concern for women, often leading to morbidity and mortality. We present the case of a 46-year-old patient without notable personal or family history of tumors, who initially presented with bilateral axillary lymphadenopathy. Given the clinical presentation and suspicion of a Cancer of Unknown Primary (CUP) syndrome, the patient underwent imaging examination, such as ultrasound of the axilla, mammography, and breast magnetic resonance imaging. The axillary tru-cut biopsy histological examination revealed ovarian metastasis. The total body computed tomography (CT) scan and pelvic MRI confirmed the presence of a left ovarian tumor. The patient underwent surgical intervention with excision of the tumor and axillary lymph node dissection. Histological analysis confirmed the diagnosis of high-grade serous ovarian carcinoma. The patient was initiated on adjuvant chemotherapy and a long-term follow-up was scheduled. This case underscores the importance of comprehensive diagnostic evaluation in patients presenting with suspicious symptoms, such as axillary lymphadenopathy, to ensure timely and appropriate management of ovarian carcinoma and related conditions.
