The goal of this study is to learn about how the hormone insulin controls blood sugar. The main question it aims to answer is about how much insulin the body actually needs to maintain a normal blood sugar level. People with obesity and high insulin levels will receive eight doses of diazoxide, a drug that suppresses the pancreas's production of insulin, and will have their fasting blood sugar and insulin levels checked daily while taking the drug.

Safety and Feasibility of Hyperkalemic Cardioplegia With Diazoxide in Cardiac Surgery (CPG-DZX) Trial

This study aims to confirm the safety and efficacy of diazoxide as an additive to hyperkalemic cardioplegia in patients undergoing cardiac surgery with cardiopulmonary bypass. The investigators hypothesize that diazoxide combined with hyperkalemic cardioplegia provides superior myocardial protection and reduced myocardial stunning compared with standard cardioplegia alone. Thirty patients will receive treatment. Safety will be assessed by comparing mean arterial blood pressure measurements, glucose levels and incidence of adverse events between the two groups. Efficacy will be assessed by comparing right and left ventricular function in pre-operative vs post-operative transesophageal echocardiograms, need for mechanical circulatory support, ease of separation from bypass and Vasoactive Inotrope Score (VIS) between the two groups. The information gained could pave the way for the use of Katp (Potassium-atp) channel openers to prevent stunning, improve patient outcomes, and reduce health care costs related to myocardial stunning that requires inotropic and mechanical support following cardiac surgery.

Persistent hypoglycemia in congenital syphilis: hyperinsulinemic hypoglycemia with a focal pancreatic lesion

Abstract:

Objectives:

Congenital syphilis can cause severe morbidities such as hydrops fetalis, preterm birth, low birth weight, hepatosplenomegaly, pneumonia, hypoglycemia, etc., and mortality. Despite hypoglycemia being reported in congenital syphilis and hyperinsulinism, only one case was described. In this article, we aimed to present a newborn with congenital syphilis born to a syphilitic mother who was diagnosed with persistent hyperinsulinemic hypoglycemia with a focal lesion during follow-up.

Case presentation:

A female patient was born with a weight of 2,450 g in the 32+3rd week of pregnancy from a 30-year-old syphilitic mother who had pleural effusion and ascites, cholestasis, thrombocytopenia, and anemia at birth. Hypoglycemia was detected on the 10th day of the patient’s follow-up (47 mg/dL). The patient was unresponsive to diazoxide, and octreotide, nifedipine, and glucagon treatments were started gradually. No variants were detected in the HH gene panel, but a focal pancreatic head lesion was detected in the pancreatic head in fluorine-18 L-3,4 dihydroxyphenylalanine positron emission tomography-computed tomography. Focal lesionectomy was recommended for the patient.

Conclusions:

In this article, we present a neonate with severe early congenital syphilis and focal pancreatic lesions with persistent hyperinsulinemic hypoglycemia unresponsive to diazoxide. Although hypoglycemia can be seen in congenital syphilis and has been reported before, hypoglycemia associated with hyperinsulinism is very rare; only a few patients have been presented, and a focal pancreatic lesion has not been previously reported. The pathology is unknown, but pancreatitis may cause this entity.

Clinical and Genetic Characteristics of Congenital Hyperinsulinism in Norway: A Nationwide Cohort Study

Abstract:

Purpose:

Congenital hyperinsulinism (CHI) is a rare, monogenic disease characterized by excessive insulin secretion. We aimed to evaluate all probands with suspected CHI in Norway registered over the past 2 decades.

Methods:

The study included 98 probands. Clinical data were cumulated from medical records. All probands were screened for variants in the genes ABCC8 and KCNJ11. Other CHI-related genes were Sanger-sequenced as indicated by the patients’ phenotype (n = 75) or analyzed by next-generation sequencing employing a panel of 30 CHI-related genes (n = 23).

Results:

Twenty-one probands (21%) received a diagnosis other than CHI, the most common being idiopathic ketotic hypoglycemia (9%) or syndromic hyperinsulinism (4%). In the final cohort of 77 CHI probands, genetic findings were revealed in 46 (60%). ABCC8 variants were most common (n= 40), and 5 novel variants were identified. One proband harbored both the pathogenic GCK variant p.(Ala456Val) and the ABCC8 variant p.(Gly505Cys). Although most ABCC8 variants caused immediate disease onset with severe hypoglycemia and were diazoxide-unresponsive, 8 probands had a heterozygous, apparently dominant variant with milder phenotype. Two probands had pathogenic variants in GLUD1, whereas variants in HADH, HNF4A, KCNJ11, and HK1 were identified in 1 proband each, the latter being noncoding. Neurologic sequelae were reported in 53% of the CHI probands. Of nonsurgically treated probands, 43% had spontaneous resolution. The minimum birth prevalence of CHI in Norway is 1:19,400 live births.

Main Conclusion:

Individuals with disease-causing ABCC8 variants dominated our cohort. Patients with known genetic etiology had earlier and more severe disease onset than genetically unsolved patients.

Management of Neonatal Hyperinsulinemic Hypoglycemia: Trends Over Nine Years.

BACKGROUND:

With increasing use of diazoxide for hyperinsulinemic hypoglycemia (HH), reporting of serious side effects of diazoxide such as pulmonary hypertension (PHT) increased.

METHODS:

Charts of all children diagnosed with HH during the study period and evaluated by Pediatric Endocrinology division of the Hasbro Children's Hospital were reviewed. We analyzed diazoxide use among infants with HH with focus on infants born small for gestational age (SGA) and preterm infants.

RESULTS:

Average timing of diazoxide initiation was later after 6/2017 compared to prior in preterm infants (45 days versus 4 days, p < 0.001) and in SGA infants (28 days versus 13 days, p < 0.001). Prescribing patterns changed further over time, corresponding with development of diazoxide-associated PHT in three infants between 10/2018-5/2020. Delays in diazoxide initiation were observed after 5/2020 compared to prior in full-term, non-SGA infants: 18 days versus12 days (p = 0.01). The proportion of SGA infants who received diazoxide was lower after 5/2020 compared to prior (23% versus 65%, p = 0.03). PHT developed in 12.5% of infants treated with diazoxide in this cohort, 75% of whom were preterm and/or SGA, and 75% were born to mothers with preeclampsia.

CONCLUSIONS:

Patterns of diazoxide use changed over time, with delayed use in preterm and/or SGA infants observed after 6/2017, reduced use in SGA infants, and delayed initiation in all infants after 5/2020. PHT was not rare in our cohort, and was more likely in infants born SGA, preterm, or to a mother with preeclampsia.

FDA delays decision of Soleno’s Prader-Willi syndrome drug

The US Food and Drug Administration (FDA) has delayed its decision on Soleno Therapeutics investigational therapy for children with Prader-Willi syndrome. The new decision date for Soleno’s DCCR (diazoxide choline) extended-release tablets is set for 27 March 2025 – three months longer than the previous FDA extended review period. The delay comes after the FDA classified recent responses from Soleno as a “major amendment” to its new drug application (NDA), requiring additional time to review. However, the agency has not raised concerns about the drug’s safety, efficacy, or manufacturing. Prader-Willi syndrome is a rare genetic disorder caused by abnormalities in chromosome 15. Symptoms include low muscle tone, developmental delays, and behavioural challenges. Hyperphagia, or extreme hunger, is one of the most serious and defining characteristics of the disorder and can lead to severe obesity and associated health complications if unmanaged. There are currently no FDA-approved treatments specifically addressing hyperphagia in Prader-Willi syndrome. The US-based biotech’s NDA is supported by data from the Phase III DESTINY-PWS trial (NCT03440814), which enrolled 127 individuals with genetically confirmed Prader-Willi syndrome. The trial did not demonstrate a statistically significant difference in hyperphagia reduction overall, but Soleno said that the DCCR group showed “nominally significant” reductions in fat mass, and overall improvements in condition as assessed by investigators. DCCR has been granted breakthrough therapy, fast track, and orphan drug designations in the US, as well as orphan drug designation in the EU. Originally slated for discussion at an FDA advisory committee (AdComm) meeting, the agency decided in October 2024 that such a meeting was unnecessary. DCCR is an extended-release tablet containing a crystalline salt formulation of diazoxide, an activator of the adenosine triphosphate (ATP)-sensitive potassium (KATP) channel. These channels are present in a variety of cells, including pancreatic β cells, and adipocytes. The regulation of these cells can help manage hyperphagia. The delay represents a setback for Soleno on its first potential approved treatment. Soleno’s stock price decreased by 6.9% from $57.22 at close on 25 November to a low of $53.25 on 26 November. According to GlobalData’s Pharma Intelligence Center, DCCR could generate up to $759m in sales by 2030. GlobalData is the parent company of Pharmaceutical Technology. The company is also evaluating DCCR as a treatment for paediatric patients with certain genetic obesities, including SH2B1 deficiency, obesity associated with PCSK1 mutation (rs6232 variant), and SIM1 deficiency. A Phase II trial (NCT05532020) is set to enrol approximately 30 patients aged five years or younger suffering from either of the three genetic obesity disorders.
