The Effects of Premedication With Pronase and Postural Exercise on Mucosal Cleanliness During Painless Esophagogastroduodenoscopy: A Prospective, Multicenter, Randomized Controlled Study Based on Artificial Intelligence Assessment

Gastric cancer and esophageal cancer are common malignant tumors that threaten people's life and health. Esophagogastroduodenoscopy (EGD) is an important tool for screening upper gastrointestinal tumors, especially early tumors, and an effective method for detecting other upper gastrointestinal lesions, including ulcers and polyps. However, during EGD examination, mucus and foam can reduce the cleanliness of the mucosa and limit the operator's visual field, resulting in missed lesions and misdiagnosis. Previous studies have shown that preoperative medication with pronase and dimeticone is correlated with the improvement of upper gastrointestinal mucosal cleanliness. However, the number of samples in such studies is limited, and the artificial judgment is not objective enough to evaluate the cleanliness of upper digestive tract mucosa. And, whether a postural exercise is necessary for premedication with pronase or dimeticone remains unclear. Our team design this experiment to examine the efficacy of using premedication of dimeticone/pronase with a postural exercise on visualization of the mucosa before painless EGD. The investigators aim to provide new evidence to optimize the use of premedication with EGD.

Treatment failure of over-the-counter pediculicide shampoos in Thai schoolgirls: Evidence of insecticide resistance

The increasing resistance to chemical pediculicides has raised concerns about the efficacy of head lice treatments, including in Thailand. This study evaluated the efficacy of three commercially available pediculicide shampoos (0.75% permethrin, 0.6% carbaryl, and 0.12% Stemona root crude extract) among infested children in Chonburi Province, Thailand. A pre-test/post-test experimental design was conducted with 135 infested female children assigned to three treatment groups (n = 45 each), with no significant differences in baseline demographic characteristics (level of head lice infestations, hairstyle, and hair length). Each group received two applications (Day 0 and Day 7) of the allocated pediculicide shampoo, following the manufacturer's instructions. The final proportion of cured individuals was assessed on Day 14 after treatment using a fine-tooth comb. The observed percentages of cured individuals were 42.22% for carbaryl shampoo, 24.44% for Stemona shampoo, and 6.67% for permethrin shampoo. No side effects were reported, although none of the shampoos achieved complete eradication, with particularly poor outcomes in children with heavy infestations. These findings provide clinical evidence of reduced efficacy of over-the-counter pediculicides in Thailand, consistent with emerging resistance trends. More effective alternatives with different mechanisms of action, including ivermectin, abametapir, and dimeticone-based products, should be considered, although their current availability in Thailand remains limited. In the interim, development of locally available herbal formulations, together with school-based screening and simultaneous treatment, represents more feasible strategies. These results highlight the need for updated treatment guidelines, restriction of ineffective products, and regular resistance surveillance to ensure effective control.

Use of pronase in screening for early cancers of the upper gastrointestinal tract

INTRODUCTION:

This study aimed to investigate the effectiveness of pronase in improving the detection rate of early cancer and enhancing visual field clarity during gastroscopy in China.

METHODS:

In total, 1450 patients who participated in an early diagnosis and treatment programme of upper gastrointestinal cancer in Wuwei, Gansu Province between 2020 and 2021 were enrolled. Cluster randomisation was utilised at the community level. All patients underwent endoscopy and biopsy. The experimental group (n=725) received pronase granules and dimethicone prior to gastroscopy; the control group (n=725) received dimethicone alone. Endoscopic visibility scores, examination durations, and lesion detection rates were recorded for both groups.

RESULTS:

Visibility scores for all regions of the stomach were significantly lower in the experimental group than in the control group (P<0.001). This finding remained consistent after adjustment for confounding factors in multiple linear regression analysis. The detection rate of precancerous lesions and early cancer was significantly higher in the experimental group than in the control group (77.5% vs 62.5%; P<0.001). Binary logistic regression analysis indicated that the likelihood of detecting early cancer was greater in the experimental group, with an odds ratio of 3.840 (95% confidence interval=1.204-12.241; P=0.023). Also, average gastroscopy time was significantly shorter in the experimental group than in the control group (6.52±2.51 min vs 10.03±1.23 min, t=33.81; P=0.001).

CONCLUSION:

The administration of pronase prior to gastroscopy enhances visual field clarity, reduces examination time, and increases the detection rates of precancerous lesions and early cancer.

Characteristics of Diaper Dermatitis in Patients Aged 0-24 Months After Congenital Heart Disease and the Effects of a Diaper Dermatitis Care Bundle

PURPOSE:

The purpose of this study was to examine characteristics of diaper dermatitis (DD) in critically ill patients aged 0 to 24 months following surgery for congenital heart disease (CHD), the effects of a diaper dermatitis care bundle (DDCB), and factors associated with the development of DD in this population.

DESIGN:

Nonrandomized comparison cohort study with a historical comparison group.

SUBJECTS AND SETTING:

Convenience sampling was used to identify children aged 0 to 24 months undergoing CHD and cared for in a pediatric intensive care unit in central Taiwan (Taichung). The sample comprised 54 participants; 24 were in the historical comparison group and 30 received the DDCB.

METHODS:

The rate of DD from the historical comparison group was compared to that in an intervention group managed with a bundle of interventions used to prevent and manage DD, which includes the routine use of a topical skin protectant. The participants in the historical comparison group received usual care to prevent and manage DD including routine cleansing with a moistened, disposable cloth every 3 hours and following defecation or urination. The DDCB comprised diaper changes at least every 3 hours and as needed after soiling, along with the routine application of a dimethicone and sodium hyaluronate-based skin protectant before diaper changes. Both groups were evaluated for use of an antifungal cream when candidiasis was present.

RESULTS:

Participants managed with the DDCB have a significantly lower rate of DD than the historical comparison group who received routine care (41.65% vs 6.67%, P = .002). Univariate logistic regression showed an odds ratio of 0.10 (95% confidence interval of 0.019-0.520, P = .006), suggesting that the DDCB acted as a protective factor against the development of DD. Multivariate analysis of the historical comparison group indicated that non-cyanotic heart disease was associated with a lower likelihood of postoperative DD compared to those with cyanotic heart disease (odds ratio = 0.01, 95% confidence interval of 0.000-0.946, P = .047).

CONCLUSIONS:

Managing critically ill patients with a DDCB recovering from cardiac surgery significantly decreased the incidence of DD. We recommend routine use of this type of care bundle for preventing DD in this vulnerable population in all children with CHD, with particular attention being paid to this with cyanotic CHD.

New Delhi: In a significant move aimed at safeguarding public health, the Indian government has banned 156 fixed-dose combination (FDC) drugs, including a variety of antibiotics, painkillers, multivitamins, and medications for treating fever and hypertension. This decision marks the largest crackdown on FDCs since 2016, when 344 such drugs were prohibited. The Union Ministry of Health and Family Welfare issued a gazette notification on Thursday, officially prohibiting the manufacture, sale, and distribution of the banned FDCs. The ban was based on the recommendations of an expert panel that reviewed a total of 324 FDCs. Resilia Pharmaceuticals Announces Return to Market of Neosalus® Cream and Neosalus® Lotion

Neosalus® Products Are Second Relaunch Announced by Resilia in Less Than One Month ATLANTA, May 3, 2023 /PRNewswire/ -- Resilia Pharmaceuticals, Inc. announced today that it has brought Neosalus® Cream and Neosalus® Lotion back to the market. Neosalus® Lotion is a non-steroidal anti-inflammatory prescription product that offers topical treatment for the management of skin disorders, including atopic dermatitis and allergic contact dermatitis. These products had not been available due to supply chain disruptions that occurred as a result of the COVID-19 public health crisis. Resilia Pharmaceuticals Brings Neosalus® Cream and Neosalus® Lotion Back to Market; Follows April 2023 Solace Relaunch Resilia acquired the rights to make, market and sell the Neosalus® products in December 2020. Neosalus® Cream received regulatory clearance from the US FDA in April 2009. Neosalus® Lotion was launched in November 2011. Both Neosalus® Lotion and Neosalus® Cream, use patented Proderm Technology® to help provide skin barrier repair, protection, and hydration1. In addition, the Neosalus® line of products is specially formulated with several key ingredients that can promote healthy skin: dimethicone to help restore the skin barrier and reduce trans-epidermal water loss (TEWL)1,2; glycerin, a humectant to help retain moisture1; and stearic/palmitic acids to produce ceramides, cholesterol, and other free fatty acids in the ratio the skin needs1.
