Patients on dual antiplatelet therapy )aspirin plus a P2Y12 inhibitor [clopidogrel, prasugrel, or ticagrelor]) undergoing screening or surveilance colonoscopy are at increased risk for bleeding from the procedure. Patients are ussually asked to stop the P2Y12 inhibitor for the procedure. Currently, recommendation is that patients only on a P2Y12 inhibitor be changes to aspirin for the procedure. This pilot study will evaluate if there is a large difference in bleeding between patients only taking aspirin compared with patients only taking a P2Y12 inhibitor. Patinets who participate will randomly be randomly asked to stop either aspirin or the P2Y12 inhibitor 1 week before the procedure. Periprocedural bleeding and bleeding after the procedure for iup to 30 days.

Improved fast reversed-phase HPLC using factorial design for concurrent analysis of ramipril, atorvastatin, and clopidogrel in laboratory synthesized tablet

A group of many medical illnesses including diabetes, obesity, hypertension, and dyslipidemia is known to cause a metabolic syndrome leading to an increasing risk of heart disease (HF).In this case, many drugs are required for treatment.Combined therapy is preferred.In this therapy, an anti-hypertensive drug is used with lipid-lowering drugs.Furthermore, antiplatelet and anticoagulant are commonly used to avoid thrombus formation.Therefore, the simultaneous determination of the combination of ramipril (RP), atorvastatin (AT), and clopidogrel (CLP) turns interesting.The objective of this work is to develop a simple, sensitive, and eco-friendly HPLC method to determine this combination effectively.A 2³factorial design model was used for the optimization of chromatog. parameters.A C18 column (150 x 4.6 mm; 5 μm) was used with a mobile phase of Et alc. and 0.05 M sodium dihydrogen phosphate (53:48 volume/volume).The o-phosphoric acid was used to set the pH to 3.The optimal detection wavelength was 210 nm with the flow rate set at 1 mL/min and a run time of 10 min.The application of full factorial design allows the accurate assessment of the drugs under study.The three drugs were eluted within 10 min.The retention times were 2.8 min for ramipril, 5.8 min for atorvastatin, and 8.7 min for clopidogrel.Calibration curves were rectilinear.The linearity range for ramipril was from 0.5to15 μg/mL with a correlation coefficient (r) of 0.9999.The range for atorvastatin was from 0.5to10 μg/mL with (r) 0.9998.The range for clopidogrel was between 0.5 and 30 μg/mL with (r) 0.9999.Concentration values (0.0287, 0.054, and 0.0707) μg/mL were the lowest concentrations detected for RP, AT, and CLP resp.Concentrations (0.0871 0.1637, and 0.2142) μg/mL were the lowest concentrations that can be quantified for RP, AT, and CLP, resp.RP, AT, and CLP were tested simultaneously in both their synthetic mixture and combined tablet formulation.The validation was conducted in agreement with the International Conference on Harmonisation guidelines.

The relationship between serum coagulation parameters and the recurrence of chronic subdural hematoma

The aim is to investigate the relationship between serum coagulation parameters (PT, APTT, D-D and FDP) before hospitalization and recurrence of chronic subdural hematoma (CSDH). 236 patients with CSDH who were diagnosed for the first time and had complete medical records were followed up for at least 90 days. Fifty patients (21.2%) had relapsed. Univariate analysis was conducted including general data, imaging data and test results. Serum coagulation parameters (PT, APTT, D-D and FDP) were detected for all CSDH patients. The study identified several factors that exhibited a significant correlation with chronic subdural hematoma (CSDH) recurrence. These factors included advanced age (p = 0.01), hypertension (p = 0.04), liver disease (p = 0.01), anticoagulant drug use (p = 0.01), antiplatelet drug use (p = 0.02), bilateral hematoma (p = 0.02), and single-layer hematoma (p = 0.01). In addition, the presence of fibrin/fibrinogen degradation products (FDP) exceeding 5 mg/L demonstrated a significant relationship with CSDH recurrence (P < 0.05). Notably, the combined assessment of D-dimer (D-D) and FDP exhibited a significant difference, particularly regarding recurrence within 30 days after surgery (P < 0.05). The simultaneous elevation of serum FDP and D-D levels upon admission represents a potentially novel predictor for CSDH recurrence. This finding is particularly relevant for patients who experience recurrence within 30 days following surgical intervention. Older individuals with CSDH who undergo trepanation and drainage should be closely monitored due to their relatively higher recurrence rate.

Platelets tune fear memory in mice

Several lines of evidence have shown that platelet-derived factors are key molecules in brain-body communication in pathological conditions. Here, we identify platelets as key actors in the modulation of fear behaviors in mice through the control of inhibitory neurotransmission and plasticity in the hippocampus. Interfering with platelet number or activation reduces hippocampal serotonin (5-HT) and modulates fear learning and memory in mice, and this effect is reversed by serotonin replacement by serotonin precursor (5-HTP)/benserazide. In addition, we unravel that natural killer (NK) cells participate in this mechanism, regulating interleukin-13 (IL-13) levels in the gut, with effects on serotonin production by enterochromaffin cells and uptake by platelets. Both NK cells and platelet depletion reduce the activation of hippocampal inhibitory neurons and increase the long-term potentiation of synaptic transmission. Understanding the role of platelets in the modulation of neuro-immune interactions offers additional tools for the definition of the molecular and cellular elements involved in the growing field of brain-body communication.
