Endovenous interventions are keyhole operations for varicose veins that are carried out from within the vein itself. Varicose veins are enlarged veins close to the surface of the skin. They are connected to the bigger deeper veins in the leg (known as deep veins). Because of this, operations to close the varicose veins can increase the chance of a blood clot forming in the deep veins. Blood clots in the deep veins happen in around 1 in 50 people after endovenous operations. A clot in the leg can cause swelling, pain, and other long-term problems. If a clot in the leg breaks off and travels to the lungs, it can cause problems with the lung' ability to move oxygen from the air into the blood and may, in rare cases, be life threatening.
Varicose vein procedures may carry a slightly higher risk of blood clot formation, and we are currently unsure if current clot reducing medicines are beneficial in preventing blood clots in people having varicose vein procedures.
This study will investigate if it is worthwhile prescribing medicines to reduce blood clots after varicose vein procedures.

A Regional Audit on Venous Thromboembolism (VTE) Prophylaxis Compliance and Cultural Considerations in Prescribing Porcine-Derived Low-Molecular-Weight Heparins

BACKGROUND:

Hospital-acquired venous thromboembolism (VTE) remains a leading cause of preventable mortality in the NHS. NICE guidelines (NG89) mandate timely risk assessment and prophylaxis, yet practice varies widely. Many low-molecular-weight heparins (LMWHs), such as tinzaparin, are porcine-derived, posing ethical challenges for patients with specific dietary or religious beliefs.

OBJECTIVE:

The objective of this study is to evaluate adherence to NICE NG89 guidelines on VTE prophylaxis across two hospitals, with a particular focus on culturally sensitive prescribing. This audit also assessed clinician awareness of LMWH origins and explored patient preferences around dietary compliance. The impact of targeted interventions was measured across two audit cycles.

METHOD:

A prospective two-cycle audit was conducted at two NHS hospitals in North East England, The James Cook University Hospital (JCUH) and University Hospital North Tees (UHNT), between March 2024 and January 2025. Patients with dietary requirements were identified via catering records and surveyed. A doctor questionnaire assessed awareness and counselling practice. Interventions included staff education, posters, prescribing alerts, and community outreach.

RESULTS:

Cycle one included 76 patients. Cycle two included 61 patients. VTE risk assessment improved by 14% at UHNT; JCUH maintained 100% compliance. However, timely prophylaxis fell in UHNT, with 80% receiving it late. Documentation and discussions regarding VTE and dietary compliance improved but remained suboptimal. Leaflet distribution rose by 10% at JCUH but was absent at UHNT. Notably, 98% (n = 137) of patients prioritised medication aligned with their beliefs, and nearly half would decline porcine-derived prophylaxis even if it prevented serious illness. Clinician questionnaire data showed 64% (n = 50) were unaware that LMWHs are animal-derived, and 94% (n = 50) did not inform patients, citing time pressures and limited knowledge.

CONCLUSION:

While guideline compliance improved following intervention, particularly for risk assessments and documentation, major gaps remain in patient communication and dietary-informed prescribing. Most patients expected transparency and alternatives to porcine-based medications. Future efforts must embed dietary screening into routine prescribing and strengthen shared decision-making through structured education and culturally competent communication strategies.

The Ottawa score for prediction of recurrent venous thromboembolism in cancer patients treated with tinzaparin: an individual patient data meta-analysis

Background:

Risk of venous thromboembolism (VTE) recurrence remains high in patients with cancer-associated thrombosis (CAT), despite therapeutic anticoagulation. Identifying patients at risk of treatment failure is still a challenge.

Objectives:

We aimed to assess the performance of the Ottawa score in predicting VTE recurrence in a large homogeneous population of patients with CAT treated with the same anticoagulant, tinzaparin, for at least 3 months.

Methods:

Individual patient data from 3 prospective cohort studies and 1 randomized controlled trial were pooled (PROSPERO: CRD42019119907). Clinical events of interest were adjudicated by independent central adjudication committees in all 4 studies.

Results:

Among the 1413 patients included, the Ottawa score could be calculated for 1088 of whom 646 (59.4%) were classified at high risk of recurrence (Ottawa score ≥ 1). The 6-month cumulative incidence of recurrent VTE was 5.0% (95% CI, 3.2-7.8) in the Ottawa low-risk group and 8.5% (95% CI, 6.6-10.8) in the high-risk group. The area under the receiver operating characteristic curve was 0.56 (95% CI, 0.51-0.62). The sensitivity of the dichotomized Ottawa score (score ≥ 1) was 72.8% (95% CI, 62.6%-83.0%), the specificity was 41.9% (95% CI, 37.8%-45.9%), the positive predictive value was 8.6% (95% CI, 6.4%-10.8%), and the negative predictive value was 95.3% (95% CI, 93.3%-97.4%). Introducing additional predictive factors failed to significantly improve the score's performance.

Conclusions:

Despite the large number of patients and anticoagulant treatment standardization, the Ottawa score failed to accurately predict recurrent VTE in patients with CAT treated with tinzaparin.

Comparative safety of enoxaparin versus other low-molecular-weight heparins in cancer-associated venous thromboembolism: a real-world cohort study from RIETE

Background:

Low-molecular-weight heparins (LMWHs) are widely used in the treatment of cancer-associated venous thromboembolism (VTE), yet their long-term safety profiles remain insufficiently compared in clinical practice.

Objectives:

The primary outcome was major bleeding over a 6-month follow-up. Secondary outcomes included VTE recurrence, non-major clinically relevant bleeding, and all-cause mortality.

Methods:

We analyzed 7287 patients with active cancer and acute VTE from the RIETE registry (2009-2022) who were treated with full-dose enoxaparin (n = 5628) or tinzaparin/dalteparin (n = 1659). Analyses were adjusted using multivariable Cox models, Fine-Gray competing risk models, frailty models clustered by center, and propensity score approaches.

Results:

Major bleeding occurred in 3.84% of patients receiving enoxaparin versus 2.53% in the tinzaparin/dalteparin group (adjusted hazard ratio [aHR] 1.56; 95% CI: 1.11-2.19), with consistent findings across all sensitivity analyses. Enoxaparin was also associated with higher all-cause mortality (28.3% vs 25.1%; aHR 1.22; 95% CI: 1.09-1.37). No significant differences were observed in VTE recurrence (3.59% vs 3.07%) or non-major bleeding (3.98% vs 3.25%). Importantly, during the first 10 days of therapy, major bleeding occurred in 1.2% of patients treated with enoxaparin twice-daily, compared to 0.4% with once-daily dosing and 0.1% in the tinzaparin/dalteparin group (P < .001).

Conclusion:

In this large, observational study, enoxaparin, particularly in twice-daily regimens, was associated with significantly increased risks of bleeding and mortality compared to tinzaparin/dalteparin. These findings may help refine LMWH selection and dosing strategies in patients with cancer-associated VTE and warrant further investigation in prospective studies.

Leo Pharma's slow response to app lapse scolded by UK marketing watchdog

Leo Pharma accepted that “appropriate actions were not taken to rectify the issue immediately” with its Innohep app. Leo Pharma’s inaccurate Innohep app has prompted a slap-down from the PMCPA. The U.K. marketing watchdog ruled Leo brought discredit on the pharma industry by leaving the app live for three months after the errors were discovered. Two healthcare professionals complained to the PMCPA about an app for patients prescribed Innohep, Leo’s tinzaparin sodium product. The app presented information from the patient information leaflet, and users were prompted to select their prescribed dosage and shown information on injection technique.Both complainants flagged inaccuracies in the dosing information presented in the app. A Leo employee identified the errors in August 2024, and Leo’s medical affairs team learned of the inaccuracies the next day, according to the PMCPA report, yet the app was still available when the PMCPA contacted Leo in November. Leo deactivated the app and made an updated version available within two weeks of being contacted by the PMCPA.Responding to the marketing authority, Leo said two senior medical affairs employees concluded that the full volume of the syringe would always be administered, despite any conflicting information in the app, and, as such, the inaccuracies didn’t affect patient safety. The PMCPA panel took a dim view of Leo’s conclusion, stating that inaccurate dosing information is a serious matter regardless of the likelihood of a safety issue. The panel said it “was extremely concerned about Leo Pharma’s lack of urgency” and the company’s “conscious decision to keep the app live with the inaccuracies for approximately three months.” The dosage volume inaccuracies and failure to withdraw the app immediately could “reduce confidence in the industry being able to produce patient material to the required quality standards,” the panel said.Leo accepted that “appropriate actions were not taken to rectify the issue immediately.” The company listed mitigating factors that may have played a role in the failings, including a disruption that affected the company from July, when the app was certified, to October, when Leo was still working through its response to the inaccuracies. “At the time of this issue occurring there was a reorganization of the thrombosis business impacting resource and headcount; many colleagues’ roles were placed at risk of redundancy. There was no dedicated medical support for the U.K. thrombosis business at this time,” Leo said. “Whilst this does not negate the seriousness of the issue and decisions made, we believe this to be a contributing factor.”The PMCPA appeal board opted against imposing additional sanctions on Leo. The board reached the decision after assessing Leo’s work to enhance its governance, train staff and update its processes to add checks and scrutiny. Leo said “it was not experienced with creating and managing apps and had relied on external expertise,” according to the PMCPA, and would act differently if in the same situation today.
