Randomized Phase II/III Trial of Sentinel Lymph Node Biopsy Versus Elective Neck Dissection for Early-Stage Oral Cavity Cancer

This phase II/III trial studies how well sentinel lymph node biopsy works and compares sentinel lymph node biopsy surgery to standard neck dissection as part of the treatment for early-stage oral cavity cancer. Sentinel lymph node biopsy surgery is a procedure that removes a smaller number of lymph nodes from your neck because it uses an imaging agent to see which lymph nodes are most likely to have cancer. Standard neck dissection, such as elective neck dissection, removes many of the lymph nodes in your neck. Using sentinel lymph node biopsy surgery may work better in treating patients with early-stage oral cavity cancer compared to standard elective neck dissection.

Randomized clinical trial of imiquimod compared with fludroxycortide in the treatment of actinic cheilitis

OBJECTIVEActinic cheilitis (AC) is a potentially malignant disorder of the lip vermillion. The study of effective therapeutic options is of the utmost importance to prevent the development of lip squamous cell carcinoma. This study aimed to evaluate the topical effect of imiquimod 5% (IM) and fludroxycortide (FC) 0.125 mg/g creams in the treatment of AC.STUDY DESIGNA randomized controlled trial was conducted. Twenty-five participants of both genders, with a clinical diagnosis of AC, were divided into IM (n=13) and FC (n=12) groups. The treatment duration was six weeks for both groups. Participants were followed up by regular visits for clinical examination and photographic documentation.RESULTSAt the end of 180 days, the clinical outcomes were evaluated. In the IM group, there was a significant improvement in AC after 180 days of follow-up (P = .005). In the FC group, there was no significant improvement (P = .317). There was a significant difference between the groups in the clinical outcomes (P = .000).CONCLUSIONThis study suggests that imiquimod may be a treatment option for AC in more advanced clinical cases, but local reactions should be expected. Conversely, fludroxycortide demonstrated no clinical efficacy, and therefore, should be avoided in AC cases presenting with leukoplakia.

Upper Lip Reconstruction With the Anchor Flap: A Modified Technique of the Reverse Yu's Flap

Various reconstruction methods using local flaps have been reported for upper lip defects. The reverse Yu's flap is a reconstruction method consisting of three flaps including the advancement flap, rotating flap, and mucosal flap. It offers the advantage of low risk of microstomia occurrence because this method restores the form of the vermilion using buccal mucosa as a mucosal flap. Herein, we report the case of a 77-year-old Japanese man who underwent reconstruction using a modified reverse Yu's flap for a defect following the resection of right upper lip basal cell carcinoma. The patient presented with a 4.2×3.5 cm mass on the right upper lip. Resection was planned, and the anticipated defect in the vermilion following resection measured approximately 3.5 cm, making primary closure unfeasible. Thus, the defect was reconstructed using a reversed Yu's flap with minor modifications to accommodate the larger defect. The modifications to the technique are described herein and have been designated as the "anchor flap" due to the characteristic shape of the incision line.

A retrospective study of lip carcinoma at a tertiary cancer center

OBJECTIVEThis study aimed to evaluate treatment outcomes and identify survival predictors in patients with lip squamous cell carcinoma (SCC). Specific focus was placed on tumor subsite, stage, and histopathological differentiation, with an emphasis on prognostic differences.STUDY DESIGNA retrospective analysis was conducted on 294 lip SCC cases treated at a tertiary cancer center in India from 2012 to 2022. Patient demographics, tumor characteristics, treatment modalities, and survival outcomes were analyzed. Survival predictors were assessed using regression analysis, and Kaplan-Meier survival analysis was performed to evaluate subsite-specific outcomes.RESULTSLower lip SCC was the most prevalent (58.7%), followed by commissure (22.4%) and upper lip (14.3%). Advanced tumors (T4A) and commissure SCC were associated with significantly poorer outcomes (P = .02). Histopathologically, moderately differentiated SCC (45.9%) was predominant. Kaplan-Meier analysis showed the best survival for upper lip SCC (mean: 31.02 months) and the worst for commissure SCC (mean: 23.46 months). Multimodal therapy improved outcomes, particularly for advanced cases.CONCLUSIONSThis study highlights the critical role of tumor subsite and stage in determining survival outcomes in lip SCC. Tailored treatment strategies, especially for high-risk subsites like the commissure, are essential to improve prognosis.
