A Phase II, Open Label, Randomized Controlled Pilot Study Evaluating Trimethoprim in Patients Commencing Androgen Deprivation Therapy for Prostate Cancer

The researchers hope to learn if Trimethoprim, 150mg taken daily for 3 months, is a safe and effective way to reduce folate levels at time of Androgen Deprivation Therapy (ADT) in men with advanced prostate cancer.
This study will help find out what effects, good and/or bad, this drug has on people who take it and on its effect on the disease. The safety of this drug in humans has been tested in prior research studies; however, some side effects may not yet be known.

REINFORCE - Reducing Infection-related Readmissions Following Cystectomy. a Multicentre Randomised Clinical Trial Testing Superiority of Individualised Targeted Antibiotic Prophylaxis Over Empiric Prophylaxis At Ureteral Stent Removal to Reduce Infection-related Readmissions Following Cystectomy.

The aim of this trial is to test whether postoperative antibiotics targeted towards bacteria in the urine can reduce the risk of infection after surgical removal of the bladder (cystectomy) compared to a standardised antibiotic prophylaxis.
Participants undergoing cystectomy will be randomly assigned to postoperatively receive (A) a standardised orally administered antibiotic prophylaxis currently given at Rigshospitalet, Copenhagen or (B) a conventional orally administered antibiotic prophylaxis targeting bacteria found in the urine postoperatively.
The investigators' hypothesis is that the targeted prophylactic antibiotic strategy will reduce the number of infection-related readmissions within 90 days of surgery compared to the standardised prophylaxis.

Tetramethylpyrazine Confers Protection Against Oxidative Stress and NLRP3-Dependent Pyroptosis in Rats with Endometriosis

Tetramethylpyrazine (TMP) has been confirmed to suppress inflammation in endometriosis (EMs). Herein, this study investigated whether and how TMP affected NLRP3 inflammasomes and oxidative stress in EMs. After establishment of an EMs rat model, rats were treated with different concentrations of TMP. The size of endometriotic lesions and the latency and frequency of torsion in rats were recorded, followed by the measurement of relevant indicators (TNF-α, IL-6, IL-2, IL-10, MDA, SOD, GSH, CAT, ROS, NLRP3, ASC, GSDMD, caspase-1, Nrf2, and HO-1). The study experimentally determined that TMP treatment markedly decreased the size of endometriotic lesions and improved torsion in rats with EMs. The levels of inflammatory proteins, oxidative stress markers, NLRP3 inflammasome, and pyroptotic proteins were elevated in rats with EMs, all of which were reversed upon TMP treatment. Additionally, the activities of SOD, GSH, and CAT were lowered in rats with EMs, which were partly abrogated by TMP treatment. Furthermore, the downregulation of Nrf2 and HO-1 was counteracted by TMP treatment. To sum up, TMP represses excessive oxidative stress, NLRP3 inflammasome activation, and pyroptosis in rats with EMs. Additionally, TMP may activate the Nrf2/HO-1 pathway.

Prevalence and characteristics of ST131-O16 and ST131-O25b clones among extended-spectrum β-lactamase-producing Escherichia coli isolates causing bloodstream infection in Iran

BACKGROUNDThe multidrug-resistant clone identified as Escherichia coli sequence type 131 (E. coli ST131) has spread worldwide. The current study is one of the first comprehensive investigations to ascertain the prevalence of ST131 and molecularly characterize the ST131-O25b and ST131-O16 subgroups causing bloodstream infections in Iran.METHODS AND RESULTSTo this end, 119 consecutive, non-repetitive E. coli clinical strains were isolated from blood samples of patients with septicemia in different hospital wards for one year in Tehran. The isolates were provided by the laboratories of tertiary hospitals affiliated with Tehran University of Medical Sciences. The disk diffusion method was used to investigate the sensitivity of bacteria to antibiotics. All phylogroup B2 isolates were screened for E. coli ST131 status using a triplex PCR assay that combines the identification of ST131-O25b and -O16 clades. The seven putative virulence factor genes (kpmstII, fimH, afa A, iroN, Sat, ibeA, and ompT) and resistance genes (bla_CTX-M-15, bla_OXA-48, and bla_CMY) were detected by PCR in E. coli ST131 isolates.CONCLUSIONSThe highest incidence of antibiotic resistance among 74/119 (62.18%) extended-spectrum β-lactamases-producing E. coli isolates was observed, respectively, against Nalidixic acid (82%), and Aztreonam (75%), followed by Ciprofloxacin (70%). Twenty out of 74 ESBL-producing E. coli isolates were found to be ST131 (27%), with 13 (65%) ST131-O25b and 7 (35%) ST131-O16 clades, respectively. The ST131-O16 isolates had a higher prevalence of resistance to Ceftriaxone, Amikacin, Aztreonam, and Cefepime than the -O25b ones. Concerning virulence capacity, our findings demonstrated that kpmstII, fimH, and ompT genes were found in 85%, 65%, and 30% of ST131 isolates, respectively. Our results reinforce the surveillance of E. coli ST131 clone dissemination as a major drug-resistant pathogen and an important new public health threat in Iran. Accumulation of multiple virulence factors, ESBL carriage, and identified antimicrobial resistance patterns of ST131-O25b and ST131-O16 clones indicate a necessity to develop strategies to control the spread of these isolates in both community and hospital settings.

Epidemiology of urinary tract infections in adults among United States emergency departments from 2016 to 2023

INTRODUCTIONInfections of the urinary tract, including cystitis and pyelonephritis, are common reasons for presentation to the Emergency Department (ED). However, there are limited recent data on incidence and admission rates. Moreover, with changes in resistance rates, advances in antibiotic options, and increasing antimicrobial stewardship, there is a need to better understand the current management. This study reports the incidence, admission rates, and antibiotic distribution among a nationwide cohort of adult ED patients with cystitis and pyelonephritis over an eight-year period.METHODSThis was a cross-sectional study of adult ED patients with a diagnosis of cystitis or pyelonephritis from 1/1/2016 to 12/31/2023. Using the Epic Cosmos database, ED visits for the above were identified using ICD-10 codes. Outcomes included total ED presentations, admission rates, outpatient antibiotic prescriptions, and antibiotics administered in the ED for admitted patients with cystitis or pyelonephritis. Data were analyzed with binary logistic regression models to measure the relationship between time and dependent variables.RESULTSThere were 205,526,173 total ED encounters, with cystitis occurring in 8,768,481 (4.3 %) and pyelonephritis in 1,044,742 (0.5 %). 23.9 % of those with cystitis were admitted and 33.4 % of those with pyelonephritis were admitted, with a slight rise over time. The most common antibiotics for patients admitted with cystitis and pyelonephritis were third-generation cephalosporins (50.6 % and 55.6 %, respectively). There was a rise in third-generation and fourth-generation cephalosporins, while fluoroquinolone use declined. For discharged patients with cystitis, the most common antibiotics were first-generation cephalosporins (36.4 %), nitrofurantoin (17.9 %), and trimethoprim/sulfamethoxazole (10.7 %). There was a rise in first-generation, second-generation, and third-generation cephalosporins, while nitrofurantoin, trimethoprim/sulfamethoxazole, and ciprofloxacin use declined. For discharged patients with pyelonephritis, the most commonly prescribed antibiotics were first-generation cephalosporins (28.6 %), third-generation cephalosporins (19.5 %), ciprofloxacin (17.0 %), and trimethoprim/sulfamethoxazole (11.5 %). The largest rise was in third-generation cephalosporins, while there was a decline in fluoroquinolones.CONCLUSIONCystitis and pyelonephritis remain common ED presentations with a rise over time. There have been notable shifts in antibiotics, with an overall increase in cephalosporins while fluoroquinolone use has declined. Understanding the current epidemiology can inform public health planning and antibiotic stewardship in the ED.

80 batches of drugs recalled urgently! A major decision by the National Medical Products Administration

Recently, the National Medical Products Administration announced an important decision to stop the production, sale, and use of Hyoscine Butylbromide Tablets, and to revoke its drug registration certificate. According to the announcement issued by the National Medical Products Administration on January 3, titled “Announcement on the Revocation of the Drug Registration Certificate for Hyoscine Butylbromide Tablets (2025 No. 2)”, Hyoscine Butylbromide Tablets, an antibacterial drug also known as Loperamide, will no longer circulate in the market of our country. This decision is effective immediately, and the products already on the market will be recalled by the marketing authorization holder and supervised by provincial drug regulatory authorities for destruction or other harmless disposal measures. A total of 80 batches of Hyoscine Butylbromide Tablets are involved. In addition, in another announcement issued by the National Medical Products Administration on December 2, 2024, it was also announced that 58 drug registration certificates, including 11 batches of Hyoscine Butylbromide Tablets, have been revoked. Based on Article 83 of the Drug Administration Law of the People's Republic of China and other relevant regulations, these drugs have been required to stop production and sale due to unclear efficacy, serious adverse reactions, or other hazards to human health. Hyoscine Butylbromide Tablets are a compound preparation, with the main active ingredients including Hyoscine Tincture, Sulfamethoxazole, and Trimethoprim, primarily used to treat dysentery caused by Shigella and other sensitive pathogenic bacteria causing enteritis. However, with the withdrawal of this drug from the market, other drugs such as Montmorillonite Powder will take on the responsibility for treating diarrhea. According to data from Miiine, the incidence of gastrointestinal diseases is on the rise and is becoming more prevalent among younger populations. In the first three quarters of 2022, the sales of anti-diarrheal drugs and intestinal anti-inflammatory/infection drugs in public hospitals in key provinces and cities in China exceeded 1.2 billion yuan, a year-on-year increase of 6.30%. The sales scale of Montmorillonite Powder in the three major terminal markets in China has exceeded 1 billion yuan, with the outpatient market being the main sales channel, rising by 60%, making it the Top 2 product in anti-diarrheal and intestinal anti-inflammatory/infection drugs. Statistics from Cyborg Blue show that 145 drug registration certificates were revoked in 2024. Chungnam National University Researchers Develop A Rapid Water Quality Monitoring Chip for Antibiotic Detection

DAEJEON, South Korea, Dec. 20, 2024 /PRNewswire/ -- Antimicrobial resistance (AMR) is a growing global health crisis because of microbes, such as bacteria, becoming resistant to antibiotics. A leading factor in this rise is the improper use and disposal of antibiotics in the environment. Effluents from wastewater treatment plants often contain various antibiotics including trimethoprim (TMP), which can harm ecosystems by disrupting microbial communities essential for nutrient cycling. In addition to contributing to AMR, TMP poses various health risks to humans through indirect exposure. Continue Reading Scientists developed a simple TMP detection device for on-site monitoring of contaminated wastewater using selenite-enriched lanthanum hydroxide electrode and polyimide-filter microfluidic channel. Traditional methods for TMP detection such as capillary electrophoresis and liquid chromatography with mass spectrometry, are often labor-intensive and time-consuming. Electrochemical (EC) methods can provide respite from these issues by offering exceptional sensitivity, real-time analytical capabilities and the potential for miniaturization. Professor Tae Yoon Lee and Dr. Natarajan Karikalan of Chungnam National University, Korea, have made a pioneering advancement in EC detection methods, that shows promise to revolutionize on-site testing for TMP in contaminated wastewater. They developed a disposable microfluidic lab-on-a-chip (LOC) EC sensor, μTMP-chip, designed for real-time TMP detection. "Efficient TMP monitoring in effluents is critical for effective control protocols. Hence, we aimed to enable in situ testing of water samples," explains Prof. Lee. Their paper was made available online on September 21, 2024 and was published in Volume 499 of Chemical Engineering Journal on November 1, 2024. The researchers designed the disposable chip by combining a special electrode made with lanthanum hydroxide and selenite, with a polyimide (PI) filter in a microfluidic channel. The analyses showed that the addition of selenite improved the electrode's ability to detect chemicals by allowing better charge flow. Additionally, the PI filter improved the μTMP-chip's real-time performance, while the efficiency dropped by 15 to 45% when the filter was removed. Additionally, the filter helped trap and isolate unwanted materials and prevented the risk of microbial growth, which could interfere with the sensor's function. The μTMP-chip sensor demonstrated impressive results in real-world testing, showing recovery rates of 94.3 % to 97.6 % in soil and water samples. These results, obtained through wireless testing, highlight the chip's potential for practical use in monitoring environmental samples. "Our current design may face challenges in detecting TMP in highly polluted environments with significant matrix interferences. However, we hope our research will inspire further exploration into developing affordable and efficient TMP detection chips," said Prof. Lee. The researchers believe that their innovative lab-on-a-chip design has the potential to improve the feasibility of on-site, real-time tracking of environmental contaminants leading to improved conservation of ecosystems and human health. Nitrofurantoin, Cranberry Products May Cut UTI Episodes in Children

MONDAY, Nov. 4, 2024 -- For children with a history of recurrent urinary tract infection (RUTI), nitrofurantoin and cranberry products may reduce the incidence of symptomatic UTI episodes, according to a review published online Nov. 4 in Pediatrics . Nikolaos Gkiourtzis, M.D., from the Aristotle University of Thessaloniki in Greece, and colleagues conducted a systematic literature review of randomized controlled trials comparing prophylaxis options for the prevention of UTI and kidney scarring in children with a history of RUTI. A total of 3,335 participants from 23 studies were included in the meta-analysis. The researchers found that cranberry products (compared with the control group) and nitrofurantoin (compared with the control, trimethoprim-sulfamethoxazole, or trimethoprim groups) led to lower odds of symptomatic UTI episodes during prophylaxis. Compared with all available documented interventions, nitrofurantoin appeared to be the best option for reduction in the incidence of UTI. None of the prophylaxis options showed a reduction in kidney scarring.
