Stress urinary incontinence (SUI) affects at least 40% of women in the United States. Synthetic polypropylene mid-urethral slings (MUS) are the gold standard treatment for SUI. Post-operative urinary tract infections (UTI) are one of the most common complications after MUS placement. Some studies have demonstrated that MUS placement can increase the risk of UTI up to 21-34%.
Post-operative UTI can lead to significant healthcare and patient burden. This additional burden further contributes to an estimated annual cost of $1.6 billion for UTI management in the United States. With increased antibiotic usage, there is simultaneous increase in bacterial resistance leading to treatment refractory UTI.
The investigators prescribe post-operative antibiotics prophylactically for 3 days after MUS placement with or without concurrent pelvic reconstructive surgery based on prior literature recommending post-operative prophylaxis. There is a greater emphasis on limiting antibiotic use given the trend of development of bacterial resistance. There are studies supporting alternatives such as methenamine for recurrent UTI prophylaxis treatment, but there are limited studies evaluating methenamine for UTI prophylaxis after MUS.

A Multidisciplinary, Multimodal Bundled Care Approach to Chronic Pelvic Pain

The goal of this clinical trial is to compare two different treatment patterns in patients with chronic bladder pain. The main questions the investigators are seeking to answer is if bladder pain improves before and after treatment using a painful bladder scale. The participant will have 5 visits to evaluate your symptoms with questionnaires, at least one procedural visit, and must participate in physical therapy and some kind of behavioral health therapy. This study will assess participant response to a bundled-care approach to chronic bladder pain both pre-and post intervention as well as compared to a group of participants receiving typical care.

Low digestibility characteristics of white kidney bean protein hydrolysate-corn starch complex: Structural and functional transformations induced by heat-moisture treatment

This study prepared complexes using corn starch and white kidney bean protein (KBP) and its hydrolysates (KBPH60 and KBPH180, hydrolysates derived via enzymatic hydrolysis for 60 and 180 min, respectively) through heat-moisture treatment (HMT). The impact of HMT on the structure, physicochemical properties, and in vitro starch digestibility of these KBP/KBPH-starch complexes was systematically analyzed. Fourier transform infrared spectroscopy (FTIR) and X-ray diffraction (XRD) results showed that KBP and its hydrolysates stabilized the starch structure weakened by HMT through hydrogen bonding, promoting a more ordered crystalline arrangement and decreasing starch digestibility. Scanning electron microscopy (SEM) confirmed that HMT promoted the encapsulation of starch granules by KBP/KBPH, acting as a barrier to amylase. Both HMT and KBP/KBPH inhibited starch gelatinization, reducing paste viscosity and disrupting the starch gel network. This increased free water content and softened gel texture. HMT intensified these effects, increasing resistant starch (RS) content. KBPH, particularly HMT-KBPH180-S (heat-moisture treated complex of KBPH180 and corn starch), exhibited a pronounced inhibitory effect on starch digestibility, yielding the highest RS content. These results provide new insights into regulating starch digestibility and stability through protein hydrolysates and HMT, laying a theoretical foundation for the developing functional foods with low digestibility.

Metabolome and oxidative stress markers in the seminal plasma of Holstein bulls and their relationship with the characteristics of fresh and frozen/thawed sperm

Seminal plasma composition has important role in sperm functionality and its freezability. The objective of this study was to test the hypothesis that seminal plasma (SP) oxidative status and metabolome are associated with fresh semen characteristics and freezability of bull sperm. To accomplish this objective, oxidative status markers and metabolome of SP of ejaculates obtained from 20 Holstein bulls (3 for each bull) were analyzed using spectrophotometry and nuclear magnetic resonance (¹H NMR). The ejaculates were classified into higher motility fresh semen (HMF) and lower motility fresh semen (LMF), according to total motility (TM) and progressive motility (PM) values of fresh semen. Then the ejaculates was cryopreserved and assigned to higher motility thawed group (HMT) or lower motility thawed group (LMT) according to TM and PM at 0 h post-thawing. Multivariate analyses were performed to identify the association between the functional characteristics of fresh and thawed semen and the SP parameters, in terms of the oxidative status and the metabolomic composition. According to our results, the advanced oxidative protein products (AOPP) and thiol concentrations in SP are significantly related to some physiological characteristics of the thawed sperm, such as higher viability, TM, PM and LIN and lower mitochondrial and cytoplasmic superoxide production in viable thawed cells. In contrast, a higher amount of C in the SP was negatively related to TM and PM of thawed semen and was associated with higher mitochondrial and cytoplasmic superoxide production. In addition, partial least squares-discriminant analysis (PLS-DA) performed on the ¹H NMR spectra indicated a discrete separation between HMF and LMF groups, and good discrimination between HMT and LMT groups. Higher levels of formic acid, lactate, glycerol and phosphocholine, were found in the SP of the HMF group than in the LMF group. On the other hand, alanine, phenylalanine, and tyrosine were higher in the SP of the LMF group than in the HMF group. GABA, glutamate, histidine and glycerol were found in higher concentrations in the HMT group than in the LMT group, while fructose decreased in the HMT group. Our results showed that the oxidative and metabolomic status of SP is related to the physiological properties of semen and its freezability and open new fields in research of SP biomarkers of bull semen preservation and fertility.

Removal of proteins and lipids affects structure, in vitro digestion and physicochemical properties of rice flour modified by heat‐moisture treatment

AbstractBACKGROUNDThe objective of this experiment was to investigate the role of endogenous proteins and lipids in the structural and physicochemical properties of starch in heat‐moisture treatment (HMT) rice flour and to reveal their effect on starch digestibility under heat.RESULTSThe findings indicate that, in the absence of endogenous proteins and lipids acting as a physical barrier, especially proteins, the interaction between rice flour and endogenous proteins and lipids diminished. This reduction led to fewer starch–protein inclusion complexes and starch–lipid complexes, altering the granule aggregation structure of rice flour. It resulted in a decrease in particle size, an increase in agglomeration between starch granules, and more surface cracking on rice granules. Under HMT conditions with a moisture content of 30%, slight gelatinization of the starch granules occurred, contributing to an increased starch hydrolysis rate. In addition, the elevated thermal energy effect of HMT enhanced interactions between starch molecular chains. These resulted in a decrease in crystallinity, short‐range ordering, and the content of double‐helix structure within starch granules. These structural transformations led to higher pasting temperatures, improved hot and cold paste stability, and a decrease in peak viscosity, breakdown, setback, and enthalpy of pasting of the starch granules.CONCLUSIONThe combined analysis of microstructure, physicochemical properties, and in vitro digestion characteristics has enabled us to further enhance our understanding of the interaction mechanisms between endogenous proteins, lipids, and starches during HMT. © 2024 Society of Chemical Industry.

Hormone-Modulating Therapy May Reduce Dementia Risk in Breast Cancer

THURSDAY, July 25, 2024 -- For women with breast cancer, hormone-modulating therapy (HMT) is associated with reduced risk of Alzheimer disease and related dementias (ADRD), according to a study published online July 16 in JAMA Network Open . Chao Cai, Ph.D., from the University of South Carolina in Columbia, and colleagues examined the association between HMT for breast cancer treatment and the risk of developing ADRD among women aged 65 years or older in a cohort study. Of 18,808 women aged 65 years and older diagnosed with breast cancer between 2007 and 2009, 65.7 and 34.3 percent received and did not receive HMT, respectively, within three years after diagnosis. The researchers found that during an average of 12 years of follow-up, 23.7 and 27.9 percent of HMT users and non-HMT users developed ADRD. Overall, HMT was associated with a reduced relative risk of ADRD (hazard ratio [HR], 0.93). The association varied by race and decreased with age. The greatest reduction in ADRD risk associated with HMT was seen for women aged 65 to 74 years who self-identified as Black (HR, 0.76); among women aged 75 years or older, the association was attenuated (HR, 0.81). The relative risk reduction was lower for women aged 65 to 74 years who self-identified as White (HR, 0.89) and was no longer significant for those aged 75 years or older. No significant association was seen for HMT and ADRD for other races. "Further research is warranted to validate the observed associations in diverse populations and to elucidate the mechanisms underlying our observations," the authors write.
