Effects of D-Chiro-Inositol Combined with Metformin on Insulin Resistance in People with Overweight or Obesity and Type 2 Diabetes Mellitus: a Placebo-controlled Randomized Double-Blind Crossover Trial

This study evaluates the efficacy of D-Chiro-Inositol (DCI) as an add-on therapy to metformin in improving insulin resistance, measured by the HOMA index, in people with overweight or obesity and type 2 diabetes mellitus (T2DM). Secondary outcomes include glycemic variability, indirect calorimetry, respiratory quotient, and other metabolic parameters.

A protein dynamics–based deep learning model enhances predictions of fitness and epistasis

Deep learning has advanced our ability to assess the effects that individual mutations have on protein function; however, predicting the complex interplay between two or more mutations remains challenging. Here, we seek to address this challenge by building a deep learning framework that incorporates information related to protein dynamics. Namely, we build a neural network architecture using a physics-based metric called the Asymmetric Dynamic Coupling Index (DCI asym ), which quantifies the degree to which each member of a pair of residues influences the flexibility of the other. DCI asym enables us to train models through an allosteric Graph Neural Network (GNN) in which each residue is linked to its distant dynamic influencers. Despite not being trained on experimental epistasis data, our GNN consistently outperforms existing approaches on deep mutational scanning datasets across four distinct proteins, highlighting its enhanced capacity to model epistatic interactions. Our GNN model was then challenged to predict the functions of 37 novel, computationally designed TEM-1 β-lactamase variants of unknown function, and it demonstrated excellent predictive accuracy for these variants. Thus, our GNN provides a pathway for better assessing the impact of multiple mutations on protein function, including epistatic relationships and mutations that have profound effects on activity despite being spatially far from the active site.

Neutrophil Extracellular Traps and Delayed Cerebral Ischemia in Patients With Subarachnoid Hemorrhage

Background:

Delayed cerebral ischemia (DCI) after spontaneous subarachnoid hemorrhage (SAH) is not well understood. Recent studies suggest the involvement of neutrophil‐mediated immunothrombosis via neutrophil extracellular traps (NETs). We aimed to assess whether trajectories of NET biomarkers in patients with acute SAH are associated with DCI.

Methods:

This was a prospective single‐center study of patients within 48 hours of SAH onset. We collected clinical, laboratory, imaging, and 3‐month outcome data. NET markers (H3Cit [citrullinated histone H3]‐DNA complexes, myeloperoxidase‐DNA complexes, cell‐free DNA), along with peptidylarginine deiminase 4, and deoxyribonuclease activity were measured in plasma collected on admission (baseline), and post‐SAH days 5 and 10. DCI was defined by clinical and imaging criteria. Generalized estimating equations examined differences in NET trajectories comparing patients with and without DCI.

Results:

We enrolled 40 patients with SAH (mean age, 57 years; n=21 [53%] women; 32 [80%] aneurysmal). DCI occurred in 12 (30%) patients. All NET levels were similar between groups at baseline. On day 5, those with DCI compared with those without DCI had elevated median H3Cit‐DNA complex (63 ng/mL versus 31 ng/mL, P =0.049), myeloperoxidase‐DNA complex (5.9 ng/mL versus 0.4 ng/mL, P =0.029), and cell‐free DNA (651 ng/mL versus 515 ng/mL, P =0.04) levels, which trended back down on day 10. In the DCI group, generalized estimating equations indicated heightened trajectories of H3Cit‐DNA complex (day 5 [compared with baseline]: β=0.7; SE, 0.3; P =0.01; day 10: β=0.5; SE, 0.3; P =0.09), and myeloperoxidase‐DNA complex levels (day 5: β=3.1; SE, 0.4; P <0.001; day 10: β=1.6; SE, 1.0; P =0.10).

Conclusions:

In patients with acute SAH, elevated plasma trajectories of NETs are associated with DCI.

Isolation, biochemical characterization, and primary structure and active site determination of Dioscorea opposita (‘Nagaimo’) oligopeptidase B

A protease was purified to homogeneity from Dioscorea opposita "Nagaimo" using ion exchange, hydrophobic and gel filtration columns, and its biochemical characterization including molecular weight, substrate specificity and kinetic parameters were determined. Protease activity was strongly inhibited by AEBSF, DCI and TLCK. The enzyme moderately inhibited by NEM and HgCl₂. The enzyme activity inhibited by NEM and HgCl₂ was restored with the addition of β-ME. These findings suggest that the enzyme is a trypsin-like serine protease, which is regulated by SH compounds. The N-terminal amino acid of this protease is blocked in an unknown manner. We determined the structure of the cDNA and deduced amino acid sequence of the protease from D. opposita. The cDNA was composed of 2420 nucleotides and encoded 751 amino acids in the coding region. The results indicated that this enzyme is an oligopeptidase B (OPB), consisting of a N-terminal region (M¹ ∼ T4⁷), a N-terminal β-propeller domain (A⁴⁸∼ L⁴⁶⁵), a connecting domain (K⁴⁶⁶ ∼ D⁵²⁷), a peptidase_S9 domain (P⁵²⁸ ∼ D⁷⁴⁴) and C-terminal region (R⁷⁴⁵ ∼ S⁷⁵¹). The overall homology of amino acid sequences of D. opposita to D. alata and D. rotundata was 99.07 % and 97.07 %, respectively. The catalytically active amino acid sites [S⁵⁹⁹, D⁶⁸⁴, and H⁷¹⁹] among these yam species were found to be highly conserved. Site-directed mutagenesis confirmed that these three the active center.
