A procedure frequently performed by orthopaedic providers is the administration of corticosteroid injections for the management of various soft tissues and joint-related conditions, such as osteoarthritis, tendinitis, carpal tunnel syndrome, trigger finger, and de Quervain's tenosynovitis. While these injections have demonstrated effectiveness in alleviating symptoms, the discomfort associated with the procedure can be a source of anxiety and apprehension for patients. This discomfort arises from the sensation of the needle entering the affected area and the burning sensation induced by the corticosteroid solution.

Ethyl Chloride Spray to Reduce Pain From Local Anesthesia During Mohs Micrographic Surgery: A Randomized Clinical Trial

Fear of needles is a well-documented issue that affects many patients. This fear may cause significant anxiety in various medical situations, including in the dermatology office 1. If not properly managed, needle phobia can persist, affecting a patient's experience during each visit. This effect may compound and lead to avoidance behaviors that can delay necessary treatments 2. This issue is particularly relevant in a Mohs micrographic surgery (MMS) clinic where each patient experiences several painful needle injections before the day is over.
Ethyl chloride is gaining popularity among dermatologists for its routine use in clinical practice due to its anesthetic and antiseptic properties, especially as a pre-injection agent. Research indicates that cryotherapy or pre-cooling the skin before administering a local anesthetic may offer benefits over topical anesthetics, such as enhanced pain relief, quicker onset, and better patient compliance 3,4. However, to our knowledge, no studies have specifically examined the effectiveness of pre-cooling with ethyl chloride prior to Mohs micrographic surgery. To address this gap, we're conducting a trial aimed at evaluating the impact of pre-cooling with ethyl chloride on pain perception in adult patients undergoing Mohs micrographic surgery with local anesthetic injections.
The trial is a single-center, split-body study. The split-body design will involve dividing the surgical site into two equal halves, with one half (right side relative to patient) designated as the treatment side (receiving ethyl chloride spray prior to lidocaine injection) and the other as the control side (no ethyl chloride spray prior to injection). During the procedure, the participant will be asked to rate the pain associated with the needle injection AND the pain from infiltration of anesthetic from each side of the wound using the visual analog scale (VAS) scoring system (1 = no pain, 10 = most amount of pain possible). The results of the study will help us better understand how to mitigate pain and anxiety for patients requiring MMS.

A Comparison of CoolStick and Ethyl Chloride Spray for the Assessment of Sensory Block to Light Touch and Cold Before Caesarean Section Under Spinal or Combined Spinal-epidural Anaesthesia

The main aim of this study is to compare CoolStick to ethyl chloride spray for the assessment of light touch following neuraxial anaesthesia for caesarean delivery.
Coolstick and ethyl chloride will also be compared for the assessment of cold sensation.
Participants will also be asked whether they experienced any intraoperative pain.

Study on volatile organic compound source profiles and dynamic emission inventories in an integrated refinery-petrochemical complex

Volatile organic compound (VOCs) emissions from integrated refinery-petrochem. complex (IRPC) were difficult to quantify due to complex production processes and significant fugitive emissions.Previous studies had largely focused on upstream refinery units, with limited research on comprehensive emission factors (EFs) and inventories for the entire IRPC.We analyzed multiple emission sources within IRPC, including equipment leaks (EL), stationary combustion (SC), cooling towers (CT), storage tanks (ST), wastewater collection and treatment systems (WT), process vents (PV), flares (F), and loading operations (LO).The EFs of IRPC was 0.21 kg VOCs/t of crude processed.Addnl., to mitigate the temporal lag associated with traditional emission inventories, we developed dynamic accounting methods for VOCs emissions in IRPC.These methods integrate monitoring data and time-varying meteorol. parameters (e.g., temperature, air pressure, wind speed) to estimate emissions from SC and ST.An integrated anal. system combining gas chromatog.-mass spectrometry (GC-MS) and flame ionization detection (FID) was used to characterize and quantify 115 VOCs species.Composite source profiles were sampling from 35 sites collected across 18 units using a weighted average method.This detailed breakdown provided a more comprehensive view of IRPC emissions compared to studies focusing solely on upstream units.In the IRPC, key pollutants identified include propane, ethane, acetone, ethanol, propylene, and dichloromethane.Alkenes and alkynes had the highest ozone formation potential (OFP) contribution, while aromatics had the greatest secondary organic aerosol formation potential (SOAP) contribution.The study's detailed source profiles and dynamic emission inventory aid petrochem. pollution control and regional air quality improvements.

Increased VOC reactivity forcing ozone pollution in the Yangtze River Delta region, China: Evidence from an eight-year observation at an urban site and implications for future control strategies

Ozone (O₃) is the most critical air pollutant in the Yangtze River Delta (YRD) region of China. This study performed long-term field observation of O₃ precursors and meteorological variables from 2015 to 2022 at an urban site in the YRD region, and we identified the driving forces and key sources of O₃ formation rate (OFR) and evaluated the future pathways for mitigating O₃ pollution, combining the positive matrix factorization and observation-based model (PMF-OBM). During 2015-2022, O₃ formation bypassed the NO_X titration trap and was in the volatile organic compound (VOC) and NO_X transitional regime, and the OFR increased. The changes of rapidly decreased NO_X mixing ratio and increased VOC reactivity were the main contributors to the increase in OFR with 69.2 %, while increased temperature, enhanced radiation and other factors contributed to the residual proportion. Our result indicates that simultaneous reductions in ambient VOCs and NO_X could significantly mitigate current O₃ pollution. The control measurements of O₃ pollution should be focused on VOC evaporative emissions (e.g., solvent usage and gasoline evaporation) and complemented by industrial emission and diesel engine exhaust. Furthermore, this study predicted the OFR could decrease around 2025 and that O₃ pollution would be under control, and the sustained reductions in pollutant emissions could enable substantial mitigation of O₃ pollution under the carbon peak and neutrality policies in China.

Effect of Astragalus polysaccharide combined with cisplatin on exhaled volatile organic compounds as biomarkers for lung cancer and its anticancer mechanism

Cisplatin (DDP) is widely used to fight lung cancer, but there is a risk of immune damage. Astragalus polysaccharides (APS) is the main active component of Astragalus membranaceus Bunge. It has demonstrated anticancer properties across a range of cancer types as well as to be effective against cisplatin induced immune damage. However, its therapeutic mechanism has not been fully explored. This study aimed to explore the antitumor mechanisms of APS and elucidate the relationship between APS and volatile organic compounds (VOCs) in exhaled breath of Lewis lung cancer (LLC) mice. Gas chromatography-mass spectrometry (GC-MS) was utilized to analyze the exhaled VOCs in LLC mice. A specific group of VOCs was identified as potential biomarkers for monitoring tumor progression. Furthermore, the effects of combined treatment with APS and DDP on the concentration of exhaled VOCs in LLC mice was evaluated. Stoichiometric analysis revealed that the levels of 12 VOCs exhibited substantial recovery following APS treatment. And a high concentration of APS (400 mg/kg), when combined with DDP, exhibited enhanced antitumor efficacy. The metabolic pathways involved in the action of APS include 12 pathways. Our methodology elucidated both the effects and mechanisms of APS on lung cancer, as well as the pharmacological enhancement of cisplatin by APS. These findings facilitate real-time monitoring of lung cancer treatments and contribute to the future development of anticancer therapies.
