A Prospective Comparison of Isotretinoin and hCG Therapy in Males With Oligospermia and Azoospermia

This study compares two medical treatments to see how well they improve sperm production in men with infertility due to low sperm counts (oligospermia) or no sperm in the semen due to non-obstructive causes (nonobstructive azoospermia). Some men with these conditions have limited nonsurgical treatment options, and current therapies do not work for everyone.
Participants in this study will be adult men ages 19 to 50 who have been diagnosed with oligospermia or nonobstructive azoospermia and do not have a correctable cause for infertility. Eligible participants will be randomly assigned to receive one of two treatments for three months: oral isotretinoin taken twice daily, or human chorionic gonadotropin (hCG) injections given three times per week. hCG is a standard hormonal therapy used in certain types of male infertility, while isotretinoin is being studied for its potential role in stimulating sperm production.
At the start of the study, participants will have blood tests to measure reproductive hormone levels and a semen analysis. Blood tests will be repeated at one month and three months, and a repeat semen analysis will be performed at three months, which corresponds to a full cycle of sperm development. The main goal of the study is to determine whether motile (moving) sperm appear in the semen after treatment. Additional goals include measuring changes in sperm count and hormone levels, as well as monitoring medication side effects.
Both study medications are FDA-approved and have known side effect profiles. Participants may or may not experience improvement in sperm production. The results of this study may help guide future treatment options for men with infertility due to low or absent sperm production.

Experts’ view on the management of scalp seborrheic dermatitis in Italy

BACKGROUND:

Acne vulgaris is a common skin disorder that negatively affects adolescents' quality of life. Recent evidence suggests that combining isotretinoin with desloratadine may enhance treatment outcomes.

OBJECTIVES AND METHODS:

This study aimed to develop a practical algorithm for SSD management in Italy, by gathering insights from Italian dermatology experts on diagnosis, treatment and long-term management of SSD.

RESULTS:

According to literature review and clinical experience, accurate diagnosis of SSD requires medical history, clinical evaluation, disease severity assessment and trichoscopy. The differentiation of SSD from psoriasis, eczema, and tinea capitis is essential to guide appropriate treatment, which should counteract the main pathogenic mechanisms underlying the disease and be tailored to the severity of clinical manifestations. Topical antifungals are the first-line treatments due to their efficacy in reducing Malassezia colonization. The use of topical anti-inflammatory agents, including corticosteroids, is useful for moderate-to-severe cases, but should be limited due to potential adverse effects. Selenium disulfide may be a useful option for both acute symptom control and long-term maintenance because of its antifungal, sebostatic, keratolytic, and microbiome-restoring properties, associated with a high degree of patient satisfaction. Systemic antifungals may be considered in refractory cases.

CONCLUSION:

This experts' view provides a structured approach to SSD management in Italy, integrating clinical experience and scientific evidence.

Sociodemographic, clinical, and behavioral determinants of treatment adherence in acne vulgaris: a multicenter study

BACKGROUND:

This multicenter study aimed to evaluate treatment adherence and satisfaction among patients with acne vulgaris.

METHODS:

A total of 2,349 patients from 18 dermatology centers across Türkiye completed structured questionnaires.

RESULTS:

The participants were predominantly female (79.4%) with a mean age of 21.6 years. Overall, 48% reported consistent adherence to therapy, with significantly lower adherence among males (p < 0.001). The most frequent reasons for nonadherence were forgetfulness (28.7%), dislike of the treatment (26.4%), and reluctance to maintain long-term therapy (21.5%). Treatment adherence was positively associated with higher educational levels (p = 0.006) and greater acne severity (p < 0.001). Logistic regression revealed that systemic oral therapy (OR = 4.37, 95% CI: 3.147-6.086; p < 0.001) and oral isotretinoin (OR = 4.81, 95% CI: 3.379-6.869; p < 0.001) significantly increased adherence compared to topical therapy. Oral treatment was also independently associated with greater satisfaction (OR = 2.33, 95% CI: 1.767-3.095; p < 0.001). Moreover, mild acne severity (OR = 1.67, 95% CI: 1.216-2.297; p = 0.002) and older age (per year; OR = 1.05, 95% CI: 1.020-1.088; p = 0.001) predicted higher satisfaction.

CONCLUSION:

Systemic therapies-particularly oral isotretinoin-were associated with improved adherence and satisfaction.

Chemical identification of the Viola phillipina extracts and their anti-acne activity: A comparison with isotretinoin and salicylic acid

Since acne is a multifactorial condition, its treatment remains challenging. The multi-target efficacy of plant extracts may be a better way to treat it. In this paper, the components of Viola philippica extracts with different polarity were analyzed by HPLC, and the anti-acne activities were studied from sebum inhibition, anti-inflammatory and bacteriostasis. Results revealed that 6,7-Dihydroxycoumarin was enriched in dichloromethane (DE，308.125 ± 0.151 μg / mg) and ethyl acetate (EAE，344.748 ± 0.071 μg / mg) extracts, while cichoriin accumulated in ethanol (EEP，56.494 ± 0.019 μg / mg) and n-butanol (BE，42.888 ± 0.069 μg / mg) extracts. In androgen-stimulated SZ95 cells, the extract reduced lipid accumulation via Androgen Receptor (AR) downregulation. In Cutibacterium acnes(C. acnes),-stimulated HaCaT cells, it suppressed tumor necrosis factor-α (TNF-α) and Interleukin-8(IL-8) secretion by lowering Toll-like receptor 2 (TLR2). Both DE and EAE demonstrated significant sebum-inhibiting and anti-inflammatory activities within safe concentrations, along with strong antibacterial effects against Staphylococcus epidermidis (S. epidermidis) and C. acnes-surpassing isotretinoin in efficacy. EAE showed exceptional sensitivity toward S. epidermidis, with a Minimum Inhibitory Concentration (MIC) of 0.391 mg/mL. Its Minimum Bactericidal Concentration (MBC) was 0.780 mg/mL. Spearman correlation confirmed that anti-acne activity was positively correlated with synergistic effects of caffeic acid and 6,7-dihydroxycoumarin. In summary, Viola philippica extract exhibits promising multi-target anti-acne potential, supporting its development as a novel multifunctional treatment strategy.

Viatris recalls extended-release Xanax over dissolution test failure

Matt Molloy/Getty Images/iStockphoto Viatris, which Pfizer created in 2020, voluntarily withdrew extended-release products made at a plant in Ireland after an analysis revealed an issue that could affect bioavailability. Viatris has recalled one batch of the anti-anxiety medicine Xanax XR in the U.S. because it failed a test of the rate and extent of drug release. The company voluntarily initiated the recall after a batch of bottles containing 60 3-mg tablets of the drug failed dissolution specifications. Drugmakers run dissolution tests to assess the conversion of oral dosage forms into solutions. That conversion determines the bioavailability of the active ingredient and thereby affects the efficacy of the drug product. Viatris found that the dissolution of a batch of extended-release Xanax manufactured in Ireland diverged from the specifications, triggering the Class II recall. The news comes four years after Viatris pulled 110 bottles of Xanax XR from the U.S. market because of out-of-specification results for dissolution. Pfizer, which used to own the drug, recalled 36,000 bottles of immediate-release Xanax in 2012 over dissolution test results. The FDA approved Xanax XR in 2003, months before Pfizer acquired the asset as part of its $60 billion takeover of Pharmacia. An immediate-release formulation of the active ingredient, the benzodiazepine alprazolam, was already available. However, the older product needed to be taken three to four times a day. Xanax XR is taken once daily. Alprazolam was off patent and facing generic competition when the FDA approved Xanax XR. Supported by the extended-release formulation, Pfizer grew sales of its Xanax franchise to $350 million in 2008. By then, generic extended-release alprazolam products were competing for market share. Actavis launched a generic copy of the drug in 2007. Impax Laboratories, which is now part of Amneal Pharmaceuticals, entered the market months after Actavis. Aurobindo Pharma’s generic won FDA approval in 2011. Xanax XR was part of Pfizer until the drugmaker merged its Upjohn business with Mylan to form Viatris in 2020. Viatris reported $139.9 million in Xanax sales last year. Sales have slipped in recent years, falling from $154.8 million in 2023 to $145 million in 2024. Teva recalled isotretinoin in January, but the FDA only classified the action this month. Teva’s three other recalls stemmed from the use of an unapproved raw material. The FDA listed Actavis Laboratories as the manufacturer of the recalled batches of Teva’s clonidine transdermal system.
