This study is proposed to evaluate the safety and efficacy of the RUTI vaccine in patients with pulmonary tuberculosis. Therapeutic vaccination of RUTI would stimulate the immune response not only against growing bacteria, but also against bacteria in a latent state that are less sensitive to antibiotic treatments. Therapeutic vaccination in patients with pulmonary tuberculosis could improve the speed of recovery of patients without inducing the appearance of drug resistance.

PURPOSE:

To investigate urine microbiome differences among healthy women, women with recurrent uncomplicated cystitis (rUC), and those with sporadic/single uncomplicated cystitis (sUC) to challenge traditional beliefs about origins of these infections.

MATERIALS AND METHODS:

Patients who underwent both conventional urine culture and next-generation sequencing (NGS) of urine were retrospectively reviewed. Symptom-free women with normal urinalysis results as a control group were also studied. Samples were collected via transurethral catheterization.

RESULTS:

In the control group, urine microbiome was detected on NGS in 83.3%, with Lactobacillus and Prevotella being the most abundant genera. The sensitivity of urine NGS was significantly higher than that of conventional urine culture in both the sUC group (91.2% vs. 32.4%) and the rUC group (82.4% vs. 16.4%). In urine NGS results, Enterobacterales, Prevotella, and Escherichia/Shigella were additionally found in the sUC group, while the recurrent urinary tract infection (rUTI)/rUC group exhibited the presence of Lactobacillus, Prevotella, Enterobacterales, Escherichia/Shigella, and Propionibacterium. Moreover, distinct patterns of urine NGS were observed based on menopausal status and ingestion of antibiotics or probiotics prior to NGS test sampling.

CONCLUSIONS:

Urine microbiomes in control, sUC, and rUTI/rUC groups exhibited distinct characteristics. Notably, sUC and rUC might represent entirely separate pathological processes, given their distinct urine microbiomes. Consequently, the use of urine NGS might be essential to enhancing sensitivity compared to conventional urine culture in both sUC and rUTI/rUC groups.

A Case of Recurrent Urinary Tract Infection Successfully Treated during Homeopathic Supportive Care in Oncology

Abstract:

Introduction A 32-year-old patient with colon cancer consulted for homeopathic supportive care (HSC). She had also suffered from recurrent urinary tract infections (RUTIs) for 20 years. Could homeopathy treat these two very different issues with the same medicine? The Patient's Main Concerns Though the main reason for the consultation was a fear of the side effects of chemotherapy, the presence of Escherichia coli-induced RUTIs affected the patient's quality of life. Interventions and Outcomes The repertory listing highlighted Pulsatilla as the patient's homeopathic constitutional medicine. It was prescribed both for HSC and also for RUTI. In association with Arsenicum album and Nerves, Pulsatilla enabled a good tolerance to chemotherapy, with rapid recovery from peripheral neuropathies. In combination with Colibacillinum, Pulsatilla provided relapse-free curing of the RUTIs. The MOdified NARanjo Criteria for Homeopathy (MONARCH) Inventory score was +9. Conclusion The one individual's two different clinical complaints were treated with a constitutional homeopathic medicine, Pulsatilla, which covered the whole case. However, in HSC, the use of the constitutional remedy alone is rarely sufficient: it was reinforced by individualised symptomatic medication, organotherapy and isotherapy. For RUTI, isopathic and tubercular miasmatic treatments were each helpful. For both complaints, individualised homeopathy gave rapid, long-lasting and effective results.

Evidence-based review of nonantibiotic urinary tract infection prevention strategies for women: a patient-centered approach

Purpose of review:

There is a growing interest in nonantibiotic prevention strategies for recurrent urinary tract infections (rUTIs). Our objective is to provide a focused, pragmatic review of the latest evidence.

Recent findings:

Vaginal estrogen is well tolerated and effective for preventing rUTI in postmenopausal women. Cranberry supplements at sufficient doses are effective in preventing uncomplicated rUTI. Methenamine, d-mannose, and increased hydration all have evidence to support their use, although the evidence is of somewhat variable quality.

Summary:

There is sufficient evidence to recommend vaginal estrogen and cranberry as first-line rUTI prevention strategies, particularly in postmenopausal women. Prevention strategies can be used in series or in tandem, based on patient preference and tolerance for side effects, to create effective nonantibiotic rUTI prevention strategies.

Olga Rue, CEO of Archivel Farma, and Dr Pere Joan Cardona, Scientific advisor of the company, participated in the inauguration meeting of the Horizon 2020 project STriTuVaD (In Silico Trial for Tuberculosis Vaccine), held in New Delhi, India, on 23 January in the presence of Indian and European authorities. The meeting was hosted by the All India Institute of Medical Sciences (AIIMS) who is the Indian lead scientific in this project driven by a consortium of European, American and Indian entities. In his inaugural speech, Dr Balram Bhargava, director-general of the Indian Council of Medical Research (ICMR), gave an overview of the research engaged in so far at national level, to address the TB burden India is facing. He had high expectations in this international consortium for finding rapidly and at affordable conditions a vaccine. Professor Dipendra K. Mitra, Head of the Department of Transplant Immunology and Immunogenetics at AIIMS, outlined how the data for the clinical trials would be collected and Dr Guleria gave an overview on the capacity at AIIMs on TB vaccine trials. Professor Marco Viceconti (Bioengineering at the University of Bologna) stressed the importance of In Silico Trials, whose development is the main aim of the STriTuVaD project. Both Dr Reinhard Glueck (representative of Zydus Cadila, Chairman of Etna Biotech coordinator of the STriTuVaD project) and Ms Tania Friederichs, Head of Research and Innovation Section at the EU delegation to India, underlined the importance of international cooperation. After the opening addresses, the consortium members presented the work in the project, to begin with by Dr. Epifanio Fichera from Etna Biotech who is the coordinator and who stressed importance to deliver the work according to agreed timelines to ensure a smooth implementation of the project. Prof. Francesco Pappalardo (Dept. of Drug Sciences at the University of Catania), Dr. Miguel Juarez (Lecturer of Maths & Statistics at the University of Sheffield, UK), Prof Anant Mohan (Dept. of Pulmonary Medicine, AIIMS), Dr Corey Casper (Chief Scientific Officer at IDRI), Ms Olga Rué and Dr. Pere-Joan Cardona (respectively CEO and Scientific and Clinic advisor of Archivel Farma) elaborated on how they plan the work to come to a collective success. At the end of two days of work, the consortium emerged with a solid plan for the execution of the clinical trial where two new therapeutic vaccines developed respectively by Archivel Farma and IDRI will be tested. The trial will also be used to quantify the predictive accuracy of the UISS computer model developed by Prof. Pappalardo, capable of predicting the individual response of patients with active TB when treated with new therapies.
