Abstract:

Objective:

This study aimed to determine long-term cardiometabolic effects of hormone therapies initiated within 3 years of onset of menopause after a 14-year follow-up study of participants of the Kronos Early Estrogen Prevention Study (KEEPS).

Methods:

KEEPS was a multisite clinical trial that recruited recently menopausal women with good cardiovascular health for randomization to oral conjugated equine estrogens (Premarin, 0.45 mg/d) or transdermal 17β-estradiol (Climara, 50 μg/d) both with micronized progesterone (Prometrium, 200 mg/d) for 12 d/mo, or placebo pills and patch for 4 years. KEEPS continuation recontacted KEEPS participants 14 years after randomization and 10 years after the completion of the 4-year clinical trial to attend in-person clinic visits.

Results:

Participants of KEEPS continuation (n = 299 of the 727 KEEPS participants; 41%) had an average age of 67 years (range, 58-73 y). Measurements of systolic and diastolic blood pressures, waist-to-hip ratio, fasting levels of glucose, insulin, lipid profiles, and homeostasis model assessment of insulin resistance were not different among the treatment groups at either KEEPS baseline or at KEEPS continuation visits, or for change between these two visits. The frequency of self-reported diabetes (P = 0.007) and use of diabetes medications was higher in the placebo than the oral conjugated equine estrogens (P = 0.045) or transdermal 17β-estradiol (P = 0.02) groups, but these differences were not supported by the laboratory measurements of glycemia or insulin resistance.

Conclusions:

There was no evidence of cardiovascular and/or metabolic benefits or adverse effects associated with 4 years use of oral or transdermal forms of hormone therapy by recently menopausal women with good cardiovascular health after 10 years.

Abstract:

Objective:

This study aimed to evaluate the short-term efficacy and safety of an oral herbal supplement containing glucosinolates, phytosterols, and citrus flavonoids for menopausal symptoms in comparison with estrogen plus progestogen therapy (EPT) among postmenopausal women.

Methods:

This was a pilot single-blinded, three-armed phase II randomized clinical trial, controlled with EPT. Sixty participants were randomly assigned to receive treatment for 3 months: (1) an oral herbal supplement of 1,500 mg/d (G1, n = 20), (2) an oral herbal supplement of 3,000 mg/d (G2, n = 20), or (3) conjugated equine estrogens 0.625 mg/d plus medroxyprogesterone acetate of 5 mg/d (EPT group, n = 20). The primary endpoint was the intensity of menopausal symptoms as measured using the Menopause-Specific Quality of Life Questionnaire (global and domain scores). The Menopause-Specific Quality of Life Questionnaire uses a 7-point scale to rate the symptom intensity, with higher scores indicating severity. The secondary endpoints were hormonal, lipid, and safety profiles.

Results:

Fifty-four participants (n = 54) completed the study. The mean, model-estimated, and global menopausal symptom scores at 3 months were 85.8 in the EPT group, 61.3 in G1, and 62.5 in G2. Participants treated with the herbal compound had lower global (13.7 [6.9-20.4],P< 0.001) and physical symptom scores (6.6 [1.6-11.5],P= 0.002) on the second month and lower psychosocial symptom scores (3.8 [1.3 to 6.3],P< 0.001) on the third month of follow-up, compared with EPT. Conversely, participants receiving EPT showed better outcomes on vasomotor symptoms since the first month of treatment (−6.1 [−8.3 to −4.0],P< 0.001). The EPT group exhibited higher values of estradiol and lower follicle-stimulating hormone and luteinizing hormone since the first month of follow-up. Also, total cholesterol, low-density lipoprotein cholesterol, and high-density lipoprotein cholesterol were significantly higher in this group than in G2.

Conclusions:

In this small single-blind exploratory trial, the oral herbal supplement was more efficacious in reducing global, physical, and psychosocial menopausal symptoms in the short term than EPT. However, further studies are needed to adequately assess the efficacy and safety of this herbal supplement in the treatment of menopausal symptoms.

Local estrogen therapy (LET) is the mainstay of treatment for vaginal dryness, dyspareunia and other urogenital symptoms because it may reverse some pathophysiological mechanisms associated with decreasing endocrine function and increasing aging. Over the years, several vaginal products including different formulations (tablets, rings, capsules, pessaries, creams, gels and ovules) and molecules (estradiol [E2], estriol [E3], promestriene, conjugated equine estrogens and estrone) have been used with superimposable therapeutic results. Low-dose and ultra-low-dose LET is the gold standard due to its minimal systemic absorption, with circulating E2 levels persistently remaining in the postmenopausal range. In healthy postmenopausal women, preference among the various products is presently the main driver and dissatisfaction with LET seems high, namely because of the delayed use in those with severe symptoms of genitourinary syndrome of menopause (GSM). Specific concerns remain in high-risk populations such as breast cancer survivors (BCS), especially those under treatment with aromatase inhibitors. Based on the multitude of symptoms under the umbrella of GSM definition, which includes vulvovaginal atrophy (VVA), it is mandatory to investigate specific effects of LET on quality of life, sexual function and genitourinary conditions by conducting studies with a patient-tailored focus.

American Society of Clinical Oncology, May 30-June 4

The annual meeting of the American Society of Clinical Oncology was held from May 30 to June 4 in Chicago and hosted more than 34,000 participants from around the world, including clinicians, academicians, allied health professionals, and others interested in oncology. The conference featured the latest advances in clinical cancer research, including oral abstract presentations and poster presentations in disease-based and specialty tracks. Presentations focused on novel targeted therapies as well as improvements in chemotherapy and radiation therapy approaches. In one study, Jefferson DeKloe, M.D., of Thomas Jefferson University in Philadelphia, and colleagues found that human papillomavirus (HPV) vaccination reduces the risk for cervical cancer and precancerous dysplasia in female patients and may reduce the incidence of other HPV-associated cancers, including head and neck cancers in men. The authors performed a retrospective cohort study using the data from the TriNetX U.S. Collaborative Network. Patients aged 9 to 39 years who received any vaccine between Jan. 1, 2010, and Dec. 31, 2023, were included in the analysis. Patients were divided into two cohorts: those vaccinated for HPV at least five years prior and those with no history of HPV vaccination. The development of malignancies across various organ systems, including head and neck, cervix, anus and anal canal, penis, vulva, and vagina, was assessed. The researchers observed lower rates of cervical cancer in female participants vaccinated for HPV compared with those not vaccinated. Furthermore, lower rates of head and neck cancer were observed in male patients vaccinated for HPV compared with those not vaccinated. "There are still many variables that influence the risk of developing an HPV-associated cancer and future research may inform us what is causing the reduced incidence of head and neck cancers in HPV-vaccinated males," DeKloe said. "This work shows that the HPV-vaccination efforts in the United States may already have some apparent benefit measured through the reduction of HPV-associated cancers. Given the rise in HPV-related cancers in the past few decades, HPV vaccination is an important means of cancer prevention that should be emphasized for male and female patients in clinical practice." One author disclosed a financial relationship with AstraZeneca. Abstract No. 10507 In a large-scale comparative effectiveness trial across 22 cancer centers, Joseph Greer, M.D., of Massachusetts General Hospital and Harvard Medical School in Boston, and colleagues demonstrated equivalence for the effect of delivering early palliative care via video versus in-person visits on quality of life in patients with advanced lung cancer. The authors focused on the use of video visits to deliver palliative care, as according to the authors, they require fewer clinical resources and may be a more efficient way for clinicians to provide care. The researchers also note that video visits save patients time and money compared with going to the hospital or clinic to meet with the clinician. The trial included 1,250 patients, with 633 in the video-visit palliative care group and 617 in the in-person palliative care group. In addition, 548 caregivers (i.e., a family member or friend who was involved in the patient's care) were enrolled in the study. All patients who participated in the study received early palliative care that began soon after their diagnosis and continued throughout the course of their disease. Patients in both study groups had contact with the palliative care clinician every four weeks, with the visits occurring predominantly in clinic for those randomly assigned to in-person palliative care and via video visits for those assigned to telehealth palliative care. The researchers found that medical care, such as palliative care, can be delivered through video visits and patients and their family caregivers receive the same quality-of-life benefits as with in-person palliative care. The two study groups also did not differ in their reported satisfaction with care or with respect to patient- and caregiver-reported mood symptoms. "The equivalence of these modalities in a population with serious illness underscores the urgent need for clinicians, health care systems, and policymakers to expand equitable access to evidence-based palliative care and develop guidelines for a new standard of care that includes the broad adoption of telehealth services," Greer said. Several authors disclosed ties to the biopharmaceutical industry. Abstract No. LBA3 In a prospective study, Kimia Sorouri, M.D., of the Dana-Farber Cancer Institute in Boston, and colleagues followed breast cancer survivors over 10 years to determine fertility outcomes in those attempting pregnancy, with the majority achieving pregnancy and most experiencing live births. The authors evaluated women with stage 0 to III breast cancer included in the Young Women's Breast Cancer Study who reported attempting pregnancy postdiagnosis (1,213 eligible female participants, including 197 reporting any attempt of pregnancy). The researchers found that 73 percent reported at least one pregnancy after diagnosis and 65 percent experienced at least one live birth after diagnosis. "This is the first prospective study with greater than 10 years of follow-up to report fertility outcomes in young breast cancer survivors accounting for attempting pregnancy," the authors write. "Our findings can be incorporated into the counseling of young breast cancer patients and survivors, and highlight the need for accessibility of fertility preservation services for this population." Women's mental agility is better when they're on their period, study finds

Participants reacted quicker and made fewer errors during menstruation, despite believing their performance would be worse, according to new research. Participants reacted quicker and made fewer errors during menstruation, despite believing their performance would be worse, according to new research from UCL and the Institute of Sport, Exercise & Health (ISEH). The study, published in Neuropsychologia, is the first to assess sport-related cognition during the menstrual cycle and is part of a larger research project supported by the FIFA Research Scholarship. The findings act as a proof-of-principle that specific types of cognition fluctuate throughout the menstrual cycle, which could have implications for injury and other aspects of women's health. Previous sports medicine research has shown that women seem to be at greater risk of sport-related injury during the luteal phase, which is the time between ovulation and menstruation. This is possibly related to the significant hormonal changes that occur throughout the menstrual cycle. But precisely how these changes are linked to an increased likelihood of injury are unknown at present. In this study, researchers at UCL and ISEH collected reaction time and error data from 241 participants who completed a battery of cognitive tests 14 days apart. Participants also completed a mood scale and a symptom questionnaire twice. Period-tracking apps were used to estimate which phase of their cycle the participants were in when they took the tests. The tests were designed to mimic mental processes that are typical in team sports. In one test, participants were shown smiling or winking faces and asked to press the space bar only when they saw a smiley face, to test inhibition, attention, reaction time and accuracy. In another, they were asked to identify mirror images in a 3D rotation task, which assesses spatial cognition. A task that asked them to click when two moving balls collide on screen measured spatial timing. Though participants reported feeling worse during menstruation and perceived that this negatively impacted their performance, their reaction times were faster and they made fewer errors. For example, their timing was on average 10 milliseconds (12%) more accurate in the moving balls task, and they pressed the space bar at the wrong time 25% less in the inhibition task. Participants' reaction times were slower during the luteal phase, which begins after ovulation and lasts between 12-14 days up to the beginning of menstruation. They were on average 10-20 milliseconds slower compared to being in any other phase. They didn't make more errors in this phase, however. Dr Flaminia Ronca, first author of the study from UCL Division of Surgery and Interventional Science and ISEH, said: "Research suggests that female athletes are more likely to sustain certain types of sports injuries during the luteal phase and the assumption has been that this is due to biomechanical changes as a result of hormonal variation. But I wasn't convinced that physical changes alone could explain this association. "Given that progesterone has an inhibitory effect on the cerebral cortex and oestrogen stimulates it, making us react slower or faster, we wondered if injuries could be a result of a change in athletes' timing of movements throughout the cycle. "What is surprising is that the participant's performance was better when they were on their period, which challenges what women, and perhaps society more generally, assume about their abilities at this particular time of the month. "I hope that this will provide the basis for positive conversations between coaches and athletes about perceptions and performance: how we feel doesn't always reflect how we perform." To put the findings in context, the authors say the fluctuation in timing could be the difference between an injury or not. Previous research has shown that a variation of just 10 milliseconds can mean the difference between a concussion and a lesser injury, for example. In the colliding balls task, participants' timing was on average 12 milliseconds slower during the luteal phase compared to every other phase, a difference of 16%. Dr Megan Lowery, an author of the study from UCL Surgery & Interventional Science and ISEH, said: "There's lots of anecdotal evidence from women that they might feel clumsy just before ovulation, for example, which is supported by our findings here. My hope is that if women understand how their brains and bodies change during the month, it will help them to adapt. "Though there's a lot more research needed in this area, these findings are an important first step towards understanding how women's cognition affects their athletic performance at different points during their cycle, which will hopefully facilitate positive conversations between coaches and athletes around performance and wellbeing." Professor Paul Burgess, senior author of the study from UCL's Institute of Cognitive Neuroscience, said: "This study emerged from listening carefully to female soccer players and their coaches. We created bespoke cognitive tests to try to mimic the demands made upon the brain at the points in the game where they were telling us that injuries and problems of timing occur at certain times of the menstrual cycle. "As suggested by what the soccer players had told us, the data suggested that women who menstruate -- whether they are athletes or not -- do tend to vary in their performance at certain stages of the cycle. As a neuroscientist, I am amazed that we don't already know more about this, and hope that our study will help motivate increasing interest in this vital aspect of sports medicine."

Certain menopausal hormone therapy could raise ovarian cancer risk, study finds

An older form of hormone therapy taken to ease the symptoms of menopause could increase the risk of ovarian cancer, according to results released Thursday from two studies involving tens of thousands of women. Another hormonal regimen didnt seem to carry this same risk, researchers found.The results should help settle debate over the relative risk of cancer associated with estrogen- and progestin-based hormonal therapy. Still, researchers noted, the absolute risk of ovarian cancer associated with either regimen is low.Before these randomized, placebo-controlled clinical trials, observational studies examining the influence of estrogen plus progestin on ovarian and endometrial cancers gave mixed results, said Rowan Chlebowski, of the Lundquist Institute in California, in a statement on the findings. Our study provides the only long-term information from a randomized clinical trial on two common cancers in postmenopausal women for two of the most commonly used medications.Part of the Womens Health Initiative that began in 1991, the studies ran between 1993 and 1998 and enrolled more than 27,000 women aged 50 to 79 years from across the U.S. The findings, which stem from a 20-year follow-up analysis, will be presented at the American Society of Clinical Oncologys annual meeting, being held in Chicago from May 31 to June 4.For postmenopausal women, hormone therapy has long been used to help manage hot flashes and sleep disturbances. During the time of the studies, the standard option for women who had had a hysterectomy, or their uteruses removed, was conjugated equine estrogen, or CEE. Women who still had a uterus often received CEE plus medroxyprogesterone acetate, or MPA.The WHI studies randomized women without a uterus to either CEE or a placebo, and those who had a uterus to receive CEE plus MPA or a placebo.Results show that CEE was associated with a higher risk of developing ovarian cancer compared to placebo, with 35 cases in the hormone therapy group versus 17 in the placebo group. The risk of dying from ovarian cancer was almost three times higher with among CEE-treated women than in those given placebo.But among women with uteruses who took CEE plus MPA, there was no statistically significant increase in the risk of developing ovarian cancer, or in dying from it. Notably, women on this regimen had a 28% lower risk of developing uterine cancer, researchers found.While this new information is an important part of patient counseling and education, given the low numbers, it should not necessarily impact a woman's decision to take menopausal hormone therapy for symptomatic relief of menopausal symptoms, said Eleonora Teplinsky, a physician at Valley Mount Sinai Comprehensive Cancer Care, in a statement provided by ASCO.CEE was for years a common menopausal treatment. But its use declined following prior WHI findings on associated heart risks that led to Food and Drug Administration warnings.Hormonal therapy is not currently recommended for all postmenopausal women. Groups like the American Cancer Society have previously flagged ovarian cancer risk, while the FDA has highlighted the risk of endometrial cancer. The regulator advises women who do take hormonal therapy for menopausal symptoms to use the lowest dose for as short a time as possible.However, WHI researchers noted that ovarian cancer is not currently incorporated as a risk in prescribing guidelines for menopausal hormone therapy. They also suggested that their findings prompt reconsideration of advice around estrogen-only use in ovarian cancer survivors.Some drugmakers, meanwhile, are developing non-hormonal medications that could become an alternative to hormone therapy for treating vasomotor symptoms.
