Adenosine is a commonly used pharmaceutical stressor at cardiac magnetic resonance examinations to assess suspected chronic coronary syndrome (CCS). However, several studies have reported that the current use of adenosine does not induce adequate hyperemic response in a substantial number of patients, leading to false diagnostics. The aim of this trial is to investigate the hyperemic effect of the standard dose of adenosine (140 microgram/kg/min) to the high dose of adenosine (210 microgram/kg/min) to improve the diagnostic methods using adenosine as a stressor and ultimately improve treatment decisions and patient prognosis in CCS.

Adenosine regulates depressive behavior in mice with chronic social defeat stress through gut microbiota

Major depressive disorder (MDD) is recognized as the most prevalent affective disorder worldwide. Metagenomic studies increasingly support a critical role for dysbiosis of gut microbiota in the development of depression. Previous studies have demonstrated that adenosine alleviates gut dysbiosis, suggesting that elevating adenosine levels could be a novel intervention for MDD; however, the mechanisms underlying this effect remain unclear. This study utilized 16S rRNA gene sequencing, fecal microbiota transplantation (FMT) and ultra-performance liquid chromatography-tandem mass spectrometry (UPLC-MS/MS) to test the hypothesis that increased adenosine alleviates depressive behaviors in male mice subjected to chronic social defeat stress (CSDS) through alterations to gut microbiota. The data showed that depression-susceptible (SUS) mice exhibited gut dysbiosis, and FMT from SUS mice increased depression-like behaviors in healthy recipients. In SUS mice, adenosine supplementation ameliorated both depression-like behaviors and abnormalities in gut microbiota, and co-administration of probiotics and adenosine not only mitigated depression-like behaviors but also enhanced gut barrier integrity. By including 83 depressed adolescents and 67 healthy controls, this study found that the level of short-chain fatty acids (SCFAs) in the depression group was reduced, this finding parallels reductions seen in SUS mice and in recipient mice after FMT from SUS donors. Conversely, supplementation with either adenosine or probiotics led increased SCFAs concentrations in the serum of SUS mice. These findings suggest that adenosine may alleviate depression-like behaviors in CSDS mice by modulating the gut microbiota. This effect is likely associated with increased serum SCFAs, metabolites produced by the gut microbiota, following adenosine supplementation.

Coronary flow reserve (CFR) and index of microcirculatory resistance (IMR) obtained through coronary bolus thermodilution are used to assess and treat patients with angina and no obstructive coronary artery disease. Previous studies demonstrate comparable results assessing epicardial ischemia by fractional flow reserve using intravenous (IV) or intracoronary (IC) adenosine. It is unknown if there is a similarity between IC and IV hyperemia with adenosine when performing coronary reactivity testing (CRT). We reviewed CRT data and baseline demographics in a cohort of patients who underwent CRT for ischemia and no obstructive coronary artery disease. We evaluated CFR and IMR in patients whereby maximal hyperemia was obtained by both IC and IV means using linear regression, one-way analysis of variance, Wilcoxon, and Bland-Altman analysis. We assessed 62 patients with a median age of 60.5 years (50 to 67), and 72% were females. The average CFR with IC adenosine was 3.12 (2.31 to 4.06) and 2.71 (2.0 to 3.88) with IV adenosine, with an R² value of 0.50 (p <0.0001). The average IMR with IC adenosine was 28.23 (16.24 to 50.72) and 22.27 (14.79 to 37.0) with IV adenosine, with an R² value of 0.33 (p <0.0001). Average intra-method variability between IC and IV adenosine was nonsignificant (p = 0.31 for CFR and p = 0.55 for IMR). Bland-Altman analysis showed reasonable agreement between IV and IC adenosine for CFR and IMR with slightly higher values using IC adenosine. Therefore, in CRT with bolus thermodilution, CFR and IMR values obtained with IC adenosine correlate well with those obtained with IV adenosine. This presents a potential alternative to IV adenosine for bolus thermodilution CRT.

Integrating network pharmacology with molecular docking for elucidation of molecular biological mechanisms of Jiedu Qingjin formula for non-small cell lung cancer

Traditional Chinese medicine is an important part of complementary alternative medicine. Jiedu Qingjin formula (JDQJF) is an effective national invention patent for the treatment of non-small cell lung cancer (NSCLC). We investigated the molecular biological mechanisms based on network pharmacology, molecular docking, and molecular dynamics simulations. Compounds of JDQJF were screened through the TCMSP, ETCM, and literature. Targets were searched by DrugBank and predicted by SwissTargetPrediction. GEO database was applied for screening differentially expressed genes between cancerous tissues and healthy tissues of NSCLC. Subsequently, the protein-protein interaction between JDQJF and NSCLC were obtained by Cytoscape. Visual analyses were carried out to extract candidate genes, then subjected to Metascape for enrichment analyses. Finally, molecular docking was performed by AutoDock, and the best complexes were subjected to molecular dynamics simulation and binding energy calculations by MMPBSA. A total of 273 compounds, 390 targets, 3146 GO terms, and 174 KEGG pathways were obtained. Five potential compounds (quercetin, adenosine, apigenin, heptadecanoic acid, and luteolin) were notably modulated by key targets AKT1, MAPK3, and RAF1. Enrichment results included cell cycle process, growth transduction factor, immune response-activating transduction, and involved PI3K/AKT, MAPK, NF-κB and VEGF pathway. RAF1-quercetin showed the highest binding affinity (-9.1 kcal/mol), revealed stable interactions during the simulation, and the highest estimated relative binding energy of the RAF1-Heptadecanoic was -184.277 kcal/mol. This study suggested that EMT-related, inflammation-related, immune-related, and angiogenesis-related pathways may be associated with JDQJF, and involved in the advancement of NSCLC, which points out the research direction for subsequent utility mechanism validation.Communicated by Ramaswamy H. Bioengineered enzyme creates natural vanillin from plants in one step

Vanilla, the most widely used flavoring compound in confectionaries and cosmetics, gets its sweet flavor and aroma from the chemical compound -- 'vanillin'. However, the large-scale production of natural vanillin is impeded by the lack of microbial processes and enzymes which can commercially generate vanillin. Now, researchers have genetically engineered a novel enzyme which can convert ferulic acid from plant waste into vanillin in a one-step sustainable process. Vanilla extract is one of the most widely used flavoring compounds in food products and cosmetics. The pleasant and sweet smell of this classic flavor is imparted by the chemical compound 'vanillin' found in the seed pods of vanilla plants belonging to the orchid family. In plants, vanillin is synthesized by the conversion of ferulic acid by the enzyme -- VpVAN. However, laboratory biosynthesis of vanillin from plant-derived VpVAN yields only very small quantities of vanillin, and is, therefore, commercially impractical. Further, although chemically derived vanilla essences are available cheaply, they do not match the flavor of natural vanilla extract, and the demand for the latter continues to remain high. Additionally, climatic restrictions for the cultivation of vanilla plants, and the relatively small yield obtained per plant, have led to a dwindling supply and a surge in the price of natural vanilla extract. Addressing these challenges, Professor Toshiki Furuya from the Department of Applied Biological Science, Faculty of Science and Technology, Tokyo University of Science, and his graduate students Shizuka Fujimaki and Satsuki Sakamoto, successfully developed an enzyme that generates vanillin from plant-derived ferulic acid. "Ferulic acid, the raw material, is a compound that can be obtained in abundance from agricultural waste such as rice bran and wheat bran. Vanillin is generated simply by mixing ferulic acid with the developed enzyme at room temperature. So, the established technology can provide a simple and environmentally friendly method for producing flavor compounds," explains Prof. Furuya. Their study was published on May 10, 2024 in Applied and Environmental Microbiology. The researchers used genetic engineering approaches to modify the molecular structure of an enzyme -- 'Ado.' Ado is originally an oxidase enzyme that adds an oxygen atom to the substrate -- isoeugenol. In its native state, it does not have the ability to convert ferulic acid into vanillin. Using structural modeling analysis, the researchers were able to predict amino acid changes in Ado which would enable its interaction with ferulic acid. On these lines, they conducted a series of experiments by replacing phenylalanine and valine amino acid residues at specific positions in the structure of Ado, with various other amino acids. They went on to examine the ferulic acid conversion ability of the various engineered mutant proteins. Following several trials and errors, they found that a mutant protein in which only three specific phenylalanine and valine residues were replaced with tyrosine and arginine, stably reacted with ferulic acid and exhibited high conversion activity. Notably, the engineered enzyme did not require any cofactors for conversion, unlike other oxidases, and produced vanillin on a gram scale per liter of reaction solution, with a higher catalytic efficiency and affinity than that of the wild-type enzyme. The reaction only required mixing of the enzyme, ferulic acid, and air (molecular oxygen), at room temperature, making it a simple, sustainable, and economically scalable process. Furthermore, the molecularly evolved enzyme also exhibited conversion activity towards p-coumaric acid and sinapic acid, which are compounds obtained from the degradation of lignin -- a common agricultural waste product. So far, no microbial or plant-derived enzymes have exhibited the ability to convert ferulic acid to vanillin at an industrial scale. Therefore, the enzyme developed in the current study shows considerable potential for enabling the commercial and economically viable production of natural vanillin. Explaining the long-term implications of their research, Prof. Furuya states, "Harnessing the potential of microorganisms and enzymes to derive valuable compounds under mild conditions from renewable plant-based resources, offers a sustainable approach to minimizing environmental footprint. Incyte whittles down pipeline, scrapping 6 R&D programs

Incyte's pipeline cuts will allow the pharma to funnel resources to “programs that can have a high impact for patients and for Incyte," according to CEO Hervé Hoppenot. Incyte is whittling down its R&D pipeline, scrapping six discovery programs—one in a rare form of anemia and five in various cancers—to focus on eight remaining programs. The Jakafi maker’s remaining R&D programs “have high potential,” Incyte CEO Hervé Hoppenot said in a May 2 release. The pipeline trim will allow the pharma to funnel its resources to “programs that can have a high impact for patients and for Incyte." The company has ended phase 3 work on a small molecule PI3K inhibitor—known as parsaclisib—in warm autoimmune hemolytic anemia, citing the “challenging regulatory landscape associated with the PI3K class.” While several PI3K inhibitors have managed to snag FDA approval, the drug class has been frequently tied to serious or even fatal side effects. Last year, the FDA lost confidence in the safety profile of PI3K inhibitors for blood cancers after data from six randomized clinical trials indicated that four marketed PI3K inhibitors may shorten blood cancer patients’ life expectancy because of their toxicity. Since then, the agency said it would put the meds under a more stringent regulatory scope. Incyte’s parsaclisib had also been assessed in a phase 3 trial as a combo treatment alongside the pharma’s approved JAK inhibitor Jakafi for certain cancers. However, the pharma’s first effort at building a better Jakafi regimen ended in failure after an independent data monitoring committee found it was on course to miss the primary endpoint of reducing spleen volume in patients with bone marrow cancer myelofibrosis, and the study was discontinued. Incyte has also ended development of its adenosine program, which includes four separate assets. First on the list is INCB106385, which was being assessed in two separate phase 1 trials among patients with advanced solid tumors. One of those trials also evaluated INCA00186, which is being discontinued, alongside anti-GITR antibody INCAGN1876 in various cancers and INCB81776 in advanced malignancies. All the adenosine programs were discontinued based on early efficacy data, according to Incyte’s release. While Incyte continues to expand the reach of pemazyre—the first FDA-approved drug designed to specifically treat bile duct cancer—in non-U.S. countries, the pharma has ended a phase 2 clinical trial dubbed FIGHT-210 that was testing out the drug in non-small cell lung cancer. So what remains? One of the eight programs the company is zooming in on is INCB123667, a CDK2 inhibitor being studied in a phase 1 trial for patients with advanced malignancies. Another of the surviving programs is INCA33890, a bispecific antibody that has shown anti-tumor effects in preclinical studies. Alongside other discovery programs, Incyte continues to work to expand labels for its approved drugs. Despite the R&D overhaul, the drug developer still seems to be investing in new science for its pipeline. Last month, Incyte paid out $7 million cash to use Biotheryx’s platform to identify and initially develop molecular glue degraders for “multiple historically undruggable oncology targets.” The deal gives Biotheryx the chance to make another $6 million in R&D funding and up to $347 million in milestone payments.
