The Effectiveness of Group Schema Therapy Utilizing Video Material in the Treatment of Borderline Symptoms in Adolescents: a Randomized and Controlled Intervention Study

The goal of this randomized and controlled trial is to assess the effectiveness of group schema therapy in the treatment of adolescents with borderline symptoms. The intervention utilizes self-recorded video material as an experiential method. The intervention comprises 30 group sessions and 8 individual sessions. Additionally, there are group sessions for the participants' parents. Participants in the control group receive treatment as usual.

Nicotinamide Adenine Dinucleotide (NAD+) Metabolism in Human Brown Adipose Tissue

A fully functional brown fat helps maintain a healthy weight and decreases the risk of metabolic diseases such as type II diabetes (T2DM). Unfortunately, in human adults, the functionality of brown fat declines with age, and it is one of the reasons for gaining unhealthy weight, particularly around the waistline (central obesity). Currently, we do not clearly understand the reasons for the decline in brown fat functionality. It is possible that the decline in the availability of the molecule Nicotinamide Adenine Dinucleotide (NAD+), which is central to several metabolic processes, plays a role in the decline in brown fat metabolism. This project will clarify whether NAD+-based molecular-targeted therapies for the enhancement of whole-body insulin sensitivity and brown fat metabolism will be successful in adult humans, which will eventually be an important target for reducing the development of obesity and its comorbidities such as T2DM.

A Clinical Tool to Relate Youth Risk Factors to Adult Cardiovascular Events and Type 2 Diabetes: The International Childhood Cardiovascular Cohort Consortium

OBJECTIVETo translate data relating childhood cardiovascular (CV) risk factors and adult CV disease and type 2 diabetes mellitus (T2DM) to clinically actionable values.STUDY DESIGNThis was a prospective observational study (n = 38 589) in the International Childhood Cardiovascular Cohort Consortium. Children at age 3 through 19 years were enrolled in the 1970s and 1980s and followed for more than 30 years. Five childhood CV risk factors (smoking, body mass index [BMI], systolic blood pressure, triglycerides, and total cholesterol) were related to adult CV events. Secondary analyses in a subset included low-density lipoprotein cholesterol, high-density lipoprotein cholesterol, glucose, and insulin level. Age- and sex-specific z scores were calculated for each risk factor, and a combined-risk z score was calculated by averaging z scores for the 5 key CV risk factors. Risk factor z scores were back-transformed to natural units for clinical interpretation, with hazard ratios for adult CV events presented in color-coded tables (green: no increased risk; orange: 1.4 to <2.0-fold increased risk; red: at least doubling of risk). Risk levels for development of adult T2DM on the basis of BMI, glucose, and insulin were similarly calculated and presented.RESULTSIncreased risk for CV events was observed at levels lower than currently defined abnormal clinical thresholds except for TC. Doubling of risk was observed at high normal levels just below the clinical cut point for abnormality. Risk for adult T2DM began at levels of BMI and glucose currently considered normal.CONCLUSIONSOn the basis of data showing significant relationships between childhood CV risk factors and adult CV events and T2DM, this study shows that risk in childhood begins below levels currently considered normal.

Parental psychopathology and offspring anxiety disorders in childhood and adolescence: A Finnish nationwide register study

BACKGROUNDResearch suggests an association between parental psychiatric disorders and offspring anxiety disorders, yet comprehensive studies are limited. This study aims to investigate the associations between various parental psychiatric disorders and anxiety disorders in their offspring.METHODUsing Finnish register data, this nested case-control study analyzed 867,175 singleton live births from 1992 to 2006, identifying 21,671 cases of anxiety disorders diagnosed between 1998 and 2016. Matched with four controls each (n = 72,414) based on age and biological sex, the study adjusted for demographic and prenatal factors using conditional logistic regression models to calculate odds ratios (aORs).RESULTSThere was a significant association between parental psychiatric disorders and increased risk of anxiety disorders in offspring. The risk was notably higher when both parents had psychiatric disorders (aOR = 5.04; 95 % CI, 4.70-5.39; p < .001). Maternal psychiatric disorders were strongly associated with offspring anxiety than paternal disorders (aOR 1.52; 95 % CI 1.43-1.61; p < .001). This association was consistent regardless of timing of parental diagnosis. Significant associations were observed between any parental psychiatric disorder and increased risks in offspring for specific phobia, social phobia, generalized anxiety disorder, separation anxiety, and panic disorder. Maternal psychiatric disorders showed elevated risk for panic disorder and social phobia among girls and separation anxiety and generalized anxiety among boys.CONCLUSIONThe findings emphasize the significant impact of parental psychiatric health on offspring anxiety disorders, highlighting the influence of genetic and environmental factors. Addressing parental mental health is crucial in preventing childhood anxiety disorders.

Childhood Risk Factors Directly Tied to Adult Cardiovascular Disease

MONDAY, June 24, 2024 -- Childhood risk factors are associated both directly and indirectly to adult cardiovascular disease (CVD), according to a study published online June 24 in JAMA Network Open . Noora Kartiosuo, from the University of Turku in Finland, and colleagues quantified the direct and indirect effects of childhood risk factors on adult CVD and their relative importance during different life-course stages. Analysis included 10,634 participants followed for a median 23.6 years. The researchers found that childhood risk factors (low-density lipoprotein cholesterol [LDL-C], total cholesterol [TC], triglycerides, systolic blood pressure [SBP], smoking, body mass index [BMI], and a combined score of these) were associated with CVD. There were direct pathways observed for childhood BMI (direct effect for incidence rate ratio [RR] per one standard deviation [SD] unit, 1.18) and LDL-C (direct effect for incidence RR, 1.16), while the indirect effects were larger for TC, triglycerides, SBP, and the combined score. CVD was only affected by childhood smoking via adulthood smoking. Childhood BMI plays nearly as important a role as adult BMI on risk of CVD, whereas for other childhood risk factors and the combined score, adulthood was the more important period. "These findings suggest that intervention for childhood risk factors, in particular BMI, is warranted to reduce incidence of adult CVD as it cannot be fully mitigated by risk factor management in adulthood," the authors write.
