Laryngopharyngeal reflux (LPR) causes chronic cough, throat clearing, hoarseness, and dysphagia and if left untreated can promote the development of laryngeal cancer. More than 20% of the United Stated population suffer from LPR, yet there is no effective medical therapy. Proton pump inhibitors (PPIs), which inhibit gastric acid production but do not prevent reflux events, continue to be prescribed for LPR despite their poor efficacy for this patient population, high cost ($26 billion/year), and associated risks. Pepsin, detected in the airway of these patients and now known to cause laryngeal inflammation and promote disease independent of gastric acid, is a key therapeutic target. We report preclinical studies of select HIV inhibitors that bind to and inhibit pepsin and thus hold promise for the treatment of LPR. In support, a very low incidence of LPR was found in patients taking these drugs compared to the general population. HIV inhibitors are ideal drugs to repurpose because they target a foreign virus. Thus, a repurposing approach can be used to safely perform proof of concept testing of the efficacy of a pepsin inhibitor for LPR. The Specific Aim of this project is to perform a 12-week randomized, double-blind, placebo-controlled clinical trial to assess the efficacy of fosamprenavirvpills for LPR. Fosamprenavir will be used at the FDA approved, manufacturers recommended dose for HIV for 12 weeks in medically refractory patients with clinically diagnosed moderate/severe LPR and combined multi-channel intraluminal impedance - pH (MII-pH) confirmed laryngeal reflux events. Routine clinical outcome measures for LPR (Reflux Symptom Index, Reflux Finding Score, Voice Handicap Index) will be documented pre- and post-treatment with Oral Fosamprenavir for LPR(n = 52) and placebo (n = 52). Additional research measures will include repeat administration of a newly created Daily Symptom Reflex Diary, as well as an intermittently distributed Patient Global Impression - Static & Change scales. There is currently no effective medical therapy for LPR and pepsin is the key therapeutic target. Identification of an FDA approved drug which inhibits pepsin allows for a clinical trial to determine efficacy using a faster and safer repurposing approach to address a significant gap.

A Phase 2, Randomized, Double-blind, Placebo-controlled Trial of Fosamprenavir-Sodium Alginate Administered Orally for 8 Weeks to Patients With Proton Pump Inhibitor Refractory Gastroesophageal Reflux Disease

The goal of this clinical trial is to learn if study drug Fosamprenavir-Sodium Alginate (FOS-SA) administered orally improves symptoms for Proton Pump Inhibitor (PPI)-refractory Gastro Esophageal Reflux Disease (GERD).
The main questions it aims to answer are:
1. Does FOS-SA significantly improve heartburn severity over the 8-week treatment period
2. Does FOS-SA significantly improve regurgitation frequency over the 8-week treatment period
3. Does FOS-SA significantly improve symptoms of persistent GERD over the 8-week treatment period
Researchers will compare FOS-SA to a placebo (a look-alike substance that contains no active drug) to see if FOS-SA works to treat PPI-refractory GERD.
Participants will:
1. Take FOS-SA or placebo every day BID (twice a day) for 8 weeks
2. Visit the Adult Translational Research Unit (ATRU) seven times for consenting, screening, and checkups and tests
3. Keep a daily diary of their symptoms of persistent GERD

Development and utilization of a multiplex PCR assay for detecting three feline enteroviruses

BACKGROUND:

Feline diarrhea is a common digestive tract disease in clinical practice, with watery feces as the main clinical manifestation. There are numerous pathogenic factors causing feline diarrhea, among which viral infections are prevalent, and feline panleukopenia virus (FPV) is the most common pathogen. In recent years, a variety of novel viruses have been detected in the intestines of cats with diarrhea. For example, feline kobuvirus (FKoV) and feline norovirus (FNoV) have been identified. These viruses may have a direct relationship with feline diarrhea or the connection has yet to be discovered. However, with the continuous emergence of these novel viruses and the frequent contact between pet cats and humans, it is prone to large-scale epidemics and outbreaks of viruses. Therefore, developing an accurate, rapid, and simple method to detect novel enteric viruses is of great significance for the early warning of emerging feline enteric viral infectious diseases.

METHODS AND RESULTS:

A detailed comparison of the genome sequences of the three aforementioned feline enteroviruses was conducted. Subsequently, three pairs of specific primers were designed by selecting the conserved gene regions, and the single and multiplex PCR amplification reaction systems as well as reaction conditions were repeatedly optimized. The target fragment sizes detected by the multiplex PCR method were 650 bp for FPV, 500 bp for FKoV, and 340 bp for FNoV. Sensitivity tests demonstrated that the lower detection limit was one-tenth of that of single PCR. Meanwhile, the detection results for feline calicivirus (FCV), feline herpesvirus (FHV), and feline coronavirus (FCoV) were all negative. Testing of a total of 209 clinical samples from various regions in Shandong Province revealed that the detection rates of the three viruses were 13.4%, 4.8%, and 3.8%, respectively, and mixed infections were present.

CONCLUSIONS:

In this study, an epidemiological investigation of the three feline enteroviruses was performed, and a sensitive, specific, and reproducible multiplex PCR assay was developed, which can be utilized for the detection of clinical samples.

Machine learning‐based multi‐pool Voigt fitting of CEST, rNOE, and MTC in Z‐spectra

Abstract:

Purpose:

Four‐pool Voigt (FPV) machine learning (ML)–based fitting for Z‐spectra was developed to reduce fitting times for clinical feasibility in terms of on‐scanner analysis and to promote larger cohort studies. The approach was compared to four‐pool Lorentzian (FPL)‐ML–based modeling to empirically verify the advantage of Voigt models for Z‐spectra.

Methods:

Voigt and Lorentzian models were fitted to human 3 T Z‐spectral data using least squares (LS) to generate training data for the corresponding ML versions. Gradient boosting decision trees were trained, resulting in one Voigt and one Lorentzian ML model. Modeling accuracy was tested, and the fitting times of the ML models and LS versions were evaluated. The goodness of fits of Voigt and Lorentzian ML models were compared.

Results:

The training time for each ML model (Voigt and Lorentzian) was less than 1 min, and the modeling accuracy compared to the corresponding LS versions was excellent, as indicated by a nonsignificant difference between the parameters obtained by LS and corresponding ML versions. The average fitting time was 20 μs/spectrum for both ML models compared to 0.27 and 0.82 s/spectrum for LS with FPL and FPV, respectively. The goodness of fits of FPV‐ML and FPL‐ML differed significantly (p < 0.005), with FPV‐ML showing an improvement for all tested data.

Conclusion:

Gradient boosting decision trees fitting of multi‐pool Z‐spectra significantly reduces fitting times compared to traditional LS approaches, allowing fast data processing while upholding fitting quality. Along with the short training times, this makes the method suitable for clinical settings and for large cohort research applications. The FPV‐ML approach provides a significant improvement of goodness of fit compared to FPL‐ML.

The complexity of the interpretation of ELISA and RT-PCR results in the diagnosis of a reticuloendotheliosis virus infection: an extended case study

Reticuloendotheliosis virus (REV) is a species of the genus Gammaretrovirus that can cause neoplasia, immunosuppression, and runting-stunting syndrome. To show the clinical relevance of REV is complicated, and requires the demonstration of the virus, REV antibodies, the presence of typical gross and microscopic lesions, and the exclusion of other oncogenic agents in the case of the presence of tumours. Under field conditions, the first tests to be used might be a commercially available REV antibody ELISA or an RT-PCR to detect the REV genome. In this short paper, we present the experiences with two commercially available ELISAs and RT-PCR that we have gained from a REV outbreak on a large multi-age layer farm and many follow-up tests on samples from control farms with and without a known history of REV and fowlpox virus (FPV). In the field, some of the FPV field strains contain large inserts of the REV genome that might interfere with REV testing. The results of the ELISAs on sera from REV- and FPV- unsuspected flocks suggested that the cut-offs of both ELISAs were somewhat low resulting in a lower specificity. However, cut-offs of 2000 and 3050 for the IDEXX and BioChek ELISAs, respectively, gave an agreement of 100%, suggesting that these cut-offs might be advisable to use. The use of the combination of RT-PCR for REV and PCR for FPV proved to be very useful in separating REV infections from FPV infections. The results of our extended field study can help to interpret REV testing results.
