A Phase 2, Randomized, Double-blind, Placebo-controlled Trial of Fosamprenavir-Sodium Alginate Administered Orally for 8 Weeks to Patients With Proton Pump Inhibitor Refractory Gastroesophageal Reflux Disease

The goal of this clinical trial is to learn if study drug Fosamprenavir-Sodium Alginate (FOS-SA) administered orally improves symptoms for Proton Pump Inhibitor (PPI)-refractory Gastro Esophageal Reflux Disease (GERD).
The main questions it aims to answer are:
1. Does FOS-SA significantly improve heartburn severity over the 8-week treatment period
2. Does FOS-SA significantly improve regurgitation frequency over the 8-week treatment period
3. Does FOS-SA significantly improve symptoms of persistent GERD over the 8-week treatment period
Researchers will compare FOS-SA to a placebo (a look-alike substance that contains no active drug) to see if FOS-SA works to treat PPI-refractory GERD.
Participants will:
1. Take FOS-SA or placebo every day BID (twice a day) for 8 weeks
2. Visit the Adult Translational Research Unit (ATRU) seven times for consenting, screening, and checkups and tests
3. Keep a daily diary of their symptoms of persistent GERD

A 12-Week Randomized, Double-Blind, Placebo-Controlled Clinical Trial to Assess the Efficacy of Oral Fosamprenavir-Sodium Alginate Suspension for Laryngopharyngeal Reflux

Laryngopharyngeal reflux (LPR) causes chronic cough, throat clearing, hoarseness, and dysphagia and if left untreated can promote the development of laryngeal cancer. More than 20% of the United Stated population suffer from LPR, yet there is no effective medical therapy. Proton pump inhibitors (PPIs), which inhibit gastric acid production but do not prevent reflux events, continue to be prescribed for LPR despite their poor efficacy for this patient population, high cost ($26 billion/year), and associated risks. Pepsin, detected in the airway of these patients and now known to cause laryngeal inflammation and promote disease independent of gastric acid, is a key therapeutic target. We report preclinical studies of select HIV inhibitors that bind to and inhibit pepsin and thus hold promise for the treatment of LPR. In support, a very low incidence of LPR was found in patients taking these drugs compared to the general population. HIV inhibitors are ideal drugs to repurpose because they target a foreign virus. Thus, a repurposing approach can be used to safely perform proof of concept testing of the efficacy of a pepsin inhibitor for LPR. The Specific Aim of this project is to perform a 12-week randomized, double-blind, placebo-controlled clinical trial to assess the efficacy of fosamprenavir-sodium alginate(FOS-SA) for LPR. FOS-SA will be used at the FDA approved, manufacturers recommended dose for HIV for 12 weeks in medically refractory patients with clinically diagnosed moderate/severe LPR and combined multi-channel intraluminal impedance - pH (MII-pH) confirmed laryngeal reflux events. Routine clinical outcome measures for LPR (Reflux Symptom Index, Reflux Finding Score, Voice Handicap Index) will be documented pre- and post-treatment with Oral Fosamprenavir-Sodium Alginate for LPR(n = 52) and placebo (n = 52). Additional research measures will include repeat administration of a newly created Daily Symptom Reflex Diary, as well as an intermittently distributed Patient Global Impression - Static & Change scales. Saliva will be collected pre-treatment for both pepsin protein analysis and kinetic activity assay to compare with clinical measures. There is currently no effective medical therapy for LPR and pepsin is the key therapeutic target. Identification of an FDA approved drug which inhibits pepsin allows for a clinical trial to determine efficacy using a faster and safer repurposing approach to address a significant gap.

Molecular ecology of novel amdoparvoviruses and old protoparvoviruses in Spanish wild carnivorans

Wild carnivorans are key hosts of parvoviruses of relevance for animal health and wildlife conservation. However, the distribution and diversity of parvoviruses among wild carnivorans are under-investigated, particularly in Southern Europe. We evaluated the presence, spread, and diversity of multi-host protoparvoviruses (canine parvovirus type 2 (CPV-2), feline panleukopenia virus (FPV)), and amdoparvoviruses in 12 carnivoran species from Northern Spain to explore viral ecology. Broad-range PCRs were used to screen spleens (N = 157) and intestines (N = 116) from 171 road-killed mustelids, viverrids, and felids; identified viruses were molecularly characterized. We detected an Asian-like CPV-2c strain in the spleen of one wildcat (Felis silvestris, 1/40, 2.5 %), a globally distributed FPV strain in the spleen of one Eurasian badger (Meles meles, 1/35, 2.9 %), a novel amdoparvovirus (European mustelid amdoparvovirus 1), in the intestine and spleen of one stone marten (Martes foina, 1/16, 6.3 %) and in the spleen of one Eurasian badger (1/35, 2.9 %), the red fox fecal amdovirus (RFFAV) in the intestine and spleen of three wildcats (3/40, 7.5 %), and a novel amdoparvovirus closely related to RFFAV (European felid amdoparvovirus 1) in one wildcat (1/40, 2.5 %). We observed a correlation between the phylogeny of carnivorans and the one of amdoparvoviruses, possibly indicating virus-host co-evolution. Species originating from North America and Eurasia formed different clades, indicating local segregation in the absence of man-linked transboundary movements. In contrast, CPV-2 and FPV strains were internationally dispersed. Different parvovirus species co-occur in sympatric host populations, and higher viral diversity and additional hosts will likely be identified in future studies.

Diffuse Ferroelectric Phase Transition-Dependent Photovoltaic Effect in BiFeO₃–BaTiO₃-Based Solid Solutions

The photovoltaic effect in ferroelectric (FE) semiconductors, i.e., the FPV effect, is attracting increasing attention. However, the microscopic mechanism of the FPV effect is still largely unknown. Here, we investigated the effects of crystal structure, dielectric, FE, and domain features on the FPV effect in the 0.7BiFe_1-xCr_xO₃-0.3BaTi_1-xMn_xO₃ (BFC-BTM) (0.01 ≤ x ≤ 0.07) system. We observed a coexisting rhombohedral and tetragonal to pseudocubic phase transition for x ≥ 0.05. This structural transition was accompanied by a significant reduction in the lattice distortion and disappearance of macro-sized FE domains. We found a substantial increase and decrease in the degree of the diffuseness of the relaxor FE phase transition and the open-circuit photovoltage V_OC, respectively, for x ≥ 0.05, in contrast to the relatively weak variation in the remnant polarization. Our results demonstrate that the FPV effect depends weakly on FE polarization but strongly on the complex submicron domain-related diffused relaxor FE phase transition and crystal structure in BFC-BTM. These results were discussed based on the non-centrosymmetry-related shift current mechanism.

Neighborhood-level fatal police violence and severe maternal morbidity in California

Abstract:

Police violence is a pervasive issue that may have adverse implications for severe maternal morbidity (SMM). We assessed how the occurrence of fatal police violence (FPV) in one’s neighborhood before or during pregnancy may influence SMM risk. Hospital discharge records from California between 2002 and 2018 were linked with the Fatal Encounters database (n = 2 608 682). We identified 2184 neighborhoods (census tracts) with at least 1 FPV incident during the study period and used neighborhood fixed-effects models adjusting for individual sociodemographic characteristics to estimate odds of SMM associated with experiencing FPV in one’s neighborhood anytime within the 24 months before childbirth. We did not find conclusive evidence on the link between FPV occurrence before delivery and SMM. However, estimates show that birthing people residing in neighborhoods where 1 or more FPV events had occurred within the preceding 24 months of giving birth may have mildly elevated odds of SMM than those residing in the same neighborhoods with no FPV occurrence during the 24 months preceding childbirth (odds ratio [OR] = 1.02; 95% confidence interval [CI], 0.99-1.05), particularly among those living in neighborhoods with fewer FPV incidents (1-2) throughout the study period (OR = 1.03; 95% CI, 1.00-1.06). Our findings provide evidence for the need to continue to examine the population health consequences of police violence.
