Introduction

A syndrome associated with inflammation of the BRACHIAL PLEXUS. Clinical features include severe pain in the shoulder region which may be accompanied by MUSCLE WEAKNESS and loss of sensation in the upper extremity. This condition may be associated with VIRUS DISEASES; IMMUNIZATION; SURGERY; heroin use (see HEROIN DEPENDENCE); and other conditions. Neuralgic amyotrophy of Parsonage and Turner. Which nerves are most frequently involved in daily practice? Data from 355 patients

OBJECTIVESThe aim of this study was to conduct a retrospective analysis of clinical patterns associated with neuralgic amyotrophy of Parsonage and Turner (NAPT) in a series of 355 patients seen in "ambulatory care".METHODSClinical, epidemiological and electrodiagnostic (EDX) data were collected by means of an electrodiagnosis consultation. Data were obtained on age, sex, body mass index, side involved, nerves involved, number of nerves involved per attack, number of attacks per patient, number of bilateral, recurrent, and hereditary cases.RESULTSMean age was 42.7 years, 218 patients (61%) were male, mean body mass index was 23.8; the right side was involved in 232 cases (65%). Inflammatory mononeuropathy (MN) was the most frequent elementary nerve lesion. It was unique (UMN) in 253 patients (71%), and multiple (MMN) in 102 (29%), with a total of 495 neuropathies, and a mean of 2.38 nerves involved in one patient with multiple mononeuropathy (MMN). Five nerves are more frequently involved: long thoracic (n=138), suprascapular (n=129), anterior interosseous (n=53), spinal accessory (SAN) (n=51), lateral antebrachial cutaneous (n=41) nerves. Other nerves (axillary, posterior interosseous, sensory median, phrenic, ulnar and medial antebrachial cutaneous nerves, and lumbo-sacral plexus) are less frequently involved, and less suggestive for NAPT diagnosis. Bilateral, recurrent and hereditary cases were observed in 22, 10 and 0 cases, respectively.CONCLUSIONSThe clinical patterns of NAPT in ambulatory care thorough an EDX consultation have showed that unique mononeuropathy (UMN) are more frequent than MMN, what results in milder cases; and that five nerves lesions are more frequent and more suggestive of NAPT.

Early aggressive constriction resection for hourglass-like constriction of peripheral nerve in the upper extremity: a retrospective study

OBJECTIVESThe hourglass like constriction (HGC) of peripheral nerves is a characteristic pathological manifestation of Neuralgic Amyotrophy. Once identified, early surgical intervention is essential. However, the method of surgery is controversial, particularly regarding whether HGC needs to be excised. This study aims to explore the efficacy of early aggressive resection of HGC in the upper limb nerves.MATERIALS AND METHODSThis retrospective study focuses on 13 nerves of spontaneous upper limb paralysis treated at our hospital from June 2019 to July 2023, in which HGC was identified during surgery. During surgery, epineurectomy and interfascicular neurolysis were performed on the constricted areas. Post-neurolysis, constriction excision was carried out if any of the following conditions were met: (1) A single constriction with constriction ≥75%. (2) Constriction combined with torsion. (3) The presence of ≥2 constrictions. Regular face-to-face follow-ups were conducted postoperatively.RESULTSFour cases with a single constriction of less than 75% underwent epineurotomy and interfascicular neurolysis; eight underwent constriction excision, of which four cases with a single constriction and associated torsion had direct end-to-end suturing after excision, and four had more than two constrictions treated with autologous sural nerve grafts. Postoperative follow-ups showed good recovery in all but one case, which had unique pathological features and had underwent only epineurectomy, showing moderate recovery.CONCLUSIONSFor early surgical treatment of HGCs in peripheral nerves of the upper limbs, if severe constriction, constriction combined with torsion, or the presence of more than two constrictions are identified during surgery, aggressive constriction resection may be a better option.

Endovascular versus open repair of ruptured abdominal aortic aneurysms with hostile neck anatomy

OBJECTIVEAneurysm neck anatomy in ruptured abdominal aortic aneurysms (rAAAs) is often complex, limiting the feasibility of endovascular repair (EVAR). The objective of this study was to compare the outcomes of EVAR and open surgical repair (OSR) for treatment of rAAAs in patients with hostile neck anatomy (HNA). The secondary aim was to review the clinical characteristics and anatomic risk factors predictive of mortality.METHODSA multi-center retrospective review was performed to identify patients with rAAAs and HNA between 2004 and 2021. HNA was defined as infrarenal aortic neck diameter >28 mm, infrarenal neck length <15 mm or angulation >60 degrees. The primary end point was 30-day all-cause mortality. Secondary end points included 90-day, 1-year and 5-year mortality. Preoperative computed tomography was analyzed using an Aquarius workstation. The Kaplan-Meier method was used to estimate survival, and univariate and multivariate Cox proportional hazard regression analysis was used to assess variables that influenced survival.RESULTS137 patients with rAAAs and HNA underwent infrarenal EVAR or OSR. Overall mean age was 74 ± 10 years and 72% were male. 85 patients (62%) underwent infrarenal EVAR and 52 (38%) underwent OSR. Mean aneurysm size at the time of rupture was 86 ± 22 mm. Patients who underwent OSR were more likely to present with a higher Garland preoperative risk score (P = .05), have a lower pH (P < .001), lower SBP (P < .001) and higher lactate (P = .005). Patients with an infrarenal neck length <15 mm were more likely to undergo OSR (EVAR 64% vs. OSR 87%, P = .004) and patients with an infrarenal neck angle >60 degrees were more likely to undergo EVAR (60% vs. 39%, P = .01). EVAR was associated with lower 30-day (17% vs. 27%; OR 0.6; 95% CI, 0.3-1.2; P = .14) and 90-day (22% vs. 33%; HR 0.6; 95% CI, 0.3-1.2; P = .17) all-cause mortality, however, this was not statistically significant. The overall median follow-up time was 19 (2-66) months. 1-year survival for EVAR and OSR were 75% and 64% (Log-rank P = .14) and 5-year survival for EVAR and OSR were 65% and 55% (Log-rank P = .28). Hemoglobin (P = .009), increasing calcification score (P = .002) and infrarenal neck length <10 mm (P = .01) were associated with all-cause mortality at 30-days for EVAR on multivariate Cox regression analysis. Lactate (P <.001) was the only variable associated with all-cause mortality at 30-days for OSR on multivariate Cox analysis.CONCLUSIONEarly and long-term survival favored EVAR in comparison to OSR in patients with rAAAs and HNA, however, this was not statistically significant. Calcification of the infrarenal neck and neck length <10 mm were associated with increased 30-day mortality for EVAR while no anatomic variables were specifically associated with 30-day mortality for OSR.
