Preventing Malaria in School Children to Protect the Whole Community in Rural Blantyre District, Malawi

This is an individually randomized, controlled, single blind three arm clinical trial of malaria chemoprevention strategies Arm 1: Intermittent preventive treatment with dihydroartemisinin-piperaquine (IPT-DP). Arm 2: Intermittent preventive treatment with sulfadoxine-pyrimethamine (SP) plus amodiaquine (AQ) (IPT-SPAQ). Arm 3: Control - students will receive standard of care (no preventive treatment). Outcomes include P. falciparum infection and parasite density, anemia, cognitive function and educational testing, as well as infection prevalence and disease incidence in young children sleeping student's households to assess the impact on transmission.

Multi-omics network pharmacology and computational validation reveal amodiaquine and desethylamodiaquine as apoptosis-regulating multi-target candidates in colon adenocarcinoma

One of the biggest causes of cancer-related mortality worldwide is still colon adenocarcinoma (COAD). Therefore, it is essential to investigate new therapy strategies. This study presents a multi-scale, hypothesis-generating framework that integrates network pharmacology with multi-omics data and quantum chemical analyses to systematically explore the repositioning potential of amodiaquine and desethylamodiaquine in colon adenocarcinoma. Eleven gene targets that were found to be shared across medication, gene expression, and illness databases included several computationally significant central hub genes with high network centrality, including BTK, SIGMAR1, SYK, and KCNH2. Amodiaquine and Desethylamodiaquine both showed high binding scores (-7.9 to -12.6 kcal·mol⁻¹) across all the selected proteins. The protein-ligand complexes' structural stability was confirmed by molecular dynamic simulations lasting more than 100 ns; RMSD, RMSF, and radius of gyration analyses collectively indicate that the protein-ligand complexes maintain structural stability, compactness, and limited conformational fluctuation throughout the simulation, reflecting preserved protein integrity rather than direct binding strength. While density functional theory investigations indicated stable geometries with high electronic polarizability, MM-PBSA calculations yielded binding free energies within a range of ΔG_bind = -59.0 to -67.0 kcal·mol⁻¹ . These results suggest that amodiaquine derivatives have anti-COAD actions due to their disruption of important immune-regulatory and apoptotic pathways. These findings computationally prioritize amodiaquine and desethylamodiaquine as candidate multi-target interactors in colon adenocarcinoma, warranting further experimental investigation rather than implying established therapeutic efficacy.

Spatial and temporal description of antimalarial drug resistance markers in Ghana using targeted amplicon deep sequencing

ABSTRACT:

Molecular markers give evidence of Plasmodium falciparum resistance to antimalarials. We determined the spatial and temporal distribution of antimalarial drug resistance markers in the P. falciparum population in Ghana using targeted amplicon deep sequencing. We contextualized our findings within the framework that duration of drug exposure and transmission intensity drive trends in molecular markers. About 87% (901/1,037) of samples collected from children ≤9 years with uncomplicated malaria from 2018 to 2023 were sequenced. There was a decreasing trend in the amodiaquine/lumefantrine pfmdr1 N F SND haplotype in the Coastal zone. For chloroquine, the pfcrt CV IET haplotype showed a decreasing trend in all zones. For sulfadoxine-pyrimethamine, the pfdhps S G KAA haplotype showed increasing trends in all zones in contrast with the pfdhpsAG KAA, pfdhfrIRN , and quintuple IRN + AG KAA haplotypes. Finally, validated markers of artemisinin resistance P441L (1/709), M476I (1/709), N537I (1/709), A481V (1/709), P574L (1/709), C469Y (2/709), P553L (2/709), R561H (2/709), A578S (4/709), and A675V (5/709) were identified. Zonal differences in trends for pfmdr1 N F SND haplotype are likely due to lower amodiaquine drug pressure in the Coastal zone. Policy review of amodiaquine/lumefantrine antimalarial use might help reduce the increasing trends seen in the Forest and Savannah zones. The declining prevalence of the pfcrt mutant CV IET after two decades of chloroquine disuse in Ghana suggests the return of chloroquine-sensitive parasites. Therapeutic efficacy studies must be done to verify this observation. The opposing trends of sulfadoxine-pyrimethamine molecular markers highlight concern for its use in malaria prophylaxis in pregnancy and children. Molecular surveillance remains vital to mitigating the risk of artemisinin-resistant parasites evolving in malaria-endemic regions.

Abstract:

Background and Aims:

Histamine signalling is critical in heart failure (HF). Histamine N-methyltransferase (HNMT) serves as the primary enzyme responsible for histamine clearance in heart, but its role in HF remains undefined. This study investigates HNMT-mediated metabolic-epigenetic crosstalk in HF and evaluates therapeutic strategies targeting this axis.

Methods:

Cardiomyocyte-specific Hnmt-knockout and Hnmt-overexpression mice were generated. Transverse aortic constriction (TAC) was applied to induce HF in mice. RNA sequencing and targeted metabolomics were employed to explore mechanisms underlying the pathological role of HNMT. Urinary N-methylhistamine levels were measured in a cohort of 53 HF and 55 control individuals.

Results:

HNMT was up-regulated in cardiac tissues from HF patients, TAC-operated mice, and phenylephrine (PE)-treated neonatal mouse cardiomyocytes (NMCMs). Urinary N-methylhistamine levels were elevated in HF patients and positively correlated with severity of HF. In mice, cardiomyocyte-specific Hnmt deletion mitigated hypertrophy and HF induced by TAC or angiotensin II. Pharmacological HNMT inhibition with amodiaquine ameliorated TAC-induced cardiac dysfunction. Conversely, Hnmt overexpression impaired cardiac function. Mechanistically, HNMT reduced intracellular S-adenosylmethionine (SAM) and impaired enhancer of zeste homologue 2 (EZH2) function, decreasing histone 3 lysine 27 trimethylation (H3K27me3) modification at the frizzled-2 (Fzd2) promoter. FZD2 up-regulation activated the WNT/calcium/calmodulin-dependent protein kinase II (CaMKII) pathway, promoting HF.

Conclusions:

HNMT exacerbates pathological hypertrophy and HF through SAM/FZD2/CaMKII axis. Amodiaquine, the inhibitor of HNMT, shows therapeutic potential against HF. Urinary N-methylhistamine emerges as a non-invasive HF biomarker.

Fosun Pharma's Self-developed Artemisinin Medicines Inject New Impetus to Malaria Prevention and Treatment in Africa

SHANGHAI, April 26, 2024 /PRNewswire/ -- World Malaria Day is marked each year on April 25. World Health Organization (WHO) gave as the theme for World Malaria Day 2024 Accelerating the fight against malaria for a more equitable world. WHO stated that malaria not only continues to directly endanger health and cost lives, but it also perpetuates a vicious cycle of inequity. People living in the most vulnerable situations including pregnant women and children under 5 years of age continue to be disproportionately impacted. Artemisinin medicines developed with China's scientific research efforts have become a ticket for China's innovative medicines to go global. According to the WHO World Malaria Report 2023, globally in 2022, there were an estimated 249 million malaria cases and 608,000 malaria deaths. Sub-Saharan Africa accounted for more than 95% of the global malaria cases and deaths. Globally, an estimated 11.7 million malaria deaths were averted in the period 2000-2022. Among them, the average malaria mortality rate (number of deaths per 100,000 population at risk) in Africa decreased from 0.14% per 100,000 population to 0.055%, from 142.6 per 100,000 population in 2020 to 55.5 in 2022. The widespread use of artemisinin medicines is one of the key success factors. Several global multi-center phase III clinical studies and real-world data have shown that artesunate injection is effective in reducing malaria mortality rate. Strides African unit nabs WHO first with nod to produce malaria treatment

Global malaria cases have been on the rise in recent years, according to the World Health Organization. Universal Corporation Limited, a unit of Strides Pharma Science, became the first African drug manufacturer to snare a World Health Organization (WHO) pre-qualification (PDF) to produce the anti-malarial drug sulfadoxine-pyrimethamine and amodiaquine (SPAQ). The treatment is a combination regimen used to prevent malaria in children living in areas with seasonal malaria transmission. The WHO's pre-qualification program is designed to set standards of quality, safety and efficacy in manufacturing medicines. With the designation, Kenya-based UCL will look to boost regional production and access to essential medicines on the African continent, where about 95% of all malaria cases and 96% of deaths from malaria occur, the company said in a Nov. 6 press release. “Universal Corporation is committed to advancing the fight against malaria and improving the health and well-being of communities across Africa,” Perviz Dhanani, founder and managing director of UCL, said in the release. “The WHO prequalification of our SPAQ production is a significant step forward in achieving these goals.” In 2022, WHO began to recommend SPAQ for any child at high risk of severe malaria in areas with seasonal malaria transmission. This recommendation resulted in more than 48 million children being protected from malaria, significantly reducing the burden of this deadly disease, the organization said. According to WHO, the reported cases of malaria hit 247 million in 2021, compared with 245 million in 2020 and 232 million in 2019. In an unrelated but recent move, Strides in September said it planned to spin off its CDMO and soft gelatin businesses into a single entity that will be known as OneSource. The goal of the spinoff, which is expected to happen in the next year, is to establish a specialty pharmaceutical CDMO company that offers services for biologics, oral softgels, complex injectables, sterile injectables and other complex drug delivery systems.
