Preventive and therapeutic effects of molecular iodine in a model of diabetes mellitus induced by streptozotocin

Diabetes mellitus (DM) is a multifactorial condition that involves oxidative alterations and dysbiosis of the gut microbiota associated with an imbalance in glucose metabolism. Therefore, the need to develop integrative therapies that are both effective and have fewer side effects has become evident in recent years. Molecular iodine (I₂) has antioxidant effects in preclinical hyperglycemic models. The present work analyzes the preventive and therapeutic effects of oral I₂ supplementation in a DM model induced by low doses of streptozotocin (STZ). Male CD1 mice (12 weeks old) were divided into the following groups: control, STZ (20 mg/kg/day, i.p., for 5 days), I₂ (0.2 mg/Kg in drinking water), preventive (STZ + I₂), and therapeutic (I₂ supplementation from day 35 to day 90; STZ + I_2(Ther)). The supplementation with I₂ prevented and normalized hyperglycemia, hypercholesterolemia, and hypertriglyceridemia associated with STZ, preserving pancreatic, liver, muscle, and adipose tissue morphology and normalizing inflammatory gene induction (TLR2, TLR4, NFkβ, TNFα) in several tissues. Furthermore, compared to the STZ group, the presence of I₂ favored a more significant abundance of beneficial bacteria that support the integrity of the intestinal epithelial barrier and higher α-diversity. In conclusion, the I₂ supplement has preventive and therapeutic effects, reducing oxidative damage and reestablishing microbiota diversity following STZ exposure.

Climate change undermines seafood micronutrient supply from wild-capture fisheries in Southeast Asia and Pacific Island countries

Marine ecosystem functions are affected by climate change impacts such as ocean warming, deoxygenation and acidification. These impacts drive changes in distributions and body size of fish species and directly affect fisheries. Wild-capture fisheries are crucial for providing nutrients, livelihoods, and employment in tropical Southeast Asia and Pacific Island countries, where coastal communities are highly vulnerable to climate change. We examined the impacts of climate change on fish stocks and nutrient availability of seven key micronutrients (calcium, Omega-3 fatty acids, iodine, iron, vitamin A, vitamin B12 and zinc) in Southeast Asia, Pacific Islands, and Oceania (Australia and New Zealand). We combined micronutrient demands by local human populations with Sea Around Us reconstructed catch time series and catch projections from a dynamic bioclimate envelope model for the 21st century. The model predicted a decline in the Maximum Catch Potential (MCP) within Exclusive Economic Zones for Oceania, Pacific Islands, and Southeast Asian countries. Under the 'strong mitigation' scenario, catch potential reductions ranged from a decline of 54-66 % in Oceania, 58-92 % in Pacific Islands, and 65-86 % in Southeast Asia by the mid to the end of the 21st century relative to the historical period, respectively. Under the 'no-mitigation' climate scenario, reductions were more severe, with a decline of 55-70 % in Oceania, 66-92 % in Pacific Islands, and 70-86 % in Southeast Asia for the same time periods. Our findings indicate that Australia and New Zealand are unlikely to meet the recommended nutrient intake demand for most micronutrients through their fisheries (not considering trade or aquaculture production), except for iodine and vitamin B12. Pacific Island countries will likely follow the same pattern while Southeast Asia is expected to face worsening deficits, except for iodine and vitamin B12. This study highlights the importance of incorporating nutritional considerations of seafood into national food, trade and economic policies.

Evaluation for primary hyperparathyroidism after radioactive iodine treatment in patients with Graves' disease

BACKGROUNDRetrospective studies are conflicting regarding the risk of primary hyperparathyroidism after radioactive iodine treatment. We hypothesized that primary hyperparathyroidism rates are greater after radioactive iodine than after thyroidectomy and that patients with hypercalcemia after treatment for Graves' disease are not adequately evaluated for primary hyperparathyroidism, contributing to underdiagnosis of radioactive iodine-associated primary hyperparathyroidism.METHODSThis retrospective review considers patients undergoing radioactive iodine or thyroidectomy for Graves' disease at a tertiary referral center between January 1, 2000, and January 31, 2022. Patients were identified using a hospital-based cohort discovery tool; exclusions included history of head/neck radiation, primary hyperparathyroidism/parathyroidectomy, renal dysfunction diagnoses, or treatment with both radioactive iodine and thyroidectomy. Patients with an elevated calcium (>10.2 mg/dL) level measured after treatment were classified as "incomplete workup" (no parathyroid hormone), "likely primary hyperparathyroidism" (parathyroid hormone >40 pg/dL), or "unlikely primary hyperparathyroidism" (parathyroid hormone <40 pg/dL).RESULTSOf 900 patients, 468 (52%) had been treated with radioactive iodine and 432 (48%) with thyroidectomy. At a median follow-up of 9.39 years (interquartile range, 5.12-13.25), 25% (n = 224) of patients did not have a serum calcium measured and 52 (8%, n = 676) patients had an elevated calcium level. Hypercalcemia was more common after radioactive iodine (10%) than thyroidectomy (6%, P = .061). Of patients with hypercalcemia, 33 (63%) were "incomplete workup," 5 (10%) were "likely primary hyperparathyroidism," and 14 (27%) were "unlikely primary hyperparathyroidism." There was no difference in primary hyperparathyroidism workup rates between patients treated with radioactive iodine (n = 23) and thyroidectomy (n = 10, P = .389).CONCLUSIONSPatients treated with radioactive iodine for Graves' disease may experience an elevated rate of hypercalcemia, but the majority are not adequately evaluated for primary hyperparathyroidism. Patients with a history of radioactive iodine should undergo regular calcium screening and, if elevated, appropriate workup for primary hyperparathyroidism.
