(NYSE: PFE) today announced that the U.S. Food and Drug Administration (FDA) has accepted the company’s Biologics License Application (BLA) for its anti-tissue factor pathway inhibitor (anti-TFPI) candidate marstacimab for individuals living with hemophilia A or hemophilia B without inhibitors to Factor VIII (FVIII) or Factor IX (FIX). The European marketing authorization application (MAA) for marstacimab also passed validation and is currently under review by the European Medicines Agency (EMA). The FDA has set a Prescription Drug User Fee Act (PDUFA) action date in the fourth quarter of 2024, and a decision from the European Commission is anticipated by the first quarter of 2025. If approved in the U.S. and EU, marstacimab is expected to become the first once-weekly subcutaneous treatment for people living with hemophilia B and the first treatment administered as a flat dose for people living with hemophilia A or B. “Marstacimab has demonstrated that it may be an efficacious treatment option with once-weekly, subcutaneous flat-dose administration via an auto-injector pen, for appropriate patients, if approved. For appropriate patients living with hemophilia A and B, the goal of this treatment is to prevent potentially life-threatening bleeds with a once-weekly, subcutaneous flat-dose administration. The submissions for marstacimab are based on efficacy and safety data from the Phase 3 BASIS trial (NCT03938792). The inhibitor cohort of the BASIS trial has completed enrollment and is expected to read out as early as late 2024. About Marstacimab Marstacimab is a human monoclonal immunoglobulin G isotype, subclass 1 (IgG1) that targets the Kunitz 2 domain of tissue factor pathway inhibitor (TFPI), a natural anticoagulation protein that functions to prevent the formation of blood clots. Marstacimab is in development as a prophylactic treatment to prevent or reduce the frequency of bleeding episodes in individuals with hemophilia A or hemophilia B with or without inhibitors. In September 2019, the U.S. FDA granted Fast Track designation to marstacimab for routine prophylaxis to prevent or reduce the frequency of bleeding episodes in hemophilia A with inhibitors or hemophilia B with inhibitors. Pfizer has three Phase 3 programs investigating treatments for people living with hemophilia: marstacimab, fidanacogene elaparvovec (hemophilia B), and giroctocogene fitelparvovec (hemophilia A). About the BASIS study BASIS is a global Phase 3, open-label, multicenter study evaluating annual bleed rate (ABR) through 12 months of treatment with marstacimab, an investigational, novel subcutaneous therapy option, in approximately 145 adolescent and adult participants ages 12 to <75 years with severe hemophilia A (defined as FVIII <1%) or moderately severe to severe hemophilia B (defined as FIX activity ≤2%) with or without inhibitors. Approximately 15% of participants are adolescents (ages 12 to <18 years old). This study is comparing treatment with a run-in period for patients prescribed factor replacement therapy or bypass therapy during a six-month Observational Phase with a 12-month Active Treatment Phase, during which participants receive prophylaxis (a 300 mg subcutaneous loading dose of marstacimab, followed by 150 mg subcutaneously once weekly) with potential for dose escalation to 300 mg once weekly. Pfizer is also conducting BASIS KIDS, an open-label study investigating the safety and efficacy of marstacimab in children ages 1 to <18 years with severe hemophilia A or moderately severe to severe hemophilia B with or without inhibitors. The study will compare 12 months of historical standard treatment to marstacimab prophylaxis. About Hemophilia Hemophilia is a family of rare genetic blood diseases caused by a clotting factor deficiency (FVIII in hemophilia A, FIX in hemophilia B), which prevents normal blood clotting. (NYSE: PFE) today presented results from the pivotal Phase 3 BASIS clinical trial ( NCT03938792 ) evaluating marstacimab for the treatment of people with severe hemophilia A and moderately severe to severe hemophilia B without inhibitors to Factor VIII (FVIII) or Factor IX (FIX). The results from the BASIS trial demonstrated a statistically significant and clinically meaningful effect on annualized bleeding rate (ABR). The findings were presented today at the 65 th American Society of Hematology (ASH) Annual Meeting and Exposition in San Diego. “For more than five decades, the most common treatment for hemophilia A and B has been intravenous infusions that are often administered multiples times per week,” 1 said James Rusnak, M.D., Ph.D., Senior Vice President, Chief Development Officer, Internal Medicine and Infectious Diseases, Research and Development, Pfizer. “Based on these results and if approved, we believe marstacimab could offer a subcutaneous option with a compelling combination of efficacy and safety that may significantly reduce the risk of bleeding. We look forward to potentially bringing this treatment option to people living with hemophilia A and B without inhibitors.” In the BASIS trial, 116 people living with hemophilia were treated with marstacimab during a 12-month active treatment period (ATP) versus a routine prophylaxis (RP) and on-demand (OD) intravenous regimen with FVIII or FIX, administered as part of usual care in a six-month observational period. Marstacimab, a novel, investigational anti-tissue factor pathway inhibitor (anti-TFPI), was administered weekly with flat (not weight-based) dosing as a subcutaneous 300 mg loading dose followed by 150 mg once weekly. The study found: The safety profile for marstacimab was consistent with Phase 1/2 results and treatment was generally well-tolerated. No deaths were reported and there have been no thromboembolic events or events of consumptive coagulopathy recorded in hemophilia patients in clinical trials investigating marstacimab. The most commonly reported adverse events of special interest among patients treated with marstacimab in BASIS and the LTE (≥5% of patients) were COVID-19, hemorrhage, hepatic disorder, hypersensitivity, hypertension and injection site reaction. One treatment-related serious adverse event (SAE) was observed (peripheral swelling), and one patient discontinued from the trial due to a non-treatment-related SAE. “Recognizing the uncertainty that living with hemophilia can present for patients, the results from the BASIS trial are particularly encouraging as reductions in ABR were seen in the 12-month treatment period and then retained in long-term follow-up,” said Davide Matino, M.D., M.Sc., Assistant Professor of Medicine, McMaster University. “Based on these results, marstacimab has shown the potential to address the diverse needs of appropriate patients with hemophilia A or B without inhibitors with weekly subcutaneous administration in a flat dose that is not weight-based, and with low monitoring requirements.” Pfizer currently has three Phase 3 programs investigating novel treatment options for people living with hemophilia. Novo Nordisk's Henrik Jarlov insisted that there’s no head-to-head data available yet to truly compare the Big Pharmas’ rival options. Novo Nordisk insists its rival to Roche's Hemlibra will make a “good offering” for hemophilia patients even though mixed phase 3 data suggest that Pfizer’s own candidate could have an edge in the race to market. The Danish drugmaker is developing concizumab, an anti-tissue factor pathway inhibitor (TFPI) antibody, to treat people with both hemophilia A and hemophilia B. Having already posted impressive phase 3 data last year from the explorer7 trial of patients with inhibitors—an immune system response to the clotting factors—the Big Pharma used the International Society on Thrombosis and Haemostasis conference in Montreal to release data from the explorer8 trial in 148 patients without these inhibitors. The top-level numbers look good. The therapy induced an 86% reduction in spontaneous and traumatic bleeds in hemophilia A patients and a 79% reduction among trial participants with hemophilia B. However, the figures looked slightly less impressive when compared to Pfizer’s own anti-TFPI drug marstacimab, which posted a 92% reduction in bleeds in a phase 3 trial of patients without inhibitors less than a month ago. In a media briefing, Henrik Jarlov, Novo Nordisk’s vice president for medical affairs, rare disease, insisted that there’s no head-to-head data available yet to truly compare the Big Pharmas’ rival options. “We’ve only seen the high-level data from marstacimab, so I think we would like to wait until we’ve seen the full data set before the scientific community makes up their mind on a treatment option,” Jarlov said. Another area where today’s concizumab data came up short was in demonstrating noninferiority to previous hemophilia prophylaxis treatment that patients receive. This was most notable among hemophilia A patients, who had a mean annualized bleeding rate (ABR) of 5.1 for those who received concizumab compared to just 3.7 for those on another prophylaxis treatment. A similar problem was reflected in hemophilia B patients, with an ABR of 5.4 for the concizumab cohort compared to 3.1 for those on previous prophylaxis. Study investigator Anthony Chan blamed this discrepancy on three patients across the hemophilia A and B cohorts who showed “substantially” increased levels of bleeding. He pointed to the estimated median ABR as a better measure as it’s “less prone to outliers.” Indeed, on this measure, bleed rates were level among hemophilia A patients and even slightly lower for hemophilia B patients who received concizumab. Still, it’s another aspect where Pfizer now appears to have the upper hand, with marstacimab showing superiority to prophylaxis in its own phase 3 trial. Meanwhile, Novo Nordisk said the safety and tolerability profile of concizumab was within the expected range. Overall, there were 104 adverse events, including 14 that were deemed serious. One of these was a fatality, which was related to intra-abdominal bleeding, Chan said on the call. There have been no more thromboembolic events since the trial was paused in 2020 while two such cases were investigated, the company noted. Despite what looks set to be a tight race against Pfizer, both Chan and Jarlov were confident that conziumab had a role to play in the hemophilia landscape. With Novo Nordisk hoping that patients will be able to administer concizumab at home with a pen injector-style device, both speakers pointed to the therapy’s subcutaneous delivery as a benefit. “One of the facts that is worth keeping in mind with concizumab is that the device and the burden for patients in terms of administering the treatment is also something that we’ve seen quite good data in the explorer8 study,” Jarlov said. “We did not share the full data in terms of the reduction of [disease] burden for patients with hemophilia, but there’s also a clear impact there.” “I believe with what we’ve seen [of] concizumab with the subcutaneous option and the data that we have, I’m quite confident that that will be a good offering for many patients with hemophilia A [and] B, with or without inhibitors,” he added. Whether regulators agree remains another matter. While Canadian authorities have already given the green light, the FDA requested additional information on dosing from Novo Nordisk last month before it will make a call.
