Effect of fourth-line antihypertensive therapy on clinic systolic blood pressure in resistant hypertension: a systematic review and meta-analysis

INTRODUCTION/BACKGROUND:

Resistant hypertension (rHTN) is defined as uncontrolled blood pressure despite ≥3 antihypertensives, including a diuretic. While fourth-line-intensification is recommended, comparative evidence remains scattered.

RESEARCH QUESTIONS/HYPOTHESIS:

To evaluate the efficacy and safety of fourth-line antihypertensives in reducing clinic systolic blood pressure (SBP) in rHTN.

METHODS/APPROACH:

PubMed, Embase, Scopus, and Web of Science were searched (2015-2026) for randomized controlled trials (RCTs) with ≥4 weeks follow-up. Risk of bias was assessed using Cochrane RoB 2. A planned network meta-analysis was not performed because statistical inconsistency and transitivity violations rendered indirect comparisons unreliable. Therefore, pairwise meta-analyses were conducted using random-effects models, with results reported as mean differences (MD) or risk ratios (RR) with 95% confidence intervals (CIs).

RESULTS/DATA:

Eleven RCTs involving 3,931 adults with rHTN were included. Compared with placebo, the largest SBP reductions were observed with lorundrostat (MD -11.70 mmHg [95% CI -16.07, -7.33]) and baxdrostat 2 mg (MD -10.10 [-12.63, -7.58]). Baxdrostat 1 mg (MD -8.56 [-11.08, -6.04]) produced reductions similar to spironolactone (MD -7.95 [-10.10, -5.80]). Bisoprolol achieved a modest SBP reduction (MD -6.71 mmHg), whereas aprocitentan 12.5 mg reduced SBP by -3.80 mmHg [-6.65, -0.95]. No intervention significantly increased serious adverse events. Higher rates of any adverse events were observed with aprocitentan 25 mg (RR 1.89 [1.39, 2.57]) and doxazosin (RR 1.55 [1.09, 2.20]).

CONCLUSIONS:

Aldosterone-targeted therapies, including aldosterone-synthase inhibitors and spironolactone, provide clinically meaningful SBP reductions in rHTN without increasing serious adverse event risk. Future trials should clarify comparative efficacy and long-term outcomes.

What is the Cost of Selectivity? Selective and Nonselective Alpha Blockade Costs Associated with Adrenalectomy for Pheochromocytoma

Abstract:

Background:

Alpha-blockade is utilized in the preoperative preparation of patients with pheochromocytomas. Both selective and nonselective alpha-blockade are safe with equivalent clinical outcomes; the goal of this study was to evaluate the comparative costs of selective and nonselective alpha-blockade.

Patients and Methods:

We performed a retrospective cohort study (2004–2022) of patients who underwent adrenalectomy for pheochromocytoma with preoperative alpha-blockade from Optum’s deidentified Clinformatics® Data Mart Database. Patients were stratified by treatment with selective (prazosin, doxazosin, terazosin) or nonselective (phenoxybenzamine) alpha-blockade. Primary outcomes were: (1) costs of alpha-blockade in the 30 days prior to surgery (AB) and (2) adjusted standard costs from admission to discharge (AC). Secondary outcomes included length of hospital stay (LOS), intensive care unit (ICU) admission, costs 30 days after discharge (AD), and cumulative costs (CC).

Results:

In total, 384 patients received selective, and 418 patients received nonselective alpha-blockade. The median age was 58 years (IQR 21 years). Median AB was significantly lower in the selective compared with nonselective alpha-blockade group ($19.73 versus $1033.70, p < 0.001). Median AC ($31,104.47 versus $31,471.90, p = 0.428) and AD ($790.78 versus $715.10, p = 0.074) were not significantly different between selective compared with nonselective alpha-blockade groups. On multivariable regression modeling, higher Elixhauser score (coeff.: $1,801.20, p < 0.001) and longer LOS (coeff.: $1842.33, p < 0.001) were associated with higher CC, while age (coeff.: $− 143.22, p = 0.044) was associated with lower CC. Notably, alpha-blockade strategy was not significantly associated with CC.

Conclusions:

When compared with nonselective alpha blockers, selective alpha blockers are associated with lower medication costs but equivalent hospitalization and post-hospitalization costs.

Design, optimization and characterization of mucoadhesive buccal film as a novel delivery system for doxazosin mesylate

Doxazosin mesylate is a selective α1-adrenergic receptor antagonist with low oral bioavailability due to poor aqueous solubility and extensive first-pass hepatic metabolism. This study aimed to design and optimize a doxazosin mucoadhesive buccal film to overcome hepatic metabolism and enable rapid drug onset. The buccal films were prepared by solvent casting, using a Plackett-Burman design to screen formulation variables affecting ex vivo mucoadhesive force, followed by a 2² full factorial design to optimize in vitro drug release by varying the concentrations of HPMC, the main film-forming polymer, and glycerin, a plasticizer. The prepared films were characterized for physico-mechanical properties, surface pH, mucoadhesive force, swelling properties, and drug release. In addition, the optimized formula was further characterized by scanning electron microscopy and ex vivo permeation study. The screening study showed that mucoadhesive strength ranged from 28 to 41 g, with the polymer type, polymer concentration, plasticizer type, and plasticizer concentration identified as the most influential variables. The optimized formula (FF3), containing 66 w/w% HPMC K15M and 21 w/w% glycerin, achieved 100% drug release within 15 min with acceptable film properties, in addition, SEM analysis showed a relatively uniform and compact surface with slight undulations, furthermore the ex vivo permeation reached 91% within 15 min, corresponding to a cumulative permeated amount of 1820 µg, a cumulative amount per unit area of 2275 µg/cm², and an estimated steady-state flux of 12750 µg/cm²/h. It was concluded that the developed buccal film is a promising system for improving doxazosin bioavailability and providing rapid therapeutic onset.

Exclusive: Fred Hutch duo's startup launches with $10M to build AI generative genomics models

If scientists run enough variations of a lab experiment, perhaps AI models could, someday, predict the results of future versions of that study. A new Seattle-based startup called Synthesize Bio believes it may be able to achieve that sooner than later, starting with gene-expression studies. Synthesize has already generated some promising results in predicting these types of lab experiments, according to a preprint published Tuesday. Synthesize will officially launch on Wednesday, its co-CEOs , Jeff Leek and Robert Bradley, exclusively told Endpoints News, as it releases its AI model, called GEM-1. The 15-employee biotech was founded in 2023 and raised a previously undisclosed $10 million seed round in October 2023, led by Madrona Ventures. Other investors include Sahsen Ventures, AI2 Incubator, Inner Loop Capital, and MD Venture Investments. Leek and Bradley came up with the idea behind Synthesize in the hallways of the Fred Hutchinson Cancer Center. Leek is the cancer center’s chief data officer, and Bradley is the scientific director of a translational data science research center. They wanted to see if biotech could follow a similar trajectory in building large AI models, similar to how companies like OpenAI and Anthropic grew large language models trained on nearly all of the internet’s data. “We felt like this is an idea where we believe it can transform how scientists go about their work,” Bradley said in an interview. The Fred Hutch duo built a model called GEM-1 over the last two years. The model is trained on tons of different gene-expression experiments that they curated and annotated from public datasets. Synthesize is already at work on the next-generation version of the model called GEM-2. “This is the worst version of the GEM model you will ever see,” Leek said. “It’s only going to get better over time.” Gene-expression experiments and single-cell studies have been a hot space within the AI bio field. Large organizations, like the Chan Zuckerberg Initiative and the Arc Institute , are championing moonshot projects that view these types of experiments as key building blocks in eventually making models of virtual cells. Virtual cells could allow more experiments traditionally done in the lab to be accurately simulated by computers. Other startups, like Noetik , Tahoe Therapeutics , and Xaira Therapeutics , are also taking their own twists on building large or rich datasets using some of these experiments. Synthesize believes its model can tackle a wider range of gene-expression experiments than the rest. To test that belief, they looked at how GEM-1’s predictions compared to real-world results. They set a cut-off date of mid-2024, training their model on experimental data in public databases before that date, and then using new experiments added after that date as tests. Overall, Synthesize’s team found GEM-1 “yielded accuracy comparable to the best-possible performance estimated by comparing the results of matched lab experiments,” according to the preprint. The preprint also found that about 95% of these new experiments were similar to those in GEM-1’s training data, meaning the model had already seen experiments that studied similar types of cells or similar perturbations. GEM-1’s predictions varied in accuracy. The model nailed the effect on gene expression of certain molecules, like the hair-loss drug finasteride and an alpha blocker called doxazosin. But predictions for other compounds were less than 50% accurate, like the HIV treatment dolutegravir or cancer drug afatinib, according to the preprint. An area of active research is working on confidence intervals, which could give users a sense of the quality of a prediction. Bradley said that’s something the broader AI field is also working on, as it’s an important problem. That could be critical in telling the finasteride and afatinib predictions apart. “Our goal is not to replace experiments,” Bradley said. “Our goal is to have a seamless blend of wet-lab work and AI experimentation. Experiments are critical. They’re always going to be the gold standard.” Instead, GEM-1 could allow scientists on finite budgets to consider more data for experiments they may not have the time or resources to run. It could also enable scientists to get a sense of results on experiments that are impossible or impractical today, Bradley said. For instance, the model could help predict a drug’s activity in tissue where you can’t take a patient’s biopsy, he said. Leek said there are blind spots, where GEM-1’s performance is wanting. That’s why they are eager to release the model openly, including free access on its website , to gain a sense of how researchers use it and its real-world performance.

Study suggests enlarged prostate drugs could protect against dementia with Lewy bodies

Researchers from the University of Iowa (UI) Health Care have suggested that certain drugs used to treat enlarged prostates have the potential to decrease the risk of dementia with Lewy bodies (DLB). Published in Neurology , the study revealed that three enlarged prostate drugs were able to increase the energy production in brain cells. Affecting one in every 1,000 people per year, DLB is the second most common type of dementia, which occurs when protein deposits called Lewy bodies develop in nerve cells in the brain, affecting thinking, memory and movement. Researchers identified more than 643,000 men with no history of DLB who were newly starting one of six drugs used to treat benign prostatic hyperplasia and followed the participants for around three years from when they started taking the medication until they left the database or developed DLB. The observational study found that the already-approved Abbott’s Hytrin (terazosin), Pfizer’s Cardura (doxazosin) and Sanofi’s Uroxatral (alfuzosin) boosted the energy production in brain cells, which could help to slow or prevent neurodegenerative diseases including DLB and Parkinson’s disease (PD), while the three other drugs, tamsulosin and two 5-alpha-reductase inhibitors (5ARIs), finasteride and dutasteride, did not. Men who took terazosin had around a 40% lower risk of developing DLB versus men taking tamsulosin and about a 37% reduction in risk compared to men taking 5ARIs, with no statistically significant difference in risk between men taking tamsulosin and ARIs. “If terazosin and these similar medications can help slow this progression – if not outright preventing the disease – this would be important to preserving cognitive function and quality of life in people with DLB,” said lead study author, Jacob Simmering, UI assistant professor, internal medicine. Previous studies have already linked certain drugs and treatments to other neurodegenerative conditions. In February, researchers from University College London revealed that erectile dysfunction drugs could also be linked to a reduced risk of developing Alzheimer’s disease (AD), the most common form of dementia. Of nearly 270,000 men over the age of 40, those who were prescribed phosphodiesterase 5 inhibitors were 18% less likely to develop AD compared to those who were not.
