ABSTRACT:

Japanese encephalitis (JE) is a mosquito‐borne viral disease and remains an important cause of viral encephalitis in Asia and parts of the Western Pacific. Earlier WHO‐supported global burden estimates published around 2014 suggested approximately 68,000 clinical cases and up to 20,400 deaths annually; however, the World Health Organization emphasises that the true burden of JE is difficult to quantify due to substantial underreporting and heterogeneous surveillance systems. Although vaccination programs have led to marked declines in incidence in several countries, JE transmission persists in endemic regions, and sporadic outbreaks continue to occur. In the absence of specific antiviral therapy, prevention remains the cornerstone of JE control, with vaccination representing the most effective public health intervention. Among available vaccines, the live‐attenuated SA14‐14‐2 vaccine has played a central role in reducing the burden of JE in endemic countries. Its high effectiveness, long‐term protection following a single dose, favourable safety profile, WHO prequalification, and incorporation into national immunisation programs have made it the backbone of JE prevention in Asia. Inactivated vaccines, such as IXIARO, and chimaeric vaccines, such as IMOJEV, provide important complementary options, particularly for travellers and non‐endemic populations. This review synthesises current knowledge on JE epidemiology, transmission, clinical manifestations, diagnostics, pathogenesis, and advances in vaccination strategies. By integrating recent outbreak data, immunological insights, and vaccine developments, we highlight the continued public health relevance of JE and the critical importance of sustained vaccination programs, surveillance strengthening, and equitable vaccine access to further reduce its global impact.

Abstract:

Background:

Zika virus (ZIKV) is an emerging public health threat, rendering development of a safe and effective vaccine against the virus a high priority to face this unmet medical need. Our vaccine candidate has been developed on the same platform used for the licensed vaccine IXIARO®, a vaccine against Japanese Encephalitis virus, another closely related member of the Flaviviridae family.

Methods:

Between 24 February 2018 and 16 November 2018, we conducted a randomized, observer-blinded, placebo controlled, single center phase 1 study to assess the safety and immunogenicity of an adjuvanted, inactivated, purified whole-virus Zika vaccine candidate in the USA. A total of 67 healthy flavivirus-naïve adults aged 18–49 years were randomly assigned to one of five study arms to receive two immunizations of either high dose or low dose (6 antigen units or 3 antigen units) with both dose levels applied in two different immunization regimens or placebo as control.

Results:

Our vaccine candidate showed an excellent safety profile independent of dose and vaccination regimen with predominantly mild adverse events (AEs). No serious AE has been reported. The ZIKV vaccine induced neutralizing antibodies in all tested doses and regimens with seroconversion rates up to 85.7% (high dose), which remained up to 40% (high dose) at 6 months follow-up. Of note, the rapid regimen triggered a substantial immune response within days.

Conclusions:

The rapid development and production of a ZIKV vaccine candidate building on a commercial Vero-cell manufacturing platform resulted in a safe and immunogenic vaccine suitable for further clinical development. To optimize antibody persistence, higher doses and a booster administration might be considered.

BACKGROUND:

Zika virus infection is a threat to at-risk populations, causing major birth defects and serious neurological complications. Development of a safe and efficacious Zika virus vaccine is, therefore, a global health priority. Assessment of heterologous flavivirus vaccination is important given co-circulation of Japanese encephalitis virus and yellow fever virus with Zika virus. We investigated the effect of priming flavivirus naive participants with a licensed flavivirus vaccine on the safety and immunogenicity of a purified inactivated Zika vaccine (ZPIV).

METHODS:

This phase 1, placebo-controlled, double-blind trial was done at the Walter Reed Army Institute of Research Clinical Trials Center in Silver Spring, MD, USA. Eligible participants were healthy adults aged 18-49 years, with no detectable evidence of previous flavivirus exposure (by infection or vaccination), as measured by a microneutralisation assay. Individuals with serological evidence of HIV, hepatitis B, or hepatitis C infection were excluded, as were pregnant or breastfeeding women. Participants were recruited sequentially into one of three groups (1:1:1) to receive no primer, two doses of intramuscular Japanese encephalitis virus vaccine (IXIARO), or a single dose of subcutaneous yellow fever virus vaccine (YF-VAX). Within each group, participants were randomly assigned (4:1) to receive intramuscular ZPIV or placebo. Priming vaccinations were given 72-96 days before ZPIV. ZPIV was administered either two or three times, at days 0, 28, and 196-234. The primary outcome was occurrence of solicited systemic and local adverse events along with serious adverse events and adverse events of special interest. These data were analysed in all participants receiving at least one dose of ZPIV or placebo. Secondary outcomes included measurement of neutralizing antibody responses following ZPIV vaccination in all volunteers with available post-vaccination data. This trial is registered at ClinicalTrials.gov, NCT02963909.

FINDINGS:

Between Nov 7, 2016, and Oct 30, 2018, 134 participants were assessed for eligibility. 21 did not meet inclusion criteria, 29 met exclusion criteria, and ten declined to participate. 75 participants were recruited and randomly assigned. 35 (47%) of 75 participants were male and 40 (53%) were female. 25 (33%) of 75 participants identified as Black or African American and 42 (56%) identified as White. These proportions and other baseline characteristics were similar between groups. There were no statistically significant differences in age, gender, race, or BMI between those who did and did not opt into the third dose. All participants received the planned priming IXIARO and YF-VAX vaccinations, but one participant who received YF-VAX dropped out before receipt of the first dose of ZPIV. 50 participants received a third dose of ZPIV or placebo, including 14 flavivirus-naive people, 17 people primed with Japanese encephalitis virus vaccine, and 19 participants primed with yellow fever vaccine. Vaccinations were well tolerated across groups. Pain at the injection site was the only adverse event reported more frequently in participants who received ZPIV than in those who received placebo (39 [65%] of 60 participants, 95% CI 51·6-76·9 who received ZPIV vs three [21·4%] of 14 who received placebo; 4·7-50·8; p=0·006). No patients had an adverse event of special interest or serious adverse event related to study treatment. At day 57, the flavivirus-naive volunteers had an 88% (63·6-98·5, 15 of 17) seroconversion rate (neutralising antibody titre ≥1:10) and geometric mean neutralising antibody titre (GMT) against Zika virus of 100·8 (39·7-255·7). In the Japanese encephalitis vaccine-primed group, the day 57 seroconversion rate was 31·6% (95% CI 12·6-56·6, six of 19) and GMT was 11·8 (6·1-22·8). Participants primed with YF-VAX had a seroconversion rate of 25% (95% CI 8·7-49·1, five of 20) and GMT of 6·6 (5·2-8·4). Humoral immune responses rose substantially following a third dose of ZPIV, with seroconversion rates of 100% (69·2-100; ten of ten), 92·9% (66·1-99·8; 13 of 14), and 60% (32·2-83·7, nine of 15) and GMTs of 511·5 (177·6-1473·6), 174·2 (51·6-587·6), and 79 (19·0-326·8) in the flavivirus naive, Japanese encephalitis vaccine-primed, and yellow fever vaccine-primed groups, respectively.

INTERPRETATION:

We found ZPIV to be well tolerated in flavivirus naive and primed adults but that immunogenicity varied significantly according to antecedent flavivirus vaccination status. Immune bias towards the flavivirus antigen of initial exposure and the timing of vaccination may have impacted responses. A third ZPIV dose overcame much, but not all, of the discrepancy in immunogenicity. The results of this phase 1 clinical trial have implications for further evaluation of ZPIV's immunisation schedule and use of concomitant vaccinations.

The Company’s main third‑party product distribution contract concluded in 2025 and this activity is now expected to naturally wind down, as previously communicated. Valneva’s total product sales in 2025 were €157.9 million compared to €163.3 million in 2024, reflecting a 42.3% planned reduction in third party product sales (€19.2 million in 2025 versus €33.2 million in 2024). Excluding third-party sales, total product sales increased by 9% at CER. JAPANESE ENCEPHALITIS VACCINE IXIARO ® /JESPECT ® In 2025, IXIARO ® /JESPECT ® sales increased by 4.6% to €98.4 million, resulting from growth in the traveler’s market. This compares with 2024, when IXIARO ® sales were impacted by supply constraints. Sales at CER grew by 7.2% compared to 2025. In January 2025, Valneva secured a new $32.8 million contract with the United States Department of Defense (DoD) 5 . CHOL ERA / ETEC 6 -DIARRHEA VACCINE DUKORAL ® In 2025, DUKORAL ® sales were €31.9 million as compared to €32.3 million in 2024. Sales in 2025 were notably affected by €1.0 million of adverse currency effects, as well as lower sales in Germany due to the gradual transition of distribution to CSL Seqirus. Sales at CER grew by 1.8%. CHIKUNGUNYA VACCINE IXCHIQ ® In 2025, Valneva reported IXCHIQ ® sales of €8.4 million, compared to €3.7 million in 2024. Sales in 2025 benefited from the vaccine’s launch in several European countries, including the supply of doses to help contain a major chikungunya outbreak on the French island of La Réunion. This growth was partly offset by the previously communicated label and user‑restriction constraints 7 . In January 2026, Valneva voluntarily withdrew the Biologics License Application (BLA) and Investigational New Drug (IND) application for IXCHIQ ® in the United States. The Company remains focused on expanding IXCHIQ ® access and sales in other territories, including Low- and Middle-Income Countries (LMICs), where medical need remains high. Clinical Vaccine Programs LYME DISEASE VACCINE CANDIDATE – VLA15 VALOR Phase 3 Results expected in the first half of 2026 Valneva and Pfizer are developing VLA15, a vaccine candidate targeting Borrelia, the bacterium that causes Lyme disease. VLA15 is the most advanced Lyme disease vaccine candidate currently in clinical development, with two Phase 3 trials underway. It leverages an established mechanism of action, inducing antibodies to Borrelia outer surface protein A (OspA) to prevent the transmission of Lyme borreliosis from infected ticks. The vaccine antigens are designed to target the six most prevalent Borrelia serotypes. Pfizer is conducting a randomized, placebo-controlled Phase 3 field efficacy trial, VALOR (Vaccine Against Lyme for Outdoor Recreationists). Phase 3 vaccinations were completed in 2025 and VALOR trial results are expected to be reported in the first half of 2026. Subject to positive Phase 3 results, Pfizer intends to submit a Biologics License Application (BLA) to the U.S. FDA and Marketing Authorization Application (MAA) to the European Medicines Agency in 2026. Upon approval and successful commercialization of VLA15, Valneva would be eligible to receive $143 million in initial milestone payments from Pfizer, plus sales royalties ranging from 14% to 22% and up to $100 million in cumulative sales milestones. CHIKUNGUNYA VACCINE - IXCHIQ ® / VLA1553 Pilot vaccination campaign ongoing in Brazil In February 2026, Valneva and Instituto Butantan announced the initiation of a Pilot Vaccination Strategy (PVS) in Brazil using Valneva’s single-shot chikungunya vaccine, IXCHIQ ® . The vaccination campaign will serve as the basis for post-marketing commitment studies evaluating the effectiveness and safety of IXCHIQ ® in a real-world setting and generating real-world evidence in a large population. The PVS, agreed to between the Brazilian Ministry of Health (MoH) and Instituto Butantan, will be implemented in ten Brazilian municipalities strategically selected based on epidemiological and operational criteria in support of the PVS. In line with the current IXCHIQ ® label in Brazil, adults aged 18 to 59 years will be invited to participate, with the objective of achieving 20% to 40% vaccine coverage within the target population. Additionally, Valneva, through its partner Instituto Butantan, will donate up to 500,000 doses of IXCHIQ ® to the Brazilian MoH, for use in the program. SHIGELLA VACCINE CANDIDATE – S4V2 First Phase 2 results expected mid-2026 S4V2 is the world’s most clinically advanced tetravalent vaccine candidate against shigellosis, the second leading cause of fatal diarrhea worldwide. Two clinical trials of S4V2, a Phase 2 infant safety and immunogenicity trial 8 , and a Phase 2b Human Challenge trial (CHIM) 9 , sponsored by LimmaTech Biologics AG, are ongoing. First Phase 2 results are expected mid-2026. Subject to positive results for both trials, Valneva will assume responsibility for all further development 10 . No approved multivalent Shigella vaccine is currently available outside of Russia or China, and the development of Shigella vaccines has been identified as a priority by the World Health Organization (WHO) 11 . In October 2024, the U.S. FDA granted Fast Track designation to S4V2, recognizing its potential to address a serious condition and fill an unmet medical need 12 . The global market opportunity for a vaccine against Shigella is estimated to exceed $500 million annually 13 . ZIKA VACCINE CANDIDATE – VLA1601 Positive Phase 1 results announced In 2025, Valneva announced positive safety and immunogenicity results for the Phase 1 clinical trial of VLA1601, its second-generation adjuvanted inactivated vaccine candidate against the Zika virus (ZIKV) 14 . Despite the medical need, regulatory pathways and market opportunity for potential Zika vaccines remain uncertain. Valneva will therefore only consider further potential development steps for VLA1601 if private and public funding opportunities materialize. Full Year 2025 Financial Review (Audited 15 , consolidated under IFRS) Revenues Valneva’s total revenues were €174.7 million in 2025 as compared to €169.6 million in 2024. The increase was driven by IXIARO ® /JESPECT ® and IXCHIQ ® sales. Other revenues, including revenues from collaborations, licensing and services increased to €16.8 million in 2025 as compared to €6.3 million in 2024. The increase for 2025 was mainly attributable to the recognition of variable consideration relating to the Company’s research collaboration and licensing agreement for the Lyme disease program. Operating Result and adjusted EBITDA Costs of goods and services sold (COGS) were €107.1 million in 2025. The gross margin on commercial product sales, excluding IXCHIQ ® , remained flat at 50.8% as compared to 50.6% in 2024. Positive effects from lower third-party product sales were offset by higher manufacturing expenditure following the completion of the manufacturing transfer to the new Almeida facility in Scotland. Gross margin for IXIARO ® reached 59.6%, slightly lower than the 61.0% achieved in 2024. The decline for 2025 was mainly driven by higher manufacturing costs at the Almeida facility combined with lower manufacturing volumes during 2025. Gross margin for DUKORAL ® reached 33.3% for 2025 as compared to 38.7% in 2024. Batch failures during the last quarter of 2025 were the primary factor behind this margin compression. Gross margin for IXCHIQ ® was negative, impacted by €8.5 million of inventory write-offs, mostly related to the termination of the supply agreement with Serum Institute of India in December 2025. Finally, COGS of €12.5 million were attributable to the third-party distribution business, €10.8 million related to idle capacity and costs not allocated to products and €7.2 million related to services. For comparison, total COGS in 2024 were €98.5 million, including €90.0 million in cost of goods and €8.5 million in cost of services. Research and development expenses amounted to €85.3 million in 2025, compared to €74.1 million in 2024. This increase for 2025 was mainly driven by higher costs related to the Shigella vaccine under the R&D collaboration agreement with LimmaTech Biologics AG and increased IXCHIQ ® post-marketing obligations. Marketing and distribution totaled €37.4 million in 2025, down from €52.4 million in 2024. The decrease for 2025 primarily reflects reduced advertising and promotional spending related to IXCHIQ ® , partly offset by higher warehousing and distribution costs. General and administrative expenses decreased to €37.3 million in 2025 from €42.8 million in 2024. The reduction for 2025 was primarily driven by lower recruiting costs, reduced insurance charges, and savings in advisory and professional services. Other income, net of other expenses, decreased to €10.4 million in 2025 from €20.7 million in 2024. The decline for 2025 was mainly driven by lower R&D tax credits in Austria and reduced grant income from Scottish Enterprise recognized during 2025. Valneva recorded an operating loss of €82.1 million in 2025 compared to an operating profit of €13.3 million in 2024. This change was primarily driven by the absence of the 2024 net gain of €90.8 million related to the PRV sale. The adjusted EBITDA loss (as defined below) in 2025 was €59.4 million, compared to an adjusted EBITDA profit of €32.9 million in 2024, which in 2024 had benefited from the gain associated with the PRV sale. Net Result Valneva recorded a net loss of €115.2 million in 2025, compared to a net loss of €12.2 million in 2024. Net result, operating loss and adjusted EBITDA are not comparable between 2024 and 2025 because of the 2024 gain from the PRV sale. Finance expense and currency effects resulted in a net finance expense of €32.1 million in 2025, compared to €24.8 million in 2024. This increase was mainly attributable to €17.9 million higher interest expenses, mostly related to one-time effects from the 2025 repayment of the loan with Deerfield Management Company and OrbiMed (D&O). Cash Flow and Liquidity Net cash used in operating activities amounted to €52.9 million in 2025, compared to €67.2 million in 2024. The reduction in 2025 was driven by efficient cost management. Valneva’s Lyme disease vaccine offers beacon of hope amid 2025 sales dip

Credit: mundissima/Shutterstock.com Valneva reported a drop in product sales in 2025 amid market restrictions for chikungunya vaccine Ixchiq, with the travel vaccine specialist betting on upcoming readouts to turn the business’ growth around. As per preliminary results, Valneva’s product sales reached €157.9m ($186.1m) in 2025, a 3.3% drop from €163.3m in 2024. The company’s commercial products include Japanese encephalitis vaccine Ixiaro and cholera vaccine Dukoral, but its lead brand is Ixchiq. Ixchiq has endured a difficult regulatory spell over the past year. In August 2025, the US Food and Drug Administration (FDA) suspended Ixchiq’s license due to “serious safety concerns”. The UK’s Medicines and Healthcare products Regulatory Agency (MHRA) followed suit in February 2026 for the same reasons. The vaccine had been temporarily suspended in people over 65 years of age in the UK in June 2025. While Valneva did not reveal Ixchiq’s sales numbers for 2025, the company said the total product sales downturn was “expected”. The segment that dragged growth was third-party sales, which decreased 42.3% – though this contributed €19.2m to the €157.9m. Despite its products not tracking well, Valneva’s total revenue increased to €174.7m in 2025, up from €169.6m in 2024. The company added that this was buoyed by the ongoing research collaboration and licensing agreement for the Lyme disease programme with Pfizer. In April 2020, Pfizer began working with Valneva to help develop the latter’s Lyme disease vaccine candidate, which was in Phase II trials at the time. Valneva expects the first Phase III (NCT05477524) data readout for the candidate, dubbed VLA15, in the first half of 2026. There is currently no approved vaccine available for the disease that is caused by a bacterial infection transmitted by infected tick bites. Recent estimates by the US Centers for Disease Control and Prevention (CDC) suggest that approximately 476,000 people may be diagnosed and treated for Lyme disease each year in the US. “VLA15’s successful commercialisation by Pfizer would enable Valneva to further expand and accelerate a pipeline of innovative vaccines in line with its strategic vision,” Valneva said in a statement. Valneva also plans to report the first Phase II data for tetravalent shigella vaccine candidate S4V2 in 2026. In-licensed from LimmaTech Biologics AG, Valneva will make a decision on the next steps of development for the jab in the second half of 2026. While shareholders prepare for pipeline updates this year, they could be set for another financially lukewarm year for Valneva. The company said it expects 2026 total revenues of between €155-170m – the higher end of this forecast would still represent a near $5m drop from 2025’s figure.
