The goal of this interventional comparative study is to assess the efficacy and safety of a novel regimen of preservative-free unit-dose topical anesthetics for controlling early postoperative pain following single-step transepithelial PRK surgery.
The main research questions to answer are:
Question 1: Are topical anesthetics effective in reducing post-PRK pain?
Question 2: Are short-term topical anesthetics toxic to the cornea?
Question 3: How to prevent abuse of topical anesthetics by patients at home?
Participants were planned to undergo a bilateral single-step transepithelial PRK surgery to correct their refractive errors and received a standard post-PRK treatment regimen for both eyes. Additionally, one eye was offered a preservative-free topical anesthetic as an experimental group and the other eye was offered a preservative-free artificial tear as a control group.

Optimizing injection accuracy for Baxter’s nerve entrapment: an ultrasonography-guided approach based on anatomical surface landmarks

INTRODUCTIONThe clinical feasibility and applicability of surface landmarks for Baxter nerve entrapment to proximal and distal sites is unclear. This study provides anatomical guidelines for optimal transducer placement using two specific landmarks, the most inferior tip of the medial malleolus (A) and the most protruding posterior tip of the calcaneus (B), to enhance the diagnostic and therapeutic injection efficacy for proximal and distal entrapment sites.MATERIALS AND METHODSEighty-six feet from 45 fresh cadavers (25 male and 20 female) were dissected to determine Baxter's nerve (BN) localization. With A and B as key landmarks, distances (OB, OBN, BBN, and BNx) were measured to accurately localize the nerve. The OB distance was divided into four equal-length quadrants or zones. Twenty feet from 10 fresh cadavers (5 males and 5 females) underwent ultrasonography-guided injection using in-plane and out-of-plane techniques. Two practitioners marked the landmarks to ensure reproducibility and an experienced anesthesiologist administered the injections. The spread of the injected dye was assessed, and statistical analyses were conducted.RESULTSAverage OB, OBN, BBN, and BNx distances were 50.55 ± 5.83 mm, 7.19 ± 5.85 mm, 43.77 ± 5.31 mm, and 12.13 ± 5.75 mm, respectively. Significant sex-related differences (O to B; B to BN) and notable disparities between the distances on the right and left sides (O to BN; BN to X-axis) were observed. Most of the BN (81.4%) was located in zone 1, representing 25% of the OB length. For entrapment site 1, the in-plane technique achieved a 100% success rate whereas the out-of-plane method achieved an 80% success rate. For entrapment site 2, the out-of-plane approach (90% success) outperformed the in-plane approach (20% success).CONCLUSIONUsing two specific landmarks provides reliable guidelines for optimal transducer placement during injections targeting compressive neuropathy at proximal and distal entrapment sites.

Trends of Buprenorphine Prescribing for Opioid Dependence Before and During the Early and Later Part of the COVID-19 Pandemic: A Study From a Large, Publicly Funded Opioid Agonist Treatment Service in India

OBJECTIVEThe COVID-19 pandemic has affected the availability of and access to medications for opioid dependence (OD). We examined the monthly trends in new buprenorphine/naloxone (BNX) treatment episodes, number of clinical visits for BNX, BNX dispensed per person, and BNX prescription over 56 months, which included the pre-pandemic period and the early and later parts of the pandemic (January 2017 to August 2022).METHODResearch data were collected from the pharmacy database of a large publicly funded treatment center in India. A flexible, low-threshold service was adopted in April 2020 in response to the lockdown implemented on March 25, 2020. Change point analyses were performed to examine monthly trends visually and statistically. We used autoregressive integrated moving averages to forecast trends from April to August 2020 and March to August 2022, using January 2017 to March 2020 and March 2020 to February 2022 as training data sets.RESULTSA total of 993 patients were started on BNX treatment; 40,452 BNX clinic attendances were made; 1,401,393 BNX tablets were dispensed; and 6,795 new patients with OD were registered. The observed data for clinic attendance for BNX was significantly lower than the projected estimates in April to August 2020; however, observed new treatment episodes and monthly BNX prescriptions were within the 95% projected estimates; BNX dispensed per person was significantly more than the projected estimate. In contrast, observed BNX prescription trends surpassed the upper limit of the 95% confidence interval in March to August 2022.CONCLUSIONSA low-threshold and flexible-treatment service could mitigate the unintended consequences of pandemic-induced restrictions.

Spring and parachute approach for piroxicam dissolution; its phenomenological model on the thermodynamics of irreversible processes

Even though the spring and parachute approach for poorly water-soluble drug candidates effectively improves their dissolution curves with eutectic mixtures, deep eutectic solvents, solid dispersion polymers, and solid solutions, we cannot consider that its enabling factor in these pharmaceutical modifications was enough to be clarified. Based on our previous study that oxybuprocaine acts as a role of parachute generator for piroxicam, the present study explored a small-molecule parachute generator and found that propranolol, a β-adrenergic-blocking drug, has a parachute effect on the supersaturated state of piroxicam. In addition, changing the concentration of tetracaine and dibucaine to 10 mM and 2.5 mM also showed a parachute effect. These parachute generators control piroxicam's dissolved state kinetically by making the supersaturated state of piroxicam a steady state. However, as the piroxicam anhydrous crystals diminished and the piroxicam monohydrate crystals grew due to Ostwald ripening, it led to attenuating the supersaturated state. This finding advances the elucidation of the mechanism of the parachute effect of polymers.
