ABSTRACT.Mycetoma is a granulomatous disease of subcutaneous tissue that is caused by different species of aerobic filamentous bacteria (actinomycetoma) or fungi (eumycetoma). Mycetoma treatment depends on the causative pathogen, and it mainly consists of antimicrobial interventions or surgery. Aspergillus terreus is an etiologic agent of invasive aspergillosis and a rare cause of eumycetoma that is common in central and southern China. The present case report describes a 45-year-old female patient with subcutaneous swelling and multiple active and healed sinuses over the dorsal aspect of the left foot who was diagnosed with eumycetoma caused by A. terreus. This is the first such case reported in China. We applied a combination treatment consisting of itraconazole with topical luliconazole cream and radiofrequency microneedling, and this was followed by significant regression of the lesion. In addition, we reviewed the current literature to summarize the epidemiological characteristics of mycetoma in mainland China and cases of eumycetoma caused by A. terreus worldwide.

Onychomycosis caused by Aspergillus species belonging to section Circumdati

AbstractWe encountered two cases of onychomycosis caused by Aspergillus (A.) subramanianii and A. sclerotiorum. These species belong to the Aspergillus section Circumdati, which is reportedly common in a wide range of habitats. The members of section Circumdati rarely cause onychomycosis. We report the second and fifth cases of A. subramanianii and A. sclerotiorum in the world. Positive findings in dermatophyte antigen kits helped us to diagnose these infections. These isolates were sensitive to ravuconazole, with a minimum inhibitory concentration of 0.5 μg/mL. They were successfully treated with oral fosravuconazole. We discuss 14 cases of onychomycosis caused by Aspergillus section Circumdati.

Adult Kerion Caused by Trichophyton tonsurans

Adult tinea capitis, especially kerion, caused by Trichophyton tonsurans is relatively rare in China. Here, we report a case caused by the agent in an old woman with normal immune function. Fungal microscopic examination and culture were positive. The isolate was further identified as Trichophyton tonsurans by internal transcribed spacer (ITS) sequencing. Skin biopsy and histopathological examination suggested diffuse inflammatory cell infiltration in the dermis and subcutaneous tissue. Notably, fungal spores were observed within the dermis by Periodic acid-Schiff (PAS) staining. After eight weeks of treatment with oral griseofulvin and topical luliconazole, the patient's abscess subsided, hair regrowth occurred without scarring, and the mycologic examination returned negative.
