Chronic kidney disease (CKD) is defined as either renal damage and/ or a glomerular filtration rate (GFR) below 60 mL/min/1.73 m2 for more than 3 months. End stage renal disease (ESRD) is defined as GFR below 15 mL/min/1.73 m2.
Secondary hyperparathyroidism (SHPT) is an insidious disease that develops early in the course of CKD and increase in severity as the GFR deteriorates. High serum levels of intact parathyroid hormone (iPTH) are known to cause high turnover bone disease, i.e. osteitis fibrosa and have also been reported to increase the mortality risk in patients undergoing hemodialysis (HD).
Standard therapy for SHPT includes dietary calcium supplementation, active vitamin D, and phosphate binders; however, these are often insufficient to allow patients to achieve their serum parathyroid hormone (PTH), calcium and calcium-phosphorus product (Ca × P) targets.
Recent preclinical studies have demonstrated that treatment with calcimimetics that increase the sensitivity of the calcium-sensing receptor (CaR) to calcium can reverse the alterations in CaR and vitamin D receptor expression and parathyroid cell proliferation that are associated with SHPT.
Calcimimetics such as cinacalect are positive allosteric modulators of the calcium-sensing receptor that increase its sensitivity by lowering the threshold for activation by extracellular calcium ions. The calcimimetic cinacalcet mainly increases the sensitivity of the CaR to extracellular Ca, thus inhibiting the release of PTH, although, as recently shown, it also decreases PTH synthesis.
Growth hormone (GH) indirectly promotes the growth of child by stimulating the production of insulin like growth factor (IGF-1), many children with renal disease have normal or elevated level of GH in their blood otherwise the levels of IGF-1 are low because the approximately 98% of IGF-1 is always bound to one of binding proteins (BP) inside the liver, so this accumulated protein will reduce the function of IGF-1.
Using cinacalcet in controlling hyperparathyroidism may reduce growth problems in children with ESRD with regular hemodialysis.

Microtubule dynamics are critical for spindle assembly and chromosome segregation during cell division. Pharmacological inhibition of microtubule dynamics in cells causes prolonged mitotic arrest, resulting in apoptosis, an approach extensively employed in treating different types of cancers. The present study reports the synthesis of thirty-two novel bis-amides (SSE1901-SSE1932) and the evaluation of their antiproliferative activities. N-(1-oxo-3-phenyl-1-(phenylamino)propan-2-yl)benzamide (SSE1917) exhibited the most potent activity with GI₅₀ values of 0.331 ± 0.01 µM in HCT116 colorectal and 0.48 ± 0.27 µM in BT-549 breast cancer cells. SSE1917 stabilized microtubules in biochemical and cellular assays, bound to taxol site in docking studies, and caused aberrant mitosis and G₂/M arrest in cells. Prolonged treatment of cells with the compound increased p53 expression and triggered apoptotic cell death. Furthermore, SSE1917 suppressed the growth of both mouse and patient-derived human colon cancer organoids, highlighting its potential therapeutic value as an anticancer agent.

Cinacalcet Reverses Short QT Interval in Familial Hypocalciuric Hypercalcemia Type 1

Abstract:

Context:

Familial hypocalciuric hypercalcemia type 1 (FHH-1) defines an autosomal dominant disease, related to mutations in the CASR gene, with mild hypercalcemia in most cases. Cases of FHH-1 with a short QT interval have not been reported to date.

Objective:

Three family members presented with FHH-1 and short QT interval (<360 ms), a condition that could lead to cardiac arrhythmias, and the effects of cinacalcet, an allosteric modulator of the CaSR, in rectifying the abnormal sensitivity of the mutant CaSR and in correcting the short QT interval were determined.

Methods:

CASR mutational analysis was performed by next-generation sequencing and functional consequences of the identified CaSR variant (p.Ile555Thr), and effects of cinacalcet were assessed in HEK293 cells expressing wild-type and variant CaSRs. A cinacalcet test consisting of administration of 30 mg cinacalcet (8 Am) followed by hourly measurement of serum calcium, phosphate, and parathyroid hormone during 8 hours and an electrocardiogram was performed.

Results:

The CaSR variant (p.Ile555Thr) was confirmed in all 3 FHH-1 patients and was shown to be associated with a loss of function that was ameliorated by cinacalcet. Cinacalcet decreased parathyroid hormone by >50% within two hours, and decreases in serum calcium and increases in serum phosphate occurred within 8 hours, with rectification of the QT interval, which remained normal after 3 months of cinacalcet treatment.

Conclusion:

Our results indicate that FHH-1 patients should be assessed for a short QT interval and a cinacalcet test used to select patients who are likely to benefit from this treatment.

Kyowa Kirin Buys Into Gene Therapy With Orchard Therapeutics Acquisition

Kyowa Kirin is expanding its scope to genetic medicines by acquiring Orchard Therapeutics, a company whose gene therapy for a rare inherited metabolic disorder is commercialized in Europe and is on track for an FDA decision in the first quarter of 2024. Tokyo-based Kyowa Kirin will pay $16 cash for each American depositary share of Orchard, amounting to $387.4 million. That price is a 144% premium to Orchard’s average share price over the 30 days before the deal was announced Thursday. When Orchard went public in 2018, it priced shares at $14 apiece. Orchard, which is based in London and has U.S. operations in Boston, develops gene therapies based on hematopoietic stem cells (HSCs). The company makes its gene therapies by harvesting HSCs from a patient, inserting a functional copy of the missing or defective gene, and then administering those modified cells back into the patient. Orchard’s main asset is Libmeldy, an HSC gene therapy for early-onset metachromatic leukodystrophy (MLD), a rare and life-threatening metabolic disorder stemming from a genetic mutation that leads to the deficiency of a key enzyme. Lacking that enzyme leads to the buildup of lipids called sulfatides, which in turn damages myelin, the protective covering on nerve cells. The disease affects motor skills and can lead to the rapid loss of the ability to walk and talk. In 2020, the European Commission approved Libmeldy for treating early onset MLD in infants and children. In 2022, Orchard reported revenue of $18.8 million for Libmeldy; $7.1 million for the first half of this year. The gene therapy is under FDA review with a target decision date of March 18, 2024. Kyowa Kirin’s top products are Crysvita, for the bone disorder X-linked hypophosphatemia; Poteligeo, which has FDA approvals for treating subtypes of cutaneous T-cell lymphoma; Regpara, a secondary hypoparathyroidism therapy; and the Parkinson’s disease drug Nourianz. When Kyowa Kirin reported its second quarter 2023 financial results in August, executives said the company was actively exploring acquisition opportunities. In addition to a commercialized gene therapy, Orchard brings to Kyowa Kirin a pipeline of gene therapy candidates. Orchard’s HSC technology has produced two programs in earlier stages of clinical development for rare enzyme deficiencies. OTL-203 is a potential treatment for mucopolysaccharidosis type I; OTL-201 is in development for mucopolysaccharidosis type IIIA, also called Sanfilippo syndrome type A. “We are excited to announce that we have signed the transaction agreement to acquire Orchard Therapeutics, one of the leading providers of HSC gene therapy,” Takeyoshi Yamashita, director of the board, chief medical officer, senior managing executive officer of Kyowa Kirin, said in a prepared statement. “With this transaction, we anticipate being able to use a new modality that can have a profound impact on patients’ lives.” After the acquisition, Orchard will operate as a Kyowa Kirin subsidiary . The deal still needs approvals from Orchard shareholders and regulators. The transaction is expected to close in the first quarter of 2024. By then, Kyowa Kirin could be responsible for paying out more. The acquisition agreement includes a contingent value right making Orchard shareholders eligible for an additional $1 per share if Libmeldy wins FDA approval. Those payments would add about $90 million to the total purchase price.
