Adenomyosis is a common disease in women aged 40-50 years. It associates with dysmenorrhea and menorrhagia. Hysterectomy was considered the main treatment that could definitively cure this disease. Other treatment options are increasingly offered, including hormonal suppression with gonadotropin-releasing hormone agonists or danazol and endometrial ablation. However, deep adenomyosis responds weakly to the above treatment options, which are commonly not considered for long-term management because of the associated side effects.
Dienogest is a progestin medication which is used in birth control pills and in the treatment of endometriosis and adenomyosis. Low-dose combined oral contraceptive (COC) pills have been widely used as the primary treatment for menorrhagia. COCs can also be used to induce endometrial atrophy and to decrease endometrial prostaglandin production, which can consequently improve menorrhagia and dysmenorrhea that are associated with adenomyosis

Oral Contraceptive Pill Pretreatment in Polycystic Ovary Syndrome Patients Undergoing IVF/ICSI Using the Gonadotrophin Releasing Hormone Antagonist Protocol (A Randomized Controlled Trial)

373 women with PCOS undergoing a trial of IVF/ICSI will receive OCP from day 2 of the preceding cycle for 21 days followed by GnRH antagonist COH.
Another 373 women with PCOS undergoing a trial of IVF/ICSI will start GnRH antagonist COH directly without OCP pretreatment.
Both groups will be followed up for effect on ongoing pregnancy rate.

PURPOSEBinge drinking (BD) is common among young people. E-Health apps are attractive to them and may be useful for enhancing awareness. We aimed to investigate the impact of a publicly available evidence-based e-Health app (Digital-Alcohol Risk Alertness Notifying Network for Adolescents and Young Adults [D-ARIANNA]), estimating current risk of BD by questions, matching identified risk factors, and providing in percent an overall risk score, accompanied by appropriate images showing mostly contributing factors in summary graphics.METHODSA natural, quasi-experimental, pre-/post-test study was conducted. Subjects were recruited in pubs, clubs, discos, or live music events. They were requested to self-administer D-ARIANNA and were re-evaluated after two further weeks.RESULTSYoung (18-24 years) people (N = 590) reported reduced BD at follow-up (18% vs. 37% at baseline). To exclude systematic errors involving those lost at follow-up (14%), the diminution in BD was confirmed in an appropriate generalized estimating equation model with unweighted data on a last observation carried forward basis.CONCLUSIONSOur study provides evidence of population-level benefit at 2 weeks, attained with D-ARIANNA. This can be disseminated easily and economically among young people. However, additional components, including regular feedback and repeated administration by gamification, may be required to make this app suitable for longer term impact.

The opinion of Marion Larat is that “nobody, but nobody should take the third- and fourth-generation pills”.1 This view is not surprising as she suffered a devastating stroke at the age of 18 years.2 A subsequent enquiry linked her cerebral vascular accident to Meliane®,3 a 20 μg pill containing gestodene that she had been prescribed for contraception. In December 2012, Marion Larat successfully sued the manufacturer, Bayer, and the French drug regulatory board Agence national de securite du medicament et des produits de sante (ANSM) for damages. Her case was widely publicised in the media and 30 similar lawsuits soon followed. In January 2013, the Minister for Health, Marisol Touraine, announced that France's social security system would no longer reimburse prescriptions for third- and fourth-generation pills.4 At the time around half of the five million French women on the combined pill were taking such pills,5 which cost more to the state than older formulations. Plans were made to strip midwives and nurses of their pill-prescribing powers.5 The ANSM advised that third- and fourth-generation pills should never be used as first-line contraception and should only be prescribed if an older formulation had not been tolerated.5 The ANSM then referred the matter to the European Medicines Agency (EMA) for further investigation. Since the introduction of the combined oral contraceptive pill (COC) in the 1960s, an association between ethinylestradiol (EE) and venous thromboembolism (VTE) has been apparent. The amount of EE in pills has lessened over time to reduce thrombotic potential as well as estrogenic side effects. Older pills contain testosterone-derived progestogens and can have androgenic side effects. Newer formulations have been developed that contain less androgenic progestogens and so aim for better tolerability. Effect of administration of oral contraceptives on the synthesis and breakdown of myofibrillar proteins in young women

Oral contraceptive (OC) treatment has an inhibiting effect on protein synthesis in tendon and muscle connective tissue. We aimed to investigate whether OC influence myofibrillar protein turnover in young women. OC‐users (24±2 years; Lindynette®n=7, Cilest®n=4) and non‐OC‐users (controls, 24±4 years n=12) performed one‐legged kicking exercise. The next day, the myofibrillar protein fractional synthesis rate (FSR) was measured using stable isotopic tracers (13C‐proline) while the subjects were fed standardized nutrient drinks. Simultaneously, a marker for myofibrillar protein breakdown, 3‐methyl‐histidine (3‐MH), was measured in the interstitial fluid of the vastus lateralis. Measurements were performed in both legs. In general, myofibrillar protein FSR was lower in OC‐users (two‐way analysis of variance, P<0.05), although the difference seemed to depend on the OC type. Interstitial 3‐MH in the skeletal muscle was not different between groups and did not vary by OC type. Exercise did not change myofibrillar protein FSR or 3‐MH concentrations. Serum androstenedione and bioavailability of testosterone were lower in OC‐users. In conclusion, the results indicate that the use of OC has an inhibiting effect on myofibrillar protein synthesis and the magnitude of the effect may depend on the type of OC. In contrast, there was no effect of OC on myofibrillar protein breakdown in the fed state.
