The goal of this phase 1/2 clinical trial is to investigate the safety of an investigational drug called VIO-01 when taken by people who have different types of solid tumor cancers. There are two parts to this trial, part 1 and part 2.
Part 1 of the trial aims to answer these questions:
The safety and tolerability of VIO-01 when it is given alone or in combination with other anti-cancer therapies.
The highest dose that people can take without having unacceptable side effects
How well your body tolerates the drug alone or in combination, how they are absorbed, and the effects they have on your disease.
Part 2 of the trial will further test VIO-01's effect in participants with advanced HRRm or HRD+ solid tumors and HRRm/HRD+ recurrent ovarian cancer.
Participants will follow a schedule of visits to the study site to have assessments done related to their health condition and to receive the trial treatment.

Nanoparticles are widely used as carrier for the delivery of Active Principle Ingredients (API). From safety point of view, the fate of nanoparticles and API after administration becomes a key question. Today qual. and quant. biodistribution of such carriers or encapsulated API can be performed by chem. modification with a fluorescent probe or a radiolabeling and require a first acquisition step for visualization and a second step for quantification by tissues extraction LC-MS/MS can be considered but the information about the localization of the API is lost. After a long period focused on the sample preparation method development, Matrix-Assisted Laser Desorption/Ionization Mass Spectrometry Imaging (MALDI-MSI) is nowadays recognized as the most promising technique for the study of biodistribution of small mol. weight API without addnl. labeling, combining a visual and quant. approach. In this study, we extended the scope of use of MALDI-MSI to polycyanoacrylate nanoparticles loaded with doxorubicin. MALDI-MSI allowed the detection and quantification of doxorubicin and polycyanoacrylate, the polymer used as carrier, forming the nanoparticles in mouse liver section. Addnl., MALDI-MSI was able to detect doxorubicinol, the metabolite of doxorubicin. This Proof Of Concept was successful emphasizing the benefits of this technique (label free and quant. approach) and offers the possibility to use a unique technique for the detection of encapsulated API and the carrier opening a wide field of application in drug delivery of formulated API.

Comparison of the Systemic and Local Pharmacokinetics of Clonidine Mucoadhesive Buccal Tablets with Reference Clonidine Oral Tablets in Healthy Volunteers: An Open-Label Randomised Cross-Over Trial

INTRODUCTION:

The clonidine mucoadhesive buccal tablet (MBT) is a novel delivery system resulting in high and sustained concentrations of clonidine in the oral cavity. In a phase II clinical trial, clonidine MBT reduced the incidence of severe oral mucositis (OM) compared to placebo in head and neck cancer patients undergoing chemoradiation. This study compared the pharmacokinetics (PK), safety and tolerability of clonidine MBT with a reference oral tablet (OT).

METHODS:

This was a randomised, three-period, single-dose crossover study in 36 healthy subjects aged 18-50 years. Eligibility was assessed within 14 days of the first dose. IMP was administered in the fasted state on day 1 of each treatment period. PK samples were collected up to 24 h (saliva)/96 h (blood) for measurement of the clonidine concentration. Safety and tolerability were evaluated at specified times throughout the study. A washout period of at least 7 days was observed between administrations.

RESULTS:

Clonidine MBT (50 and 100 µg) applied to the upper gum resulted in a dose-proportional increase in saliva (C _max and AUC_0-t ) and plasma (C_max and AUC_0-inf) clonidine levels. Clonidine MBT was considered to mimic a continuous release of clonidine in plasma, significantly decreasing the C _max and AUC and increasing the T _max when compared with the reference clonidine HCl tablets. Clonidine MBT exhibited high and prolonged concentrations in saliva where concentrations with the clonidine HCl tablet were negligible. Clonidine MBT exhibited a favourable safety profile with significantly fewer subjects reporting AEs (dry mouth and fatigue) and a reduction in blood pressure when compared to the reference clonidine HCl tablets.

CONCLUSION:

Clonidine MBT is well tolerated and exhibits proportional saliva and plasma PK over the 50-100-µg dose level. The MBT results in higher saliva concentrations and lower systemic exposure than OT, which was associated with a trend towards fewer adverse events and less dry mouth, fatigue and hypotensive effect.

Existing pharmacological inhibitors for nicotinamide phosphoribosyltransferase (NAMPT) are promising therapeutics for treating cancer. By using medicinal and computational chemistry methods, the structure-activity relationship for novel classes of NAMPT inhibitors is described, and the compounds are optimized. Compounds are designed inspired by the NAMPT inhibitor APO866 and cyanoguanidine inhibitor scaffolds. In comparison with recently published derivatives, the new analogues exhibit an equally potent antiproliferative activity in vitro and comparable activity in vivo. The best performing compounds from these series showed subnanomolar antiproliferative activity toward a series of cancer cell lines (compound 15: IC50 0.025 and 0.33 nM, in A2780 (ovarian carcinoma) and MCF-7 (breast), respectively) and potent antitumor in vivo activity in well-tolerated doses in a xenograft model. In an A2780 xenograft mouse model with large tumors (500 mm(3)), compound 15 reduced the tumor volume to one-fifth of the starting volume at a dose of 3 mg/kg administered ip, bid, days 1-9. Thus, compounds found in this study compared favorably with compounds already in the clinic and warrant further investigation as promising lead molecules for the inhibition of NAMPT.
