The Effects of Premedication With Pronase and Postural Exercise on Mucosal Cleanliness During Painless Esophagogastroduodenoscopy: A Prospective, Multicenter, Randomized Controlled Study Based on Artificial Intelligence Assessment

Gastric cancer and esophageal cancer are common malignant tumors that threaten people's life and health. Esophagogastroduodenoscopy (EGD) is an important tool for screening upper gastrointestinal tumors, especially early tumors, and an effective method for detecting other upper gastrointestinal lesions, including ulcers and polyps. However, during EGD examination, mucus and foam can reduce the cleanliness of the mucosa and limit the operator's visual field, resulting in missed lesions and misdiagnosis. Previous studies have shown that preoperative medication with pronase and dimeticone is correlated with the improvement of upper gastrointestinal mucosal cleanliness. However, the number of samples in such studies is limited, and the artificial judgment is not objective enough to evaluate the cleanliness of upper digestive tract mucosa. And, whether a postural exercise is necessary for premedication with pronase or dimeticone remains unclear. Our team design this experiment to examine the efficacy of using premedication of dimeticone/pronase with a postural exercise on visualization of the mucosa before painless EGD. The investigators aim to provide new evidence to optimize the use of premedication with EGD.

Characteristics of Diaper Dermatitis in Patients Aged 0-24 Months After Congenital Heart Disease and the Effects of a Diaper Dermatitis Care Bundle

PURPOSEThe purpose of this study was to examine characteristics of diaper dermatitis (DD) in critically ill patients aged 0 to 24 months following surgery for congenital heart disease (CHD), the effects of a diaper dermatitis care bundle (DDCB), and factors associated with the development of DD in this population.DESIGNNonrandomized comparison cohort study with a historical comparison group.SUBJECTS AND SETTINGConvenience sampling was used to identify children aged 0 to 24 months undergoing CHD and cared for in a pediatric intensive care unit in central Taiwan (Taichung). The sample comprised 54 participants; 24 were in the historical comparison group and 30 received the DDCB.METHODSThe rate of DD from the historical comparison group was compared to that in an intervention group managed with a bundle of interventions used to prevent and manage DD, which includes the routine use of a topical skin protectant. The participants in the historical comparison group received usual care to prevent and manage DD including routine cleansing with a moistened, disposable cloth every 3 hours and following defecation or urination. The DDCB comprised diaper changes at least every 3 hours and as needed after soiling, along with the routine application of a dimethicone and sodium hyaluronate-based skin protectant before diaper changes. Both groups were evaluated for use of an antifungal cream when candidiasis was present.RESULTSParticipants managed with the DDCB have a significantly lower rate of DD than the historical comparison group who received routine care (41.65% vs 6.67%, P = .002). Univariate logistic regression showed an odds ratio of 0.10 (95% confidence interval of 0.019-0.520, P = .006), suggesting that the DDCB acted as a protective factor against the development of DD. Multivariate analysis of the historical comparison group indicated that non-cyanotic heart disease was associated with a lower likelihood of postoperative DD compared to those with cyanotic heart disease (odds ratio = 0.01, 95% confidence interval of 0.000-0.946, P = .047).CONCLUSIONSManaging critically ill patients with a DDCB recovering from cardiac surgery significantly decreased the incidence of DD. We recommend routine use of this type of care bundle for preventing DD in this vulnerable population in all children with CHD, with particular attention being paid to this with cyanotic CHD.

AbstractKinetic magnetic sand, composed of ultra‐fine sand and dimethicone in a 98%–2% ratio, is a versatile sensory toy known for its moldable properties and structural stability (1). Despite the name, it lacks actual magnetic features. Ingesting kinetic sand can pose risks, including choking and gastrointestinal issues, especially in young children. This case report details a unique incident involving a 3‐year‐old who ingested a significant amount of kinetic sand. Although the sand's ingredients are generally hypoallergenic and nontoxic, its grainy texture presented challenges for retrieval. The patient was closely monitored, and ultimately, the sand passed without complications. While no official guidelines exist for managing such cases, individual assessments, considering factors such as ingestion time, symptoms, and age, are crucial for determining the appropriate course of action, which may range from observation to more invasive procedures like endoscopy or surgery.

AbstractPurposeThe objective of this study was to investigate whether different dispensing processes can alter the physicochemical and structural (Q3) attributes of a topical cream product, and potentially alter its performance.MethodsAcyclovir cream, 5% (Zovirax®) is sold in the UK and other countries in a tube and a pump packaging configurations. The structural attributes of the cream dispensed from each packaging configuration were analyzed by optical microscopy, confocal Raman microscopy and cryo-scanning electron microscopy. Rheological behavior of the products was also evaluated. Product performance (rate and extent of skin delivery) was assessed by in vitro permeation tests (IVPT) using heat-separated human epidermis mounted in static vertical (Franz-type) diffusion cells.ResultsDifferences in Q3 attributes and IVPT profiles were observed with creams dispensed from the two packaging configurations, even though the product inside each packaging appeared to be the same in Q3 attributes. Visible globules were recognized in the sample dispensed from the pump, identified as dimethicone globules by confocal Raman microscopy. Differences in rheological behaviour could be attributed to these globules as products not dispensed through the pump, demonstrated a similar rheological behaviour. Further, IVPT confirmed a reduced rate and extent to delivery across human epidermis from the product dispensed through a pump.ConclusionsDifferent methods of dispensing topical semisolid products can result in metamorphosis and Q3 changes that may have the potential to alter the bioavailability of an active ingredient. These findings have potential implications for product developers and regulators, related to the manufacturing and comparative testing of reference standard and prospective generic products dispensed from different packaging configurations.

How to Prevent and Treat Dry, Chapped Lips; American Academy of Dermatology

ROSEMONT, Ill., Jan. 26, 2021 (GLOBE NEWSWIRE) -- For many people, winter skin probably conjures up images of dry, cracked hands, flaky skin, and windburn. However, the lips can be affected too. And while it may seem that dry, cracked lips are something you have to live with until spring, dermatologists from the American Academy of Dermatology say it’s possible to have soft, supple lips year-round by following a few simple steps. “Cold, dry weather; sun damage; and frequently licking your lips are just some of the reasons your lips might feel dry and chapped this winter,” says board-certified dermatologist Noëlle Sherber, MD, FAAD. “Understanding these causes and anything else that might trigger your chapped lips goes a long way in preventing and treating them.” To prevent and treat dry, chapped lips at home, Dr. Sherber recommends the following tips: Choose non-irritating lip products. Look for lip balms, lipsticks and other lip products that contain ingredients like castor seed oil, ceramides, dimethicone or mineral oil. Make sure your lip product is also labeled “fragrance free” and “hypoallergenic.” Avoid ingredients such as camphor, menthol or eucalyptus, as these can irritate your lips. If your lips burn, sting or tingle after using a lip product, stop using that product. Apply lip balm throughout the day and before you go to bed. If your lips are very dry and cracked, try a thick ointment, such as petroleum jelly. Ointment seals in moisture longer than waxes or oils. Protect your lips outdoors. Apply a lip balm with an SPF of 30 or higher before going outdoors, even in the winter. Look for a lip balm with titanium dioxide and/or zinc oxide and reapply it every two hours while outdoors. Keep your skin hydrated. Drink plenty of water and use a humidifier at home — especially in your bedroom while you sleep. Stop licking, biting or picking at your lips. When your lips feel dry, it may feel natural to wet them by licking them, but this can worsen the problem. When you catch yourself licking your lips, apply a non-irritating lip balm instead. Avoid holding metal items with your lips.
