This study is being done to test a microdevice, which is a small device designed to test drugs directly on skin conditions like atopic dermatitis (eczema) and psoriasis.
The small device, about the size of a grain of rice, has up to 20 tiny reservoirs that hold medications that are approved by the Food and Drug Administration (FDA) for atopic dermatitis and psoriasis. Very small amounts of these medications will be released into the skin (at levels in your body much lower than are typically used). In this study, the device will be tested to see if it's safe and works well for predicting how the skin will react to standard treatments. We will also look at how these reactions are connected to genetic information and overall treatment results.

Efficacy and Safety of Crisaborole 2% Versus Tacrolimus 0.1% in Children With Mild to Moderate Atopic Dermatitis

This randomized, open-label, parallel-group study will compare the efficacy and safety of Crisaborole 2% ointment and Tacrolimus 0.1% ointment in children with mild to moderate atopic dermatitis. A total of 66 participants will be randomized (1:1) to receive either Crisaborole or Tacrolimus, applied twice daily to affected areas for 28 days. Primary endpoint is proportion of participants achieving Investigator's Static Global Assessment (ISGA) success (clear/almost clear with ≥2-point improvement from baseline) at Day 28. Safety assessments include adverse events and local application site reactions.

Treatment of discoid lupus erythematosus with crisaborole

PURPOSE:

Discoid lupus erythematosus (DLE) is a chronic autoimmune skin disorder that primarily affects sun-exposed areas and may lead to permanent scarring and dyspigmentation if inadequately treated. Therapeutic options for DLE remain limited, and long-term use of topical corticosteroids or calcineurin inhibitors is often constrained by adverse effects, particularly on facial skin. This report aims to describe the clinical response of facial DLE to topical crisaborole treatment.

MATERIALS AND METHODS:

We report the case of a 20-year-old woman with a 7-year history of a persistent erythematous plaque on the nose, previously misdiagnosed and refractory to multiple treatments, including systemic antimalarials and topical agents. Histopathological examination and direct immunofluorescence findings supported a diagnosis of DLE without systemic involvement. The patient was treated with topical crisaborole 2% ointment once daily in combination with hydroxychloroquine.

RESULTS:

Marked clinical improvement was observed after two months of treatment. Sustained remission was maintained during a seven-month follow-up period with continued crisaborole use and dose reduction of hydroxychloroquine, without recurrence or adverse effects.

CONCLUSIONS:

Crisaborole, a topical phosphodiesterase-4 inhibitor approved for atopic dermatitis, may represent a well-tolerated nonsteroidal therapeutic option for facial DLE, particularly in patients who are unresponsive to or intolerant of conventional treatments. Further studies are needed to clarify its efficacy and long-term safety in DLE.

Efficacy and safety of crisaborole ointment combined with 308-nm excimer laser in the treatment of non-segmental vitiligo: a randomized, intra-patient left-right controlled study protocol

BACKGROUND:

Non-segmental vitiligo is an autoimmune disorder causing melanocyte loss and depigmentation. The 308-nm excimer laser is effective but achieving sustained repigmentation is challenging. Crisaborole, a PDE-4 inhibitor, may synergize with laser therapy. This study evaluates crisaborole 2% ointment plus laser vs. laser monotherapy with vehicle using an intra-patient left-right controlled design.

METHODS:

This is a single-center, randomized, double-blind, intra-patient controlled trial. Eligible adults (18-65 years) with stable NSV and at least two comparable symmetrical lesions will be enrolled. Lesions will be randomly assigned to receive either crisaborole + 308-nm excimer laser or vehicle + 308-nm excimer laser. Laser treatment will be administered twice weekly, and ointments applied twice daily over 24 weeks. The primary endpoint is the proportion of lesions achieving ≥75% improvement in the Vitiligo Area Scoring Index (VASI/F-VASI) at the end of treatment. Secondary endpoints include mean percentage change in VASI/F-VASI, time to first repigmentation, recurrence at three months post-treatment, and patient-reported satisfaction. Adverse events will be monitored throughout the study.

CONCLUSION:

This trial aims to provide high-quality evidence regarding the potential synergistic effect of crisaborole combined with excimer laser therapy, potentially establishing a more effective and safe treatment option for NSV.

Natural PDE4 inhibitors from plants: A comprehensive review of discovery, structure-activity relationships, and clinical translation potential

Phosphodiesterase 4 (PDE4) is a validated target for chronic inflammatory diseases such as COPD, asthma, and psoriasis. Although four PDE4 inhibitors have received FDA approval (roflumilast, apremilast, crisaborole, and ensifentrine), all suffer dose-limiting gastrointestinal side effects due to insufficient PDE4B/PDE4D selectivity. Plant-derived natural products offer complementary chemical space for discovering inhibitors with novel binding modes and improved isoform selectivity. This review systematically surveys plant-derived PDE4 inhibitors reported from 1979 to 2024, spanning seven major scaffold classes: flavonoids (∼40%), terpenoids, coumarins, xanthones, benzofurans, fluorenones, and alkaloids. Structure-activity analyses reveal that hydroxylation patterns, C2C3 unsaturation, and B-ring substitution govern flavonoid potency. Structure-guided optimization of toddacoumalone ultimately yielded compound 33a (IC₅₀ = 3.1 nM, PDE4D), the fluorenone selaginpulvilin K (IC₅₀ = 11 nM, 30-909-fold family selectivity), and a semi-synthetic α-mangostin derivative (IC₅₀ = 17 nM). Despite promising potencies, critical gaps persist in subtype selectivity profiling, pharmacokinetic characterization, and metabolic stability. We propose that integrating AI-guided optimization, structure-informed isoform-selective design, nanodelivery systems, and biotechnology-enhanced production can accelerate clinical translation of these scaffolds.

Treatment There are more treatments for eczema than ever before, though standard treatments like regular moisturizing remain effective in managing eczema in children. The guidelines provide 26 evidence-based recommendations for topical therapies (prescription and non-prescription), phototherapy, and systemic therapies based on the treatment's reduction of eczema symptoms and itch severity, inclusive of the below. The guidelines find insufficient evidence or no benefit to dietary or environmental interventions, including early food introduction, human milk consumption, probiotic or vitamin D supplementation, water softening, and dust mite avoidance. Treatment There are more treatments for eczema than ever before, though standard treatments like regular moisturizing remain effective in managing eczema in children. The guidelines provide 26 evidence-based recommendations for topical therapies (prescription and non-prescription), phototherapy, and systemic therapies based on the treatment's reduction of eczema symptoms and itch severity, inclusive of the below. Strong recommendations reflect treatments in which benefits clearly outweigh the risks and burdens. These recommendations apply to most children with eczema in most circumstances. Strong recommendations were made for the use of the following: Moisturizers to reduce the severity of patients' dry, itchy skin. Topical calcineurin inhibitors (pimecrolimus 1% cream and tacrolimus 0.03% or 0.1% ointment) to manage patients' flares when their dry, itchy skin worsens and as intermittent maintenance therapy. Topical corticosteroids (steroid creams), considered a first-line treatment in most cases due to affordability and accessibility, to manage patients' flares and as maintenance therapy. Phosphodiesterate-4 inhibitors (crisaborole ointment and roflumilast cream) to reduce patients' itchiness and decrease the frequency of flares. When used proactively, roflumilast can keep skin clearer more consistently. Topical JAK inhibitors (ruxolitinib cream and tapinarof cream) to decrease the severity of the dry, itchy skin in patients with mild to moderate eczema. Monoclonal antibodies (dupilumab, tralokinumab, and lebrikizumab) to decrease the severity of eczema symptoms, reduce flares and improve itching in patients with moderate to severe eczema. JAK inhibitors (upadacitinib, abrocitinib, and baricitnib) to decrease the severity of eczema symptoms and improve itching in patients with moderate to severe eczema. Conditional recommendations were made for the use of treatments inclusive of the following: Bathing for treatment and maintenance for pediatric patients, followed by a moisturizer as standard care. Wet wrap therapy, under the guidance of a healthcare professional skilled in eczema management, are typically encouraged for patients during eczema flares. Phototherapy (light therapy) at a dermatologist's office can be used to treat patients with severe cases of eczema involving many body sites. Strong recommendations were made against systemic corticosteroids, which should be reserved exclusively for patients with sudden, severe flares and as short-term bridge therapy. Conditional recommendations were made against the use of topical antimicrobials and PUVA phototherapy for children with eczema. "These guidelines were developed to educate and empower patients, caregivers, and the medical community so children with eczema receive the best care possible. Early, proactive intervention allows improvement in symptoms and quality of life for patients and their families," said board-certified dermatologist Dawn Davis, MD, FAAD, co-chair of the AAD's Atopic Dermatitis Guideline Workgroup.
