Introduction

Scaly papule or warty growth, caused by five fungi, that spreads as a result of satellite lesions affecting the foot or leg. The extremity may become swollen and, at its distal portion, covered with various nodular, tumorous, verrucous lesions that resemble cauliflower. In rare instances, the disease may begin on the hand or wrist and involve the entire upper extremity. (Arnold, Odom, and James, Andrew's Diseases of the Skin, 8th ed, p362)

Retrospective Descriptive Observational Study of the Epidemiological, Clinical and Therapeutic Profile of Patients With Rare Infections of Dermatological Interest at Three Major Reference Hospitals in Costa Rica From 2019-2023

In Costa Rica there are some descriptive studies of some of these rare infectious diseases (sporotrichosis, chromomycosis and mycetomas); however, there are no recent published reports. Most of the publications on the subject date back to the previous century; from 2000 to the present year there are only publications of small series of cases, so the current behavior of these skin infections is unknown. The primary objective is to analyze the epidemiological, clinical, and therapeutic profile and determine the risk factors in patients with a diagnosis of rare infections of dermatological interest treated at Hospital México, Hospital Rafael Calderón Guardia, and Hospital San Juan de Dios during the period 2019-2023.

Effects of ALA-PDT on the murine footpad model of Fonsecaea monophora infection and its related mechanisms in vivo

BACKGROUND5-aminolevulinic acid photodynamic therapy (ALA-PDT) has received growing attention for treating chromoblastomycosis (CBM) and has shown efficacy in a handful of clinical case reports. However, there is insufficient information regarding the effects of ALA-PDT on Fonsecaea monophora in mouse infection model and the related mechanisms. This study investigated these issues in vivo.METHODSA F. monophora infection mouse model inoculated in footpads was used. Changes in the footpad volume, tissue fungal burden, and histopathological characteristics were investigated to determine the efficacy of ALA-PDT. Scavenger receptor MARCO (Macrophage receptor with collagenous structure) was further evaluated at the gene and protein levels. Serum cytokines TNF-α, GM-CSF, IL-4, and IL-10 were measured using enzyme-linked immunosorbent assay to indicate changes in the immune microenvironment after PDT.RESULTSALA-PDT reduced infected footpad volume, fungal burden, and pathological inflammatory infiltration in vivo. It also increased the expression of Marco in the murine infection model. Furthermore, PDT upregulated the anti-inflammatory cytokines IL-4 and IL-10 while downregulated the pro-inflammatory cytokines TNF-α and GM-CSF in mouse serum.CONCLUSIONSALA-PDT demonstrated fungicidal effects in a mouse footpad infection model with F. monophora and attenuated the inflammatory reactions. It may also assist against the intracellular fungi by the host through macrophage receptor MARCO and regulation of the immune microenvironment. This study provides scientific evidence for the protocol selection of ALA-PDT as a promising adjunctive modality for treating chromoblastomycosis.

Successful Treatment of Chromoblastomycosis caused by Cladophialophora carrionii with Itraconazole: Clinicopathology, Susceptibility, and Molecular Identification of a Case in Northern China

Chromoblastomycosis (CBM), a prevalent subcutaneous mycosis in tropical and subtropical regions caused by dematiaceous fungi, often presents as refractory lesions. This case report details a 63-year-old woman from Tianjin, China, with CBM caused by Cladophialophora carrionii after plant trauma. Successful treatment with itraconazole was performed. After 3 months, the lesions improved significantly. The case highlights the importance of accurate diagnosis and tailored antifungal therapy, as well as the need for long-term follow-up because of the chronic nature of the disease.

Simultaneous distinct cutaneous fungal infections with chromoblastomycosis due to Exophiala xenobiotica and hyalohyphomycosis due to Scedosporium apiospermum in a patient with severe cellular immunodeficiency

AbstractDeep‐seated dermatomycosis is a rare disease that is often caused by trauma and/or systemic immunodeficiency. We describe a case of chromoblastomycosis complicated by hyalohyphomycosis that occurred simultaneously at different sites. A 92‐year‐old Japanese man who had been taking oral prednisolone for an IgG4‐related respiratory disease visited our clinic. He developed brownish plaques with grayish‐white scales with pseudo‐carcinomatous hyperplasia and numerous brownish muriform cells developing in the dermis of his right hand, and multiple painful abscesses with pustules and papules and numerous hyphae within and around the histiocytes in the dermis of his right lower leg. Upon skin tissue culture and DNA sequencing, Exophiala xenobiotica and Scedosporium apiospermum were detected separately. He had severe cellular immunodeficiency indicated by low levels in the phytohemagglutinin (PHA)‐stimulated lymphocyte transformation test (LTT) and serum interferon‐gamma (IFN‐γ), although his humoral immunity was normal. The patient died of bacterial pneumonia, despite antifungal drug treatment for 2 months. IFN‐γ producing type 1 T helper (Th1) cells play an important role in the defense against fungal infections, however, corticosteroids specifically suppress Th1 cell responses and promote the induction of fungal infection. Measurement of PHA‐stimulated LTT and serum IFN‐γ may be useful in determining the severity and prognosis of deep‐seated dermatomycosis in patients undergoing corticosteroid treatment.
