Delving into the Latest Updates on Desoximetasone with Synapse

Psoriasis and atopic dermatitis are chronic inflammatory disease that account for a significant amount of patients in most dermatological practices. Topical corticosteroid agents are often prescribed for treatment of both these conditions, especially when they are localized rather than wide spread. The development of resistance to treatment is termed tachyphylaxis. Poor adherence, rather than down regulation of receptors, may be the primary cause of tachyphylaxis to topical corticosteroids. The primary objective of the study is to determine, under conditions designed to assure good adherence, whether topical 0.25% desoximetasone spray improves clinical outcomes in patients who have resistant inflammatory skin disease defined by failure of previous topical steroid treatment.

Risk Factors for Worsening Dry Eye Symptoms in Patients Undergoing External Levator Advancement Surgery

Purpose::

Few studies have examined dry eye syndrome (DES) as a consequence of external levator advancement surgery. The purpose of this retrospective study was to examine risk factors for dry eye signs and symptoms after external levator advancement in order to improve preoperative planning.

Methods::

A retrospective chart analysis was performed on all patients who underwent external levator advancement from January 2010 to April 2015 performed by one surgeon (B.J.W.). Margin reflex distance-1 (MRD1) and margin reflex distance-2 were measured preoperatively and postoperatively. Dry eye symptoms (DSXs) were recorded for visits preoperatively, postoperatively at week 1, and postoperatively at month 3. DSXs were graded 0 to 3. Subgroup analysis was performed on patients with pre-existing DES diagnosis and on eyes undergoing additional simultaneous procedures. Statistical measures included linear regression, Pearson χ 2 tests, and analysis of variance with post hoc comparison tests.

Results::

A total of 190 eyes (115 patients) were included; 39% were patients with DES and 56% had simultaneous surgeries. Postoperative values for both MRD1 and margin reflex distance-2 and the change in values for MRD1 (postoperatively at month 3 to preoperatively) were statistically significant between patients with worsening versus those without worsening DSX. Differences in DSX in patients with DES were significant preoperatively and postoperatively at week 1 but not postoperatively at month 3. Comparing those patients with DES with worsening DSX versus those with improving DSX, those with worsening DSX had no difference in final MRD1 but did have a greater change in MRD1.

Conclusions::

Larger postoperative values of MRD1, margin reflex distance-2, and change in MRD1 are associated with worsening postoperative dry eye symptoms in those undergoing external levator advancement. In patients with DES, there was no significant difference in final MRD1; however, a greater change from preoperative and postoperative MRD1 was associated with worsening DSX. Oculoplastic surgeons may consider doing simultaneous lower eyelid surgery that decreases margin reflex distance-2 to potentially help reduce the chance of postoperative dry eye symptoms.

Sakuranetin modulates IL-17A-related inflammation and enhances skin barrier function in an imiquimod-induced psoriasis model

Sakuranetin, a methoxylated flavanone with known anti-inflammatory and antioxidant properties, has yet to be explored for its therapeutic potential in psoriasis treatment. This study evaluates its efficacy in an imiquimod-induced psoriasis-like murine model, focusing on both immune modulation and skin barrier restoration. Topical administration of sakuranetin significantly attenuated psoriatic inflammation, with high-dose treatment achieving comparable efficacy to desoximetasone. RNA sequencing and qPCR validation revealed that sakuranetin modulates keratinization, cornified envelope formation, and kallikrein-related peptidase activity, suggesting its role in epidermal homeostasis. Notably, sakuranetin selectively downregulated IL-17A while maintaining IL-17F expression, supporting a targeted immunomodulatory effect. Additionally, sakuranetin upregulated KRT1, KRT16, KLK7, Sprr2f, and Wfdc18 while downregulating Sprr2k and Sprr3, indicating a refined regulation of skin barrier proteins. The observed upregulation of Tslp suggests a potential role in epidermal repair mechanisms. These findings highlight sakuranetin as a novel, plant-derived therapeutic candidate with dual anti-inflammatory and skin barrier-enhancing properties, providing a potential corticosteroid-sparing approach for long-term psoriasis management.

Unbiased Metabolomic and Chemometric profiles of three Sargassum polycystum extracts using GCMS and LCMS/MS: content analysis, correlation analysis and molecular docking

Sargassum polycystum (S. polycystum) is a brown macroalga with a high phytochemical content, making it a nutritious and bioactive food source. However, information on factors contributing to health benefits, like antioxidants and cytotoxicity, is less explored for Malaysian S. polycystum. In this study, three extracts of S. polycystum were characterized using a combination of analytical techniques. Despite similar carbohydrate content across all extracts, water extract exhibited the highest protein [21.90 ± 1.01 albumin equivalent (μg/mg)] and phenolic [7.73 ± 1.95 gallic acid equivalent (μg/mg)] contents. However, it displayed the lowest antioxidant and anticancer activities [half-maximal inhibitory concentration (IC₅₀) of > 2000 μg/mL]. Interestingly, ethanolic extract demonstrated the strongest scavenging activity (IC₅₀ of 397.90 ± 20.43 μg/mL) and selective anticancer activity against MCF7 breast cancer cells (IC₅₀ of 338.63 ± 48.98 μg/mL). Untargeted metabolomic profiling confirmed the differences in the chemical composition of the extracts. Subsequently, correlation and docking analyses were used to identify the potential bioactive compounds within the extracts. The ethanolic extract is a rich source of these bioactive compounds with superior antioxidant and anticancer properties, highlighting the need for further research on its potential utility in the food industry.
