A variety of analgesic strategies are available following mixed hemorrhoids surgery, including pharmacological interventions (7), acupuncture, moxibustion, and electroacupuncture . Medications such as opioid analgesics and non-steroidal anti-inflammatory drugs (NSAIDs) are commonly used but can be associated with side effects including nausea, vomiting, and gastrointestinal bleeding, with long-term use potentially leading to addiction (8). Complementary therapies require skilled administration and regular treatment sessions.
Methylene blue (MB), a cationic thiazine dye extensively utilized as a biological stain and chemical indicator, has been increasingly recognized for its potential analgesic properties (9). In the present study, methylene blue infiltrating injection (MBI) was administered to treat anal pain resulting from Milligan-Morgan surgery, with the aim of assessing its analgesic efficacy and safety profile.

Recovery of left ventricular function after surgery for aortic and mitral regurgitation with heart failure

BackgroundSevere aortic regurgitation (AR) and mitral regurgitation (MR) can lead to left ventricular (LV) systolic dysfunction; however, there are limited data about recovery of LV after surgery for AR or MR. Little is known to guide the management of combined AR and MR (mixed valvular heart disease [VHD]). This study is sought to investigate the predictors of postoperative LV function recovery in left-sided regurgitant VHD with reduced left ventricular ejection fraction (LVEF), especially for mixed VHD.MethodsFrom 2010 to 2020, 2053 adult patients underwent aortic or mitral valve surgery at our center. The patients with valvular stenosis, infective endocarditis, concomitant revascularization, and preoperative LVEF ≥40 % were excluded. A total of 127 patients were included in this study: 22 patients with predominant AR (AR group), 64 with predominant MR (MR group), and 41 with combined AR and MR (AMR group).ResultsThe mean preoperative LVEF was 32.4 %, 30.7 %, and 30.2 % (p = 0.44) in the AR, MR, and AMR groups, respectively. The AR group was more likely to have postoperative LVEF recovery. The cut-point of left ventricular end-systolic diameter (LVESD) for better recovery was 49 mm for the MR group and 58 mm for the AMR group.ConclusionLV dysfunction due to combined AR and MR has similar remodeling reserve as AR, and better recoverability than MR. Thus, double-valve surgery is recommended before the LVESD is > 58 mm.

Effect of Bixie combined with perioperative multimodal analgesia management on postoperative pain and wound healing of mixed hemorrhoids.

We were to explore the effect of Bixie combined with perioperative multimodal analgesia management on postoperative pain and wound healing of mixed hemorrhoids. A total of 240 patients with mixed hemorrhoids in this hospital from June 2022 to September 2023 were chosen, with 120 in the control group receiving perioperative multimodal analgesia management and 120 in the observation group receiving Bixie combined with perioperative multimodal analgesia management. The results showed that compared with the control group, the observation group had lower visual analogue scale (VAS) scores at 1, 3, 5 and 7 days after the surgery; the 5-hydroxytryptamine (5-HT), prostaglandin E2 (PGE2), nitric oxide (NO), interleukin-2 (IL-2), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) of the observation group at 24 hours and 72 hours after the surgery were lower; the vascular endothelial growth factor (VEGF), basic fibroblast growth factor (bFGF), platelet-derived growth factor (PDGF) and wound healing rate of the observation group at 7 days after the surgery were higher; the total effective rate of the observation group was higher. Therefore, combining Bixie with perioperative multimodal analgesia management could improve postoperative pain, decrease inflammatory factor levels, promote angiogenesis and accelerate wound healing in patients with mixed hemorrhoids.
