Emphysema is a progressive phenotype of chronic Obstructive Pulmonary Disease (COPD), poses a significant global health burden characterized by irreversible lung parenchymal destruction and airspace enlargement.
In recent years, bronchoscopic lung volume reduction (BLVR) has considerable interest as a minimally invasive approach to reducing hyperinflation and improving lung function in patients with emphysema.
BLVR techniques aim to achieve lung volume reduction by occluding or ablating emphysematous lung tissue through endobronchial delivery of various agents. Many techniques are available for BLVR as coil, endo-bronchial valve, and biological as thrombin, autologous blood and chemicals as silver nitrate solution.

Investigation of the effect of nitrate and L-arginine intake on aerobic, anaerobic performance, balance, agility, and recovery in elite taekwondo athletes

BACKGROUNDTaekwondo is a complex martial art that requires speed, balance, agility, and endurance. This study aims to examine the effects of nitrate and L-arginine supplementation on acute aerobic and anaerobic performance, balance, agility, and recovery in elite taekwondo athletes.METHODThis study was conducted as a double-blind, randomized, crossover study with the participation of 15 experienced taekwondo athletes aged 19.06 ± 0.96 years and 8.93 ± 1.27 years of training experience. Participants visited the laboratory a total of nine times, including a practice session and anthropometric measurements. These visits consisted of eight experimental sessions conducted at 72-hour intervals. The experimental sessions were conducted with nitrate, L-arginine, and a combination of both supplements (NIT*L-ARG) and placebo. Nitrate supplementation was provided by homogenizing fresh spinach (837.40 mg/kg), while L-ARG was given as a single dose of 6 g in powder form three hours before exercise.RESULTSNIT*L-ARG supplementation significantly improved the anaerobic performance of athletes in Wingate peak power and peak power (w/kg) compared to placebo and in mean power compared to NIT, L-ARG, and PLA. In addition, NIT*L-ARG supplementation significantly improved blood lactate levels and agility performance immediately after Wingate and Shuttle run tests.CONCLUSIONThe combined intake of NIT*L-ARG was found to be effective in improving aerobic, anaerobic, and agility performances as well as fatigue levels of athletes. It was determined that taking NIT and L-ARG supplements alone contributed to the improvement of improving athletes' performance in Wingate mean power values and subsequent fatigue level compared to PLA.

Comparative effectiveness of silver-coated implants in periprosthetic infection prevention: A systematic review and meta-analysis

IntroductionDespite the implementation of numerous preventive measures in recent years, the persistent challenge of periprosthetic infections remains. Among the various strategies, metallic modification of implants, particularly with silver, has emerged as a promising avenue. Silver's antimicrobial properties, coupled with its low human toxicity, render it an appealing option. However, ongoing debate surrounds its comparative efficacy in infection prevention when contrasted with titanium-coated prostheses.MethodsThe PubMed database was systematically searched up to March 2024. Studies in English that met predetermined inclusion/exclusion criteria and utilized "Megaprosthesis AND infection" and " silver-coated AND infection " as key terms were included. The Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) statement guided the article selection process.ResultsFrom a pool of 1892 potential papers after literature screening, 11 studies with a total of 1419 patients were meticulously selected for analysis. Among these patients, 638 were treated with silver-coated implants, while 781 received titanium-coated implants, resulting in 166 recorded cases of infection. Remarkably, the infection rate stood at 9.2 % for the silver-coated group, contrasting with 13.4 % for the titanium-coated group. The subsequent analysis unveiled a notable discrepancy in proportions (P difference = -0.0473, 95 % CI: -0.088 to -0.006), signaling a statistically significant decrease in infections within the silver-coated cohort. Furthermore, the I2 statistic, denoting heterogeneity in effect sizes, stood at 21.8 % (95 % CI: 0.0-66.9), indicating a modest degree of variability among the studies. These findings offer compelling insights into the comparative effectiveness of silver-coated implants, suggesting their potential superiority in infection prevention.ConclusionIn conclusion, our systematic review and meta-analysis shed light on the potential of silver-coated implants in mitigating periprosthetic infections. Despite the persistent challenge posed by such infections, our findings suggest a statistically significant decrease in infection rates among patients treated with silver-coated implants compared to those with titanium-coated ones.

Xeroform gauze versus silver sulfadiazine for mixed-depth pediatric scald injuries: A retrospective study

IntroductionSilver sulfadiazine 1 % cream had historically been the mainstay initial treatment for scald wounds at our institution. However, we transitioned to using closed dressings of only petrolatum-impregnated 3 % bismuth tribromophenate gauze (Xeroform) for all partial-thickness burns. Xeroform adheres to the wound while allowing the exudates to drain and acts as a scaffold for re-epithelialization, after which it falls off without traumatizing the wound bed, theoretically allowing viable tissue to declare itself while requiring less frequent dressing changes.Materials and MethodsA retrospective chart review was conducted of patients aged ≤5 years with mixed-depth scald injuries between the years 1) 2004 and 2008, during which silver sulfadiazine was the standard initial choice and 2) 2015 and 2018, when only Xeroform was used as the standard.ResultsThe study included 347 patients, among whom 200 were treated with silver sulfadiazine and 147 were treated with Xeroform alone. The 2 groups had similar burn sizes and rates of skin grafting (silver group 30/200 [15.0 %] and Xeroform group 20/147 [17.7 %]) However, the Xeroform group showed longer time from injury to grafting (24 vs. 9.9 days, p = 0.002) but showed a significantly smaller mean graft size than the silver group (147 vs. 336 cm², p = 0.027).ConclusionsThese findings suggest that using Xeroform may promote better wound healing than using silver sulfadiazine. In addition, patients with Xeroform can be discharged with their dressings in place for grafting in the outpatient setting, during which time they are in closed dressings without frequent changes and associated discomfort.

Orthobond wins FDA de novo approval for antibacterial technology that could have vast device applications

The Monmouth Junction, New Jersey-based device developer says it’s the first time the FDA has granted a de novo request for a non-eluting coating designed to actively kill bacteria that could contaminate the surface of a medical device. Orthobond was founded by Princeton University Chemistry Professor Emeritus Jeffrey Schwartz and the late Dr. Gregory Lutz, who was physiatrist-in-chief emeritus at the Hospital for Special Surgery. Lutz died of cancer on March 5, but knew FDA approval was on the horizon. Orthobond CEO David Nichols called the FDA approval a “testament to Gregory’s legacy” in a statement shared with MassDevice. “We are proud to be able to carry it on into this next chapter.” The first application will be Ostaguard-treated SeaSpine Mariner pedicle screws, according to FDA records. Those could be used in patients early next year, Nichols (a former Zimmer Biomet executive) said in an exclusive interview with MassDevice. Orthobond’s proprietary technology is a quaternary ammonium compound coating that’s highly effective in neutralizing the different bacteria responsible for most device-related infections and can be applied to almost any device, he said. It doesn’t come off the device — it is considered a device — and it actively kills bacteria.” The FDA gave the new category a generic name of “spinal fusion device with quaternary ammonium compound coating,” defined as “a rigid metallic implant device or system comprised of single or multiple components intended to facilitate fusion in skeletally mature patients. The device includes a quaternary ammonium compound coating that is covalently bonded to the device. Where applied, the coating is intended to reduce microbial contamination on the surface of the device prior to implantation. The device does not contain antimicrobial agents that act within or on the body and this device type does not include combination products.” According to an approval letter shared by Ostaguard, the FDA classified the product as a Class II device and established special controls, which Orthobond declined to divulge. While Orthobond said it found no evidence of local or systemic toxicity in sheep, guinea pig and canine testing, the coating technology has not been evaluated in human clinical trials. Orthobond says the technology’s intellectual property is protected through 2037 through three patent fences covering the covalent surface chemistry, antimicrobial layer and analytical processes, including 27 patents and 12 pending applications. Orthobond intends to seek 510(k) clearances for additional orthopedic spinal implant indications before expanding to other fields such as joint reconstruction, oncology, sports medicine, plastic surgery and cardiovascular. “We’re working on a neuromodulation device, there’s a trauma device we’re working on, and we’re going to work on a cochlear implant,” Nichols said. “If you use a cochlear implant and it gets infected, you pretty much lose your hearing. That’s something that they want to solve.” “We’d like to be as broad as possible,” he continued. “… This should be on all devices that get used. There’s no reason why not.
