This study evaluates critical environmental limits (temperature and humidity) above which older adults are unable to effectively thermoregulate. Participants will exercise in a series of different environmental conditions to identify combinations of temperature and humidity above which age-related physiological changes cause uncompensable heat stress, resulting in increased risk of heat illness.

A Prospective, Multicentre, Randomized, Open Label, Blinded Endpoint, Phase 3 Trial to Assess the Safety and Efficacy of Prophylactic TicagrelOr With Acetylsalicylic Acid Versus CLopidogrel With Acetylsalicylic Acid in the Development of Cerebrovascular EMbolic Events During Transcatheter Aortic Valve Implantation (TAVI) OperationS - the PTOLEMAIOS Study.

Ticagrelor administered with Acetylsalicylic Acid (ASA) will provide better cerebral protection from microembolization in the cerebral circulation during Transcatheter Aortic Valve Implantation (TAVI) and 30 days afterwards, than Clopidogrel plus ASA. This hypothesis will be investigated by measuring the number of High Intensity Transient Signals (HITS) as assessed with transcranial Doppler (TCD) on middle cerebral arteries.

Melatonin (MT), an indole compound, can boost plant growth under abiotic stress conditions. This experiment aims to elucidate the synergistic effect of MT and ascorbic acid (AsA) in mitigating salinity stress by assessing the photosynthetic and antioxidant capacity of the maize inbred lines H123 and W961. The results indicated that exogenous MT and AsA significantly improved photosynthetic efficiency and biomass of maize under salinity stress. Additionally, exogenous MT and AsA also improved antioxidant enzyme activities, promoted regeneration of AsA and GSH, decreased reactive oxygen species contents, suppressed Na⁺ accumulation, and improved the K⁺/Na⁺ ratio of maize seedlings. Additionally, the AsA inhibitor lycorine decreased the endogenous content of AsA and eliminated the positive effects of MT, while the MT inhibitor p-chlorophenyl alanine (CPA) reduced the endogenous content of MT, which could not eliminate the promoting effects of AsA. The results suggested that AsA may act as a downstream signal involved in the regulatory effects of MT on maize under salinity stress.

Comprehensive Evaluation of the Ability of Comorbidity and Health Status Indices to Improve the Prediction of Perioperative Morbidity and Long-Term Survival Outcomes After Radical Cystectomy

BACKGROUNDIn the era of standardized outcome reporting, it remains unclear if widely used comorbidity and health status indices can enhance predictive accuracy for morbidity and long-term survival outcomes after radical cystectomy (RC).PATIENTS AND METHODSIn this monocentric study, we included 468 patients undergoing open RC with pelvic lymph node dissection for bladder cancer between January 2009 and December 2017. Postoperative complications were meticulously assessed according to the EAU guideline criteria for standardized outcome reporting. Multivariable regression models were fitted to evaluate the ability of ASA physical status (ASA PS), Charlson comorbidity index (± age-adjustment) and the combination of both to improve prediction of (A) 30-day morbidity key estimates (major complications, readmission, and cumulative morbidity as measured by the Comprehensive Complication index [CCI]) and (B) secondary mortality endpoints (overall [OM], cancer-specific [CSM], and other-cause mortality [OCM]).RESULTSOverall, 465 (99%) and 52 (11%) patients experienced 30-day complications and major complications (Clavien-Dindo grade ≥IIIb), respectively. Thirty-seven (7.9%) were readmitted within 30 days after discharge. Comorbidity and health status indices did not improve the predictive accuracy for 30-day major complications and 30-day readmission of a reference model but were associated with 30-day CCI (all P < .05). When ASA PS and age-adjusted Charlson index were combined, ASA PS was no longer associated with 30-day CCI (P = .1). At a median follow-up of 56 months (IQR 37-86), OM, CSM, and 90-day mortality were 37%, 24%, and 2.9%, respectively. Both Charlson and age-adjusted Charlson index accurately predicted OCM (all P < .001) and OM (all P ≤ .002) but not CSM (all P ≥ .4) and 90-day mortality (all P > .05). ASA PS was not associated with oncologic outcomes (all P ≥ .05).CONCLUSIONWhile comorbidity and health status indices have a role in predicting OCM and OM after RC, their importance in predicting postoperative morbidity is limited. Especially ASA PS performed poorly. This highlights the need for procedure-specific comorbidity assessment rather than generic indices.

AbstractIntroductionThere is an expectation that healthcare professionals display competence in teaching, assessment and providing feedback. Development begins with formative peer‐assisted learning and teaching in the undergraduate environment. Using peers or near‐peers (in this case having 1 year more experience than the examination cohort) to provide assessment in summative exams remains unexplored. This study investigates how the use of near‐peers compares to marking by academic staff in a summative OSCE.Materials and MethodsBDS4 Peer assessors (PAs) developed an OSCE question and marking schedule. Each PA (n = 3) was paired with an academic staff assessor (ASA) (n = 3). Peer and academic marked the candidates independently. Two years later, the process was repeated on the same cohort of candidates with the PA now 1‐year post qualification. Statistical analysis compared the scores awarded by PA during each timeframe and against the marks awarded by the ASA.ResultsDuring round 1, 28 students (62.2%) were awarded the same score by PA and ASA. On 17 occasions, there was a discrepancy (37.8%). Bias was skewed in favour of PA scoring higher (mean difference of differences −0.0667). During round 2, 27 students (55.1%) were awarded the same score by PA and ASA. On 22 occasions (44.9%), there was a discrepancy. Bias was skewed in favour of ASA scoring higher (mean difference of differences 0.0612).DiscussionLevels of agreement between PA and ASA are strong. Our results show PA mark more leniently as undergraduates and less leniently at 1‐year post graduation.ConclusionsPeer assessors are able to write OSCE stations, produce marking schemes and effectively assess their near‐peers.
