An overview of local anesthetics in over‐the‐counter products

Abstract:

Over‐the‐counter (OTC) local anesthetics have historically been used to alleviate pain in several common conditions including toothache and sore throat. With a rise in chronic conditions and an aging population, there has been an increase in associated chronic pain‐related disorders. Individuals with chronic pain often seek OTC treatments for quick and accessible pain relief. There are several common OTC local anesthetics, including benzocaine, lidocaine, and dibucaine, which are readily available to patients in several formulations. In order to appropriately advise patients on the use of local anesthetics, it is important to understand their key characteristics, including the mechanism of action, clinical properties, pharmacokinetics, clinical applications, and adverse reactions, which may occur.

Effect of Food Viscosity on Drug Dissolution

PURPOSE:

The purpose of the present study was to investigate the effect of food viscosity on the dissolution rate of a drug. There are two types of viscosity, macroviscosity and microviscosity. Macroviscosity affects the diffusion layer thickness, whereas microviscosity affects the molecular diffusion coefficient. The mass transfer coefficient (k_c) in the intrinsic dissolution rate (IDR) depends on the viscosity (η) as k_c ∝ η^a (a is an exponent on η). In theory, for rotating flow over a disk, if a thickener increases only macroviscosity, a = -1/6, and if it increases both macroviscosity and microviscosity equally, a = -7/6.

METHOD:

Benzocaine was used as a model drug. Hydroxypropyl cellulose (HPC) and methylcellulose (MC) were employed as control thickeners that increase only macroviscosity. Sucrose was employed as a control thickener for both macroviscosity and microviscosity. The FDA breakfast homogenate (BFH) was diluted with distilled water or 1 mM HCl with/without pepsin digestion. The IDR value was measured by the paddle-over-disk method.

RESULTS:

The η value of 30% BFH distilled water was 209 mPa∙s, about 300 times higher than distilled water. It was further increased by HCl (430 mPa∙s), and reduced by pepsin digestion (35 mPa∙s). The k_c value was little affected by BFH (a = 0.00 to -0.09), slightly less than those in HPC (a = -0.19) and MC (a = -0.21). Sucrose decreased the k_c value more significantly (a = -0.70).

CONCLUSION:

The IDR and k_c values of benzocaine were little affected by BFH, suggesting that BFH increased only macroviscosity.

Toward automated infrared spectral analysis in community drug checking

Abstract:

The body of knowledge surrounding infrared spectral analysis of drug mixtures continues to grow alongside the physical expansion of drug checking services. Technicians trained in the analysis of spectroscopic data are essential for reasons that go beyond the accuracy of the analytical results. Significant barriers faced by people who use drugs in engaging with drug checking services include the speed and accuracy of the results, and the availability and accessibility of the service. These barriers can be overcome by the automation of interpretations. A random forest model for the detection of two compounds, MDA and fluorofentanyl, was trained and optimized with drug samples acquired at a community drug checking site. This resulted in a 79% true positive and 100% true negative rate for MDA, and 61% true positive and 97% true negative rate for fluorofentanyl. The trained models were applied to selected drug samples to demonstrate a proposed workflow for interpreting and validating model predictions. The detection of MDA was demonstrated on three mixtures: (1) MDMA and MDA, (2) MDA and dimethylsulfone, and (3) fentanyl, etizolam, and benzocaine. The classification of fluorofentanyl was applied to a drug mixture containing fentanyl, fluorofentanyl, 4‐anilino‐N‐phenethylpiperidine, caffeine, and mannitol. Feature importance was calculated using shapely additive explanations to better explain the model predictions and k‐nearest neighbors was used for visual comparison to labelled training data. This is a step toward building appropriate trust in computer‐assisted interpretations in order to promote their use in a harm reduction context.

TARRYTOWN, N.Y., Dec. 22, 2023 (GLOBE NEWSWIRE) -- Insight Pharmaceuticals, a Prestige Consumer Healthcare Inc. company (“Insight”), is voluntarily recalling one lot of Americaine® 20% Benzocaine Topical Anesthetic Spray to the consumer level. While benzene is not an ingredient in any Americaine® 20% Benzocaine Topical Anesthetic Spray products, a recent review by our manufacturer and their third-party lab found that a sample from one lot of the product showed a low level of benzene came from the propellant that sprays the product out of the can. Importantly, no other lots of Americaine® 20% Benzocaine Topical Anesthetic Spray (either before or after this batch code) are in the scope of this recall and may continue to be used by consumers safely and as intended. Risk Statement: Benzene is classified as a human carcinogen. Exposure to benzene can occur by inhalation, orally, and through the skin and it potentially can result in cancers including leukemia and blood cancer of the bone marrow and blood disorders which can be life threatening. AMERICAN ACADEMY OF DERMATOLOGY URGES AMERICANS TO USE SUN PROTECTION AHEAD OF JULY 4th WEEKEND

Board-certified dermatologist provides tips to relieve discomfort from sunburns ROSEMONT, Ill., June 27, 2023 /PRNewswire/ -- The July 4th weekend is a time when many Americans will celebrate outdoors at parades, pools and cookouts, but without proper sun protection, they put themselves and their children at risk of getting sunburned, which increases the chances of developing skin cancer. A recent American Academy of Dermatology survey of more than 1,000 U.S. adults revealed that one-third of American parents reported having a child who was sunburned in 2022. Sunburn, which is the result of skin receiving too much exposure from the sun's ultraviolet (UV) rays, can cause long-lasting damage to the skin. "The survey data is troubling because blistering sunburns and childhood sunburns greatly increase your risk of getting skin cancer, including melanoma, the most serious form," said board-certified dermatologist A. Yasmine Kirkorian, MD, FAAD, chief of dermatology at Children's National Hospital in Washington, D.C. "To some extent, I think parents are aware of the need for sun protection, but at the same time are falling short of adequately protecting their children." People can avoid sunburn by seeking shade, remembering that the sun's rays are the strongest between 10 a.m. and 2 p.m., she said. Additionally, people should wear a lightweight and long-sleeved shirt, pants, a wide-brimmed hat, and sunglasses with UV protection, when possible. The third component of sun protection is applying a broad-spectrum, water-resistant sunscreen with an SPF of 30 or higher to all skin not covered by clothing and reapplying every two hours or after swimming or sweating, said Dr. Kirkorian. "I tell parents to make sure that their children are willing to apply sunscreen themselves," said Dr. Kirkorian. "For their faces in particular, it's often best to try a stick sunscreen because it's easier to apply. I also recommend that they tell their children to go inside to rest during the times of the day when the sun is the strongest." If you get a sunburn, it is important to begin treating it as soon as possible, so the best thing to do is get out of the sun and go indoors, she said. Dr. Kirkorian and the AAD recommend the following tips to help relieve discomfort from a sunburn: Take frequent cool baths or showers to help relieve the pain. As soon as you get out of the bathtub or shower, gently pat yourself dry, but leave a little water on your skin. Then, apply a moisturizer to help trap the water in your skin. This can help ease the dryness. Use a moisturizer that contains aloe vera or soy to help soothe sunburned skin. If a particular area feels especially uncomfortable, you may want to apply a hydrocortisone cream that you can buy without a prescription. Do not treat sunburn with "-caine" products (such as benzocaine), as these may irritate the skin or cause an allergic reaction. Consider taking aspirin or ibuprofen to help reduce any swelling, redness and discomfort. Drink extra water. A sunburn draws fluid to the skin's surface and away from the rest of the body. Drinking extra water when you are sunburned helps prevent dehydration. If your skin blisters, allow the blisters to heal. Blistering skin means you have a second-degree sunburn. You should not pop the blisters, as blisters form to help your skin heal and protect you from infection. Take extra care to protect sunburned skin while it heals. Wear clothing that covers your skin when outdoors. Tightly-woven fabrics work best. When you hold the fabric up to a bright light, you shouldn't see any light coming through. "If your skin starts to blister and you feel unwell from the blisters or heat exhaustion, then you should seek care from an emergency room," said Dr. Kirkorian. "Similarly, if the blisters are very large, you'll want to seek medical care.
