In this study, participants will receive, in randomized, double-blind fashion, an intragastric bolus administration of either (i) 300 mg quinine, (ii) 5 g L-isoleucine, (iii) a combination of (i)+(ii), or (iv) control, before 350 ml (500 kcal) of a mixed-nutrient drink, to evaluate the effects on postprandial blood glucose, gastric emptying, and the hormone, and appetite, responses to the mixed-nutrient drink. Study visits will be separated by 3-7 days and participants will receive one treatment per visit.
On each study visit, the participant will be intubated with a nasogastric feeding tube. At t= - 60 min (08:30 am), a baseline blood sample, visual analogue scale questionnaire (VAS), and breath sample will be collected and quinine or control will be administered through the feeding tube. 30 min later (at t= - 30 min), L-isoleucine or control will be administered over 2 min after which the feeding tube will be removed immediately. At t = -45, -30, -15, and -1 min further blood samples will be collected and VAS completed. At t = -1 min, participants will consume, within 1 minute, a mixed-nutrient drink, labeled with 100 mg of 1-13C-acetate for measurement of gastric emptying by breath sampling. Blood samples, VAS, and breath samples will be taken at regular intervals between t = 0-180 min.

An aDaptive, multicEnter, rAndomized, Open-Label, Controlled Trial to Assess Effectiveness and Safety of Quinine Sulfate for COVID-19 in Hospitalized Adults

This is a multicenter, randomized, open-label, controlled trial to evaluate the effectiveness and safety of Quinine Sulfate as an add-on therapy in hospitalized adults with COVID-19.
The study is a multi-center trial that will be conducted in up to approximately 2 sites nationally. New sites may be added as needed after appropriate assessment. Interim monitoring will be conducted to evaluate the arms and for safety and effectiveness. Any changes would be accompanied by an updated sample size.
Subjects will be assessed while hospitalized. All subjects will undergo a series of laboratory tests (CBC, SGOT, SGPT, Ureum, Creatinine, EKG, and PCR), clinical examination (clinical assessment, vital signs, accompanying drugs, and other medical conditions) and safety assessment (serious adverse events/ SAE)
Randomization will be performed 1:1 for each arm. Arm 1 = Standard of Care (SoC) alone, arm 2 = SoC + Quinine Sulfate

An electrochemical quinine detection approach based on small molecule promoted split aptamer click ligation reaction

Quinine is a natural antimalarial drug that also serves as a food additive; however, excessive consumption can result in harmful side effects. Here, we propose a simple and cost-effective electrochemical strategy for quinine detection based on the small molecule-templated split aptamer click ligation (SMT-SpA-CLR), where the integrity of the aptamer is restored through the template-promoted proximity effect. We first investigate the effects of two splitting sites on the affinity of quinine aptamer and find that the loop of the stem with more Watson-Crick base pairs is more suitable for splitting, which can maintain the affinity to a great extent. The affinity of the Split 1 design (K_d = 48 μM) is slightly reduced compared to the affinity of the parent aptamer. The stable quinine recognition and its sensitive detection are achieved by integrating the SMT-SpA-CLR with an electrochemical signal output. The linear response range for quinine detection is from 0.05 μM to 10 μM with a low detection limit of 25 nM (at S/N = 3). This strategy provides a novel ideal for the design of small molecule-templated split aptamer reaction and can potentially be expanded for detecting other small molecules and proteins.

Simultaneous and instrument-free detection of quinine and crystal violet based on a dual-emissive Eu3+-functionalized metal-organic framework

Simultaneous detection and quantification of multi-targets is a challenge using one single sensor in the complicated environment of real samples. Herein, we developed a dual-emissive sensor (EuUCHNA) that selectively and rapidly responds to crystal violet (CV) and quinine (QN), two commonly used anti-infective drugs for fungi, bacteria, and parasites, through distinct responsive modes. CV can effectively quench the blue and red fluorescent emission bands of EuUCHNA. By contrast, QN can remarkably enhance the blue emission band. The limit of detection (LOD) for CV and QN are 2.4 nM and 32.0 nM, respectively, which are superior to most of the reported sensors. EuUCHNA has been used to determine CV and QN in fish meat and fish pond water with good performance. Moreover, CV and QN can be simultaneously quantified by EuUCHNA in practical fish pond water. Furthermore, we constructed an instrument-free detection method via a smartphone with a color recognition app, which was successfully employed to detect CV and QN in fish pond water. This work provides a promising fluorescent sensor and a new analytical method for the simultaneous quantification of two targets in real samples using a single sensor.

High-throughput screening of new psychoactive substances and related compounds in food by UHPLC-Q/Orbitrap

Currently, there is concern about food products to which new psychoactive substances (NPS) have been added illegally. However, rapid, high-sensitivity, and high-throughput screening methods for such harmful substances have not been reported yet. In this study, a high-throughput non-targeted screening strategy was developed based on UHPLC-Q/Orbitrap. First, optimized pretreatment methods for typical high-risk matrices were established. Second, an accurate, efficient, and scalable high resolution mass spectrometry database containing 206 NPS and related compounds was constructed. The fragmentation pathways of three different types of fentanyl-related compounds were then systematically investigated to enhance compound identification. Additionally, a non-targeted screening strategy was developed using warning ions derived from 24 fentanyl-related compounds, streamlining the identification of NPS. This strategy was applied to 100 food samples with possible illegal additives and demonstrated its effectiveness. In summary, this study provides robust technical support for regulatory authorities in combating the illegal addition of food products.
