Delving into the Latest Updates on Quinine Sulfate with Synapse

In this study, participants will receive, in randomized, double-blind fashion, an intragastric bolus administration of either (i) 300 mg quinine, (ii) 5 g L-isoleucine, (iii) a combination of (i)+(ii), or (iv) control, before 350 ml (500 kcal) of a mixed-nutrient drink, to evaluate the effects on postprandial blood glucose, gastric emptying, and the hormone, and appetite, responses to the mixed-nutrient drink. Study visits will be separated by 3-7 days and participants will receive one treatment per visit.
On each study visit, the participant will be intubated with a nasogastric feeding tube. At t= - 60 min (08:30 am), a baseline blood sample, visual analogue scale questionnaire (VAS), and breath sample will be collected and quinine or control will be administered through the feeding tube. 30 min later (at t= - 30 min), L-isoleucine or control will be administered over 2 min after which the feeding tube will be removed immediately. At t = -45, -30, -15, and -1 min further blood samples will be collected and VAS completed. At t = -1 min, participants will consume, within 1 minute, a mixed-nutrient drink, labeled with 100 mg of 1-13C-acetate for measurement of gastric emptying by breath sampling. Blood samples, VAS, and breath samples will be taken at regular intervals between t = 0-180 min.

An aDaptive, multicEnter, rAndomized, Open-Label, Controlled Trial to Assess Effectiveness and Safety of Quinine Sulfate for COVID-19 in Hospitalized Adults

This is a multicenter, randomized, open-label, controlled trial to evaluate the effectiveness and safety of Quinine Sulfate as an add-on therapy in hospitalized adults with COVID-19.
The study is a multi-center trial that will be conducted in up to approximately 2 sites nationally. New sites may be added as needed after appropriate assessment. Interim monitoring will be conducted to evaluate the arms and for safety and effectiveness. Any changes would be accompanied by an updated sample size.
Subjects will be assessed while hospitalized. All subjects will undergo a series of laboratory tests (CBC, SGOT, SGPT, Ureum, Creatinine, EKG, and PCR), clinical examination (clinical assessment, vital signs, accompanying drugs, and other medical conditions) and safety assessment (serious adverse events/ SAE)
Randomization will be performed 1:1 for each arm. Arm 1 = Standard of Care (SoC) alone, arm 2 = SoC + Quinine Sulfate

Spectral properties of quinine sulfate in PVA films for front-face format emission measurements

Abstract:

The luminescence properties of quinine sulfate (QS) embedded in poly(vinyl alcohol) (PVA) film have been studied in detail. Fluorescence excitation and emission spectra were measured using a front-face configuration, and the corresponding intensities were tabulated. A fluorescence quantum yield of 0.51 was determined for QS-doped PVA, using QS dissolved in 1N H 2 SO 4 . Both excitation and emission anisotropy measurements indicated effective immobilization of QS molecules within the polymer matrix. The fluorescence lifetime of QS-doped PVA was measured using a picosecond-pulsed 375 nm laser diode and analyzed with both exponential and Lorentzian distribution models. The broad lifetime distribution suggests a complex interaction between QS molecules and the polymer matrix. The QS-doped PVA films are well suited for measurements involving solid materials and front-face geometries.

Evaluation of Gustatory and Olfactory Perception in Type II Diabetic Patients – A Clinical Observational Study

Abstract:

Introduction::

Diabetes mellitus is a metabolic syndrome characterized by hyperglycemia and disturbances in the metabolism of carbohydrates, proteins, and lipids. Taste and smell may influence the acceptance or rejection of food, and abnormalities in taste and smell receptors are known to influence the intake of specific foods, thus increasing the risk of obesity, diabetes, and metabolic syndrome.

Aim::

To estimate the random blood sugar (RBS) levels, assess the taste and smell perception in type II diabetic patients and healthy individuals.

Materials and Methods::

This study was carried out in 120 individuals (Group A–60 type II diabetics and Group B–60 healthy individuals). Blood samples of 120 participants were collected, and RBS levels were measured. The smell test was conducted by Sniffin’s sticks test using six commercially available scent-tip pens. The taste test was conducted by soaking sterile cotton pads in 4 different solutions (quinine sulfate, sucrose, sodium chloride, and citric acid) and applying them over the dorsum of the tongue one after the other, and the results were recorded.

Results::

Among the 120 subjects, Group A subjects showed reduced taste identification than the Group B subjects. A highly statistically significant difference was found in age, smell, and RBS (mg%) between Group A and Group B. Furthermore, among the two groups, impairment of taste identification was higher in the group A subjects. There was a statistically significant association in the identification of sweet, salt, and sour taste among the Group A and Group B subjects.

Conclusion::

From the results of the study, we infer that the identification of smell, salty, sweet, and sour taste was impaired in type II diabetic patients.

The Role of Quinine to Prevent Ventricular Fibrillation and ICD Shocks in Early Repolarization Syndrome

BACKGROUND:

The role of quinidine in patients with early repolarization syndrome (ERS) is known, but it is not available in many countries. We share our experience with quinine sulfate in the suppression of ventricular fibrillation in a patient with ERS.

CASE SUMMARY:

A 25-year-old man presented with cardiac arrest. Electrocardiogram demonstrated J-point elevation with QRS notching in the inferolateral leads. He developed recurrent ventricular fibrillation requiring resuscitation, repeated cardioversions, and intracardiac defibrillator implantation for secondary prevention. Despite amiodarone and propranolol, he experienced repeated intracardiac defibrillator shocks. Initiation of oral quinine achieved sustained suppression of premature ventricular complexes and ventricular fibrillation during 2 years of follow-up.

DISCUSSION:

The suppression of ventricular fibrillation in ERS involves use of quinidine, but access remains a barrier. Even though the use of quinine is reported in the literature in Brugada syndrome, the role of quinine in ERS is rarely reported.

TAKE-HOME MESSAGE:

This case highlights the possible role of quinine in suppression of short-coupled premature ventricular complexes and ventricular fibrillation in patients with ERS.
