Introduction

Common occlusive arterial disease which is caused by ATHEROSCLEROSIS. It is characterized by lesions in the innermost layer (ARTERIAL INTIMA) of arteries including the AORTA and its branches to the extremities. Risk factors include smoking, HYPERLIPIDEMIA, and HYPERTENSION.

[Application of minimally invasive osteotomy with preservation of blood supply to transversely transported bone segment in tibial transverse bone transport].

ObjectiveTo investigate the efficacy of minimally invasive osteotomy with preservation of blood supply to the transversely transported bone segment in the treatment of chronic ischemic lower limb diseases using tibial transverse bone transport.MethodsA retrospective analysis was conducted on the clinical data of 12 patients with chronic ischemic lower limb diseases who met the selection criteria and were treated between June 2016 and December 2023. The cohort included 7 males and 5 females, aged 26-87 years (mean, 61.2 years). Among them, 7 patients had diabetic foot (DF), including 2 cases complicated by arteriosclerosis obliterans (ASO), with a DF duration ranging from 1.7 to 23.0 months (mean, 9.6 months) and ulcer sizes ranging from 3.2 cm×2.4 cm to 10.0 cm×6.6 cm. Three patients had ASO with a disease duration of 1.7-23.0 months (mean, 10.4 months) and ulcer or post-amputation wound sizes ranging from 2.2 cm×2.1 cm to 12.0 cm×7.7 cm. Additionally, 2 patients with thromboangiitis obliterans (TAO) had a disease duration of 7 and 12 months, respectively. Preoperatively, the mean foot skin temperature was (27.63±0.34)℃, and the visual analogue scale (VAS) score was 8.5±0.7. All patients underwent tibial transverse bone transport using minimally invasive osteotomy while preserving the blood supply to the transported bone segment. Postoperative complications and foot ulcer healing were observed. The effectiveness of tibial transverse bone transport was evaluated by comparing preoperative and postoperative foot skin temperature, VAS scores, as well as through CT angiography (CTA).ResultsOne DF patient with ASO was lost to follow-up, while the remaining 11 patients were followed up 10-29 months (mean, 20.5 months). One DF patient had a displaced fracture at the site of tibial transport due to trauma, which healed after plaster immobilization; 1 DF patient had a small amount of pigmentation in the skin of the donor site, but the blood circulation was good; no complication such as pin tract infection, skin necrosis, or osteomyelitis occurred in the donor site of all patients. At 2-3 weeks postoperatively, the foot skin temperature was (34.21±0.65)℃, and the VAS score was 0.3±0.1, both significantly improved compared to preoperative ones ( t=-31.578, P<0.001; t=49.000, P<0.001). Two TAO patients experienced recurrent, persistent, intolerable pain at 7-10 days postoperatively, with no reduction in ulcer size or necrotic areas; 1 underwent toe amputation, and the other required a mid-leg amputation. The remaining patients achieved complete ulcer healing, with healing time ranging from 5.1 to 9.2 weeks (mean, 6.8 weeks). CTA showed no new vascular occlusions or embolisms. X-ray films at 4 months postoperatively demonstrated proper repositioning and good healing of the transported tibial bone segment.ConclusionThe application of minimally invasive osteotomy with preserved blood supply to the transversely transported bone segment in tibial transverse bone transport for the treatment of chronic ischemic lower limb diseases effectively minimizes extensive soft tissue dissection, reduces surgical soft tissue damage, and better maintains local blood supply. This approach lowers the risk of postoperative infection and skin necrosis while accelerating postoperative recovery.

Exploring the Novel Potential of Serum SIRT1 and TIMP3 as Biomarkers for In-Stent Restenosis Following Percutaneous Transluminal Angioplasty and Stenting in Arteriosclerosis Obliterans Patients

This study aimed to investigate the potential of serum Sirtuin 1 (SIRT1) and tissue inhibitor of metalloproteinase 3 (TIMP3) levels as biomarkers for in-stent restenosis (ISR) in arteriosclerosis obliterans (ASO) patients following percutaneous transluminal angioplasty (PTA) and stenting. A total of 256 ASO patients who underwent successful PTA with stent implantation were included. Serum levels of SIRT1 and TIMP3 were assessed at baseline and 4 weeks post-procedure. After 6 months, 65 patients were identified with ISR. Significant differences were noted in serum SIRT1 and TIMP3 levels between ISR and non-ISR groups at 4 weeks. TIMP3 had a higher AUC (0.782, 95% CI: 0.726-0.831) than SIRT1 (0.737, 95% CI: 0.678-0.789) for predicting ISR at 6 months. Correlation analysis showed a positive association between SIRT1 and TIMP3 levels in ISR patients at 4 weeks, but not in non-ISR patients. Multivariate analysis revealed diabetes (OR = 1.436, 95% CI: 1.205-1.925) and carotid stenosis (OR = 4.551, 95% CI: 1.364-15.185) significantly increased ISR risk, while lower SIRT1 (OR = 0.985, 95% CI: 0.978-0.992) and TIMP3 (OR = 0.574, 95% CI: 0.464-0.710) levels were significantly associated with ISR. Serum SIRT1 and TIMP3 levels at 4 weeks post-procedure are significant predictors of ISR in ASO patients following PTA and stenting. Lower SIRT1 and TIMP3 levels correlate with higher ISR risk. These findings suggest that monitoring serum SIRT1 and TIMP3 levels could be a valuable tool in predicting ISR, which could inform clinical decisions and patient management.

Therapeutic Erythrocytapheresis is Effective in Treating 2 Patients with High-Altitude Erythrocythemia

BACKGROUNDDue to environmental hypoxia on the high-altitude local residents often exhibit a compensative increase in hemoglobin concentration to maintain the body's oxygen supply. In certain people, the number of red blood cells continues to grow, resulting in high altitude polycythemia (HAPC) which is characterized by headache, disorientation, sleeplessness, and bone discomfort. HAPC is often associated with multiple complications, of which lower extremity arteriosclerosis obliterans (LEASO) is rare.METHODSHere we report two long-term Tibetan male patients living in plateau areas, who were diagnosed with HAPC complicated with LEASO. The diagnostic standards for HAPC are based on the Qinghai score, which is that the patient currently or once lived in high-altitude areas, and the results of conventional blood tests show that hemoglobin Hb > 21 g/dL in males and Hb > 19 g/dL in females. Due to severe complications caused by abnor-mally high hemoglobin levels, they underwent an individualized regimen of therapeutic erythrocytapheresis (TE).RESULTSAfter one TE treatment, both hemoglobin and hematocrit decreased significantly in 2 patients (Hb decreased from 20 g/dL and 21 g/dL to 17.6 g/dL and 18 g/dL in the two patients), alleviating the further deterioration of the disease caused by abnormal erythrocyte elevation.CONCLUSIONSTE can reduce the hemoglobin and hematocrit of HAPC patients in a timely and effective manner and play a clinical role in alleviating the disease progression of patients to a certain extent, especially for patients with complications. TE needs to be implemented in time.
